docs(changelog): record 0.4.41 live validation results

Replaces the pre-release "Pending live checks" checklist in the 0.4.41 CHANGELOG section with what actually happened. The checks ran on chinchill's customer-demo Slack app on 2026-10-01:

• [4.41/SL1] Slack outbound mentions: code/URL-aware resolution, native-stream path #206 and [4.41/SL3] Slack: rotate long native streams before expiry #208 verified live.
• Chinchill git-ref smoke test done.
• Teams 2.1 not live-verified (no Teams tenant; suite-verified on 2.0.13.4, 2.0.16 and 2.1.0).
• [4.41/SL10] Slack Agent Sessions lifecycle + native stop #215 Agent Sessions native Stop not live-verified (opt-in; chinchill doesn't enable agent_view).
• Notes the msg_too_long gap, tracked in Slack native streams: rotate before Slack's message-length limit and recover from msg_too_long #310 (upstream has the same gap).
