refactor: unwrap SessionEntry namespace + self-reexport - #22977

Unwrap `export namespace SessionEntry` in packages/opencode/src/v2/session-entry.ts into flat top-level exports + self-reexport for tree-shaking and Node native TS runner compatibility. Public API preserved via `export * as SessionEntry from "./session-entry"`. No consumer imports change. Verified: tsgo --noEmit clean, SDK generate produces no drift, relevant tests pass.
