1. PR Lifecycle Hygiene Investigator

Trigger: Scheduled (daily or weekly) + workflow_dispatch
Purpose: Detect long-lived draft PRs, long-lived blocked PRs (for example merge conflicts), and open one deduplicated tracking issue with prioritized follow-ups so maintainers can batch-triage PR debt.
Proposed safe outputs: create-issue (single aggregated report), noop when no actionable PR hygiene debt is found.
