The MCP Gateway container image reference used in compiled workflows (DIFC_PROXY_IMAGE) should be bumped to v0.2.16.

Current: ghcr.io/github/gh-aw-mcpg:v0.2.12
Target: ghcr.io/github/gh-aw-mcpg:v0.2.16

This picks up the string-only OTLP headers change (gh-aw-mcpg#3337) and other recent fixes.
