[deep-report] Agent Performance Analyzer still creating duplicate discussions (1-second apart) #31040

Description

Problem

The Agent Performance Analyzer continues to create two near-identical discussions on each run, despite this being flagged in yesterday's DeepReport (2026-05-07).

Observed cases:

The pair fires within 1 second; one of the two has an empty body (787 chars vs 12,028 chars), suggesting the safe-output create_discussion is being invoked twice with one truncated payload.

Suggested Fix

In .github/workflows/agent-performance-analyzer.md (or its compiled lock file), audit the prompt for two create_discussion invocations and remove the duplicate. Alternatively, add a close-older-discussions policy keyed on the weekly title prefix so the second discussion is auto-replaced rather than co-existing.

Also consider an idempotency guard in the safe-output handler that rejects a second create_discussion with the same exact title within the same workflow run.

Suggested Agent

agentic-workflows — single workflow .md edit plus recompile.

Estimated Effort

< 1 hour.
