Thanks for the report. #6980 fixes part of this. Here is what we found and what we still need from you.

What your screen means. "Installed but unhealthy" on the Desktop row is not a broken proxy. OpenCodex Desktop is running the proxy, but Startup safety could not verify its Start at Login registration. While the app runs the proxy, the dashboard deliberately disables the background service and launcher shim installers. A shim launch could otherwise start a second CLI proxy while Desktop is restarting its own.

    Fixed in #6980

    • The Desktop row now says Running · Start at Login not verified. The guidance explains that the tray checkbox only shows that the login item file exists, and that macOS can still block it under System Settings → General → Login Items & Extensions.
    • Dashboard Install/Repair failed whenever the dashboard was served by a compiled ocx, such as the Desktop app's bundled one or the Linux deb. The installer child got an unknown argument. That is fixed.
    • ocx codex-shim install now shows the real reason when it refuses an fnm temporary path, instead of "Native Codex launcher not found".

Tracked separately: allowing the shim while Desktop runs the proxy (#6971), the tray checkbox vs launchd state (#6972), and finding Codex when the only install is the Codex desktop app (#6973).

    Workarounds for now

    • Keep the Desktop app and make Start at Login stick. Check it in the OpenCodex menu. If it is already checked, open System Settings → General → Login Items & Extensions and allow OpenCodex.
    • Or move startup to the CLI: quit OpenCodex, then run ocx codex-shim install (or ocx service install) in Terminal.

Could you answer these? They decide which of the follow-ups is your actual blocker.

    1. OpenCodex version (OpenCodex menu, or /Applications/OpenCodex.app/Contents/MacOS/ocx --version), and where OpenCodex.app is installed.
    2. How Codex is installed, plus the output of which -a codex and codex --version in Terminal: Homebrew, npm/bun global, nvm/fnm/volta/mise, or only the Codex desktop app (ChatGPT.app)?
    3. Where you see "doesn't pick up codex", with the exact message or a screenshot (dashboard page, Codex app, or terminal).
    4. The exact output of ocx codex-shim install when you ran it from Terminal, and which ocx that was (which -a ocx).
    5. Is Start at Login checked in the OpenCodex menu? Please also send the output of ls -l ~/Library/LaunchAgents/OpenCodex.plist and launchctl print-disabled gui/$(id -u) | grep -i opencodex.

Please redact anything personal from command output.

Two pieces of this report are addressed in the 2.84.0 train:

If Codex is still not detected after updating, please share ocx doctor output (it redacts paths).

The remaining part of this report, installing or repairing the Codex launcher shim from the dashboard while OpenCodex Desktop runs the proxy, is fixed by #7032 (merged into dev as 148cfef, not yet released). The dashboard installer fix and the Desktop Start at Login wording landed earlier in #6980, and ChatGPT.app Codex discovery in #6973. Closing. Please reopen with ocx doctor output if the shim still can't be installed after updating.
