Update the owned Responses transport contract
This changes the downstream SSE contract from a generic retryable upstream_server_error to a terminal upstream refusal code, but structure/transports/responses.md:652-658 still describes bare-error handling only as preserving the message and synthesizing response.failed; it does not record refusal-code preservation or the new precedence over a subsequent read failure. Update that owned transport section in the same change so future relay work preserves the new terminal semantics.

Honor the first structured code before refusal classification.
Lines 252-254 search all candidate codes and select any refusal code. A root code: "upstream_reset" with a nested historical error.code: "invalid_prompt" therefore becomes non-retryable.
Select the first structured code in candidate order. Classify only that code. Use message classification only when no structured code exists.

Latch a captured refusal across later error events.
Lines 303-307 overwrite both fields for every bare error event. If a refusal is followed by an ordinary diagnostic error and then a read failure, the ordinary error clears upstreamRefusalCode. The catch path then emits retryable upstream_reset framing and restores the reconnect loop.
Do not replace a captured refusal with a later non-refusal error. Add a regression case for refusal → ordinary error → read failure.

A safety block interrupting a native turn reached Codex App as repeated "Reconnecting N/5" instead of a terminal error (#5176). Trace the path before changing it. Codex classifies a response.failed terminal by error.code alone (codex-rs/codex-api/src/sse/responses.rs:417-467): context_length_exceeded, insufficient_quota, usage_not_included, cyber_policy, misalignment_policy_violation, invalid_prompt and bio_policy end the turn, and every other code falls to the trailing _ => ApiError::Retryable arm, which is retried up to stream_max_retries. Codex has no handler for a bare SSE "error" event, so the terminal this proxy synthesizes is the only carrier of the refusal. relay.ts upstreamErrorTailFrame stamps upstream_server_error on that synthesized terminal unconditionally, discarding whatever code the upstream sent. The message survives, which is why the client quotes the refusal text verbatim while still treating the turn as a retryable disconnect, and why all 44 sampled requests logged 502 / upstream_server_error. These cases assert the mapping as it currently stands, for both the tee and eager relays and both the flat and nested bare-error shapes. Refs #5176
A safety block interrupting a native turn reached Codex App as repeated "Reconnecting N/5" instead of a terminal error (#5176). The preceding commit traced why: relay.ts upstreamErrorTailFrame stamped upstream_server_error on the terminal it synthesizes from a bare upstream "error" event, discarding whatever verdict the upstream sent. Codex classifies that terminal by error.code alone (codex-rs/codex-api/src/sse/responses.rs:423-462) and treats everything outside its fatal set as retryable, so the refusal text was shown while the turn was still treated as a recoverable disconnect. The synthesized terminal now carries the upstream verdict, in both the tee and eager relays: - A structured code decides on its own. A code that is not a refusal means the upstream did not refuse, whatever its diagnostic text quotes, so a transport error echoing refusal copy stays retryable. - Recognized refusal copy is read only when no code was carried anywhere on the event, which is the shape the field report shows. - Code and message are read from the same candidate topology, so a refusal nested under response.error is no longer half-seen. - A refusal code sent without a message still ends the turn; Codex accepts that shape and supplies its own copy, and this proxy now supplies one too. - A read that fails after a refusal was already captured reports the refusal rather than a generic upstream_reset, because the upstream ended the turn before the socket did. This is a classification change, not a broadened terminal. The refusal set is the four content refusals Codex ends a turn on (cyber_policy, misalignment_policy_violation, invalid_prompt, bio_policy); insufficient_quota and context_length_exceeded are left to the quota and context paths that already own them. Message matching uses whole distinctive phrases taken from upstream copy, for the same reason isCyberPolicyMessage does: a loose match would be the mirror-image defect, stranding a turn a reconnect would have finished. Request-log accounting is deliberately unchanged. A row that ends on a refusal still records the transport-level status, because making the log follow requires the terminal-detection path to treat a bare refusal as a failed terminal, and classifying it in the log alone lets an early bare error latch a verdict a later real terminal should replace. Regressions cover both relays: a refusal ends the turn, a non-refusal code wins over refusal copy in its message, a nested refusal is seen, a code-only refusal still terminates, a captured refusal survives a read failure, and a transient upstream failure keeps upstream_server_error and stays retryable. invalid_promptand "flagged for possible biological risk" withbio_policy. Pairing the reported "blocked by our safety systems" withinvalid_promptis an inference from those two, and the code comment says so rather than implying the upstream source states it.Deliberately out of scope: request-log accounting is unchanged.
