## Round-2 audit fold

- r3-R2-3 (Medium): the cross-layer contract paragraph now names the SD1' account-based identity (with `loginId`, without `authType`) and the SD2' `kiroAccountEvidence(account, now?)` signature. 020 still neither reads nor writes either.

## wp3 P re-verification (2026-09-27, branch `codex/kiro-lb2-020-transport` on dev `6581b561a7`, which contains 010)

Executable plan for the 020 build; **overrides** earlier sections where they conflict.
Architect proposal recorded in the task transcript (handle `01a0dddd`).

| ID | Disposition |
|---|---|
| D020-S1 | Accept: every Kiro source anchor re-verified exact on `6581b561a7`. |
| D020-S2–S5, S7 | Accept: `AdapterFetchContext` `src/adapters/base.ts:166-176`; Google test under `describe("vertex retry fetch")` `tests/adapters/google/google-vertex-http.test.ts:35`; Kiro row `structure/providers-and-adapters.md:106`; insert after `structure/providers/kiro.md:68`; registry lines after `scripts/test-layout/layout.json:1082` and `tests/fixtures/test-layout-expected.json:903`. |
| D020-S6 | Accept: the Kiro order stays *pacing, then budget reservation* (the pacing-refusal test depends on it); the doc sentence claiming `physical-send.ts` uses the same order is corrected — that file reserves first. |
| D020-G1 | **Decision: one executor, pinned by tests.** Every Kiro physical send uses `ctx.executor`. Its `dispatchOverride` rebuilds only when the account selection changed after build, and it rebuilds for the *current* account (`src/server/responses/request-transport.ts:408-479`), so credentials cannot mix; the cost is that an alternate-host or completion-fallback leg sent after such a switch becomes a canonical send of the rebuilt request. Devin's inner ladder already accepts the same contract. New tests in the 020 sibling file: `an account switch before the alternate leg sends the rebuilt request for the new account` and `an account switch before the completion fallback never sends the old account's bearer`, both driving a stub executor with a dispatch override. |

P1 Badge Move the unreleased credential analysis out of devlog

This added _plan entry publishes pre-release credential-mixing analysis, dispatch-override behavior, and an old-bearer test plan while the corresponding fix is still under review. Because devlog/ is tracked and public, move these working notes to .tmp/ or other scratch space and publish only the shipped outcome (under _fin/ if a retrospective is needed); the repository explicitly prohibits unreleased security findings and pre-disclosure patch plans in devlog/.

| D020-G2 | Accept: `structure/providers/kiro.md` states that reset, 429, alternate and fallback sends now wait for a pacing slot (only the first send is pre-paid), and that Kiro web-search turns are paced. |
| T1 | Accept: `tests/providers/kiro/kiro-retry.test.ts:50-78` "does not replay a per-attempt TimeoutError" now expects a resolved 504 with the fixed gateway-timeout text, keeping `calls === 1` and the timeout-reason assertions. |
| T2 | Already planned: `kiro-retry.test.ts:235-243` moves its 503 to 500. |
| Grep | No other test asserts Kiro HTTP 5xx text; non-Kiro 500 fixtures (`adapter-inner-send-budget-wiring`, Devin) are untouched. The 010 lesson applies: C also runs the four hosted-CI shards' worth of the suite in the clean worktree at `/tmp/ocx-kiro-verify` before claiming no regression. |

Verifier set for C: `bun run typecheck`; `bun test tests/providers/kiro/ tests/adapters/google/google-vertex-http.test.ts tests/adapters/adapter-inner-send-budget.test.ts tests/adapters/adapter-inner-send-budget-wiring.test.ts tests/adapters/credential-redirect-guard.test.ts` (paths confirmed at build time); layout, ratchet, lab-boundary tests; `bun run privacy:scan`; `bun run structure:check`; full `bun run test` in `/tmp/ocx-kiro-verify`.

### wp3 reflection fold (same architect: MISALIGNED → folded)

1. **G1 cost, stated in full.** After an account switch, the completion-fallback leg sends the
rebuilt *initial* request (URL and body) for the new account, not the fallback prompt
(`src/server/responses/request-transport.ts:477-479`); the alternate leg likewise becomes a
canonical send. `commitKeyAttemptSend` (427) and `recordKeyAttemptFailure` (438) now fire on
every Kiro inner send; this is accepted, because each inner send is a real physical send that
the key-attempt ledger should see. The two G1 tests assert the URL and body actually sent and
the bearer, and the fallback test asserts the client stream carries no duplicated output. The
earlier "Devin already accepts this" sentence is withdrawn: no evidence was found that Devin's
executor carries the dispatch override.
2. **Verifier set corrected:** `tests/lib/credential-redirect-guard.test.ts` (not
`tests/adapters/`), plus `tests/lib/upstream-retry.test.ts`,
`tests/server/server-kiro-completion-e2e.test.ts`, `tests/server/server-kiro-oauth-401-replay.test.ts`.

### wp3 A round 1 fold (reviewer 01a0de82: FAIL, 2 High → folded)

1. **No gateway rotation after output.** `fetchKiroWithRetry` gains an option
`allowGatewayRotation` (default `true`). The completion fallback in
`src/adapters/kiro/stream.ts` (~1049) passes `false` whenever the first stream has already
emitted client-visible progress; the 502/503/504 → `q.*` branch requires the option. Connection
failures and the existing 400/403/404/405 signature fallback keep their current behaviour. Test:
`a canonical completion fallback returning 503 after visible progress is not rotated` in the 020
sibling file (asserts one send and the fixed 5xx text).
2. **Alternate derived from the URL actually dispatched.** For the gateway branch, the alternate
is computed from `response.url` when it is a non-empty canonical runtime URL, else from the
planned URL; if `response.url` names a non-canonical host, no rotation happens. This follows a
dispatch-override rebuild into another region. Test:
`an account switch across regions before a 503 rotates to the new region's alternate`, using a
stub executor that returns a Response whose `url` is the rebuilt region's canonical host
(set with `Object.defineProperty`), asserting the second send targets that region's `q.*` host.

### wp3 A round 2 fold (finding 2 re-opened → folded)

The gateway-branch base URL is chosen in this order: `response.url` when non-empty; otherwise the
adapter request's `url` **re-read after the send** (`Object.assign(wireRequest, rebuilt)` at
`src/server/responses/request-transport.ts:471` mutates the same object Kiro received); only if
both are unavailable, the planned URL. Rotation happens only when that base is a canonical runtime
URL. Second test: `an account switch across regions rotates to the new region's alternate even
when the 503 Response has an empty url` — the stub executor mutates the request object's `url` to
the other region (as the override does) and returns `new Response(null, { status: 503 })`; the
assertion is that the second send targets the new region's `q.*` host.

## wp3 build notes

- Kiro physical sends now use the supplied provider executor and wait for pacing before budget admission. The completion fallback retains the executor; gateway rotation uses the actual dispatched canonical URL and is disabled after visible progress.
- Header deadlines return fixed HTTP 504 text, caller aborts retain their reason, and final HTTP 5xx client text and opt-in diagnostics exclude upstream message content. Shared retry and Google production code were unchanged.
- The synthetic account-switch tests model request mutation and the server's dispatch override rebuilding an initial request for the new bearer. They do not instantiate the full OAuth selection loop; the existing Kiro server integration tests remain in the verifier set.

Synchronize the Kiro sections in the remaining locale pages.

The Kiro sections in ja, ko, ru, and zh-cn only describe generic bounded retries. They omit the new canonical-endpoint fallback for HTTP 502/503/504 responses, the 504 header deadline, and fixed public text for final HTTP 5xx responses.

Update these four sections to match docs-site/src/content/docs/reference/adapters.md:335-344.

Remove the “sıradan” qualifier.

The English source states that all other service errors are not replayed. The Turkish text limits this statement to “ordinary other service errors,” which can imply that other service errors may be replayed. Remove the qualifier to keep the translation aligned with the canonical English documentation.

Map only the Kiro deadline to HTTP 504.

If executor.unpacedFetch ?? executor rejects a TimeoutError while the caller signal is not aborted, the wrapper rethrows that executor error. The outer catch then maps it to the fixed 504 response because it checks only error.name. This can hide a distinct executor failure and violates the Kiro contract, which limits this mapping to the header deadline.

In `@src/adapters/kiro-retry.ts` around lines 361 - 370, Update the Kiro
header-deadline handling so only a timeout originating from the header deadline
maps to HTTP 504. Mark that timeout distinctly where the wrapped fetch detects
the deadline, then have the outer catch map only that marker; rethrow executor
`TimeoutError` failures unchanged. Locate the changes in the fetch wrapper and
the retry loop’s catch block.
