Summary

When Kiro's safety filter refuses a turn that has no tools, opencodex returned a retryable 502 {"type":"overloaded_error","message":"Kiro returned a successful but empty response stream"}. On Claude models this filter is the model's own safety classifier (for example the BIO category), so the verdict repeats on every retry. Clients kept retrying a deterministic refusal, and the user never learned it was one.

A captured upstream stream for such a request (claude-sonnet-5.5, no tools) contains no content at all: initial-response, then metadataEvent {"stopReason":"CONTENT_FILTERED","stopDetails":{"refusal":{"category":"BIO","explanation":"The selected model cannot continue this conversation…"}}}, then contextUsageEvent.

Root cause. In parseKiroAttemptEvents (src/adapters/kiro/stream.ts), explicit stop reasons were only mapped inside the mode === "required" block. A tool-less turn runs in disabled mode, skipped that block, and fell through to the retryable empty_kiro_stream fallback. The same gap existed in the bounded text_fallback retry, which mapped a refusal to empty_kiro_fallback. And in disabled mode, partial text followed by a refusal completed as a normal done with end-of-turn set.

Fix. It reuses the existing representation of a refusal, the non-retryable incomplete with reason: "content_filter", so no new error shape is introduced:

  • Messages renders it as stop_reason: "refusal", which Claude clients already understand.
  • Responses renders it as a terminal response.failed with code invalid_prompt, which Codex does not retry.
  • The empty-completion guard already passes incomplete events through unchanged; a test confirms it never retries the refusal.

Changes:

  • src/adapters/kiro/stream.ts: CONTENT_FILTERED and GUARDRAIL_INTERVENED are checked in every completion mode, before the empty-stream fallback and before any completion retry. The old required-only check is removed as redundant. A refused text_fallback retry is mapped the same way, and its partial text goes out as commentary, never as an accepted answer.
  • src/adapters/kiro-events.ts: reads stopDetails.refusal (category and explanation) leniently, so a malformed shape only drops the details, and caps the field lengths (64 and 400 characters). The message reads Kiro stopped with CONTENT_FILTERED, category BIO: <explanation>. This is Kiro's model-side wording, never request content.
  • structure/providers/kiro.md: new "Content-filter stops" section.

Behaviour changes:

  • In disabled mode, text followed by a refusal now ends with the refusal instead of a normal completion. That matches required mode and the Anthropic API.
  • In required mode the outcome is unchanged; only the message text changes, to include the category and explanation.

Combined with ##8071: the two PRs merge cleanly in either order (all four open Kiro/T3 PRs, #8070–#8073, were checked pairwise and merged together). On the combined tree, bun run typecheck, bun run structure:check, ./tests/providers/kiro (734 pass) and the Kiro server end-to-end set (114 pass) all pass. On that tree a refused structured-output turn ends as the non-retryable content_filter refusal, ahead of any structured-output handling. One edge case is left for a follow-up once both have landed: when the bounded structured-output retry is itself refused, the retry's partial text is released as commentary before the refusal. The turn still ends as a refusal, so it is never accepted as a JSON answer.

Verification

All results below are for head 782930b44.

Review follow-ups (CodeRabbit):

  • When the bounded retry is the attempt that gets filtered, the first attempt's held progress is released instead of being superseded, followed by the retry's partial text and then the refusal. The regression test fails on the previous head.
  • The three tool-less server cases now also run with partial text before CONTENT_FILTERED; those variants fail against dev's Kiro code.
  • The checks were moved ahead of the completion branches, with explicit guards that keep a validated final answer and a real tool call ahead of the stop. Behavior is unchanged, and the PR no longer shares an insertion point with fix(kiro): emulate structured output instead of refusing it #8071.
  • Non-streaming /v1/responses is covered too, asserting the shared buffered contract (an incomplete body with content_filter and no error); both variants fail against dev's Kiro code.
  • docs-site/src/content/docs/guides/providers.md describes the non-retryable refusal and what a filtered bounded retry shows.
  • Doc comments on the functions this PR touches, for CodeRabbit's docstring-coverage pre-merge check (it requires 80% of touched functions with branches). Comments only, no behavior change; the results below were re-run on this head.
Check Command Result
New adapter tests (built from the captured frames) bun test tests/providers/kiro/kiro-content-filter.test.ts 10 pass
New server tests (/v1/messages non-streaming and streaming, /v1/responses streaming and non-streaming, each with and without partial text before the stop; each asserts a single upstream send) in tests/server/server-kiro-refusal-e2e.test.ts pass
Without the fix same new tests 10 of 12 fail. The two that still pass are deliberate controls: an empty stream with no stop reason keeps today's retryable behaviour, and malformed refusal details are dropped
Kiro provider tests bun scripts/test.ts --parallel=1 ./tests/providers/kiro 696 pass, 0 fail
Kiro server end-to-end and empty-completion guard bun scripts/test.ts --parallel=1 ./tests/server/server-kiro-completion-e2e.test.ts ./tests/server/server-kiro-refusal-e2e.test.ts ./tests/server/server-kiro-usage-log-e2e.test.ts ./tests/responses/empty-completion-guard.test.ts 110 pass, 0 fail
Layout and file-size ratchet bun test tests/test-layout.test.ts tests/test-layout-tooling.test.ts tests/ci-workflows/file-size-ratchet.test.ts 38 pass, 0 fail
Typecheck / structure SSOT / privacy scan bun run typecheck, bun run structure:check, bun run privacy:scan pass

Live check against the Kiro runtime, using a probe that runs this branch's adapter directly. The request that tripped the BIO classifier now returns incomplete / content_filter with Kiro's category and explanation after a single send. Before the fix it was a retryable 502 every time (6 of 6).

Not changed:

  • A content-free disabled-mode stream with another explicit stop reason (for example MODEL_CONTEXT_WINDOW_EXCEEDED) still falls back to the retryable empty_kiro_stream.
  • A real tool call followed by CONTENT_FILTERED still completes as a tool turn.

Full-suite scope exception: bun run test was not run locally. The machine (WSL2, 16 GB shared with other work) runs out of memory on the full suite, so only the domains above were run. The rest of the suite is left to CI.

Checklist

  • Scope stays focused and avoids unrelated cleanup.
  • Docs or release notes were updated when needed. (structure/providers/kiro.md.)
  • Security-sensitive changes were reviewed for secrets, auth, and unsafe defaults. (Upstream refusal text is bounded and contains no request content; no auth or credential change.)

Review readiness checklist

This PR stays in draft until every box below is ticked. Tick all four boxes once the requirements are met:

  • Required local validation passed; commands, results, and any full-suite exception are documented.

  • I pushed my PR to a recent dev commit (at most 10 behind; a maintainer may still ask for the exact tip before merge).

  • I resolved all correct Codex and CodeRabbit findings.

  • My PR is ready for review.

Summary by CodeRabbit

  • Bug Fixes
    • Kiro content-filtered responses now report a refusal instead of being treated as an empty stream or overload, including when a response contains partial text.
    • Refusal responses can include the filter category and explanation when available.
    • Filtered requests are marked as non-retryable, preventing unnecessary repeated attempts. Streams without a content-filter stop retain their existing retry behavior.
  • Documentation
    • Updated provider guidance to describe Kiro refusal responses and retry behavior.

A tool-less turn that Kiro's safety filter refuses (CONTENT_FILTERED, e.g. the Claude BIO classifier) arrives with no content. Explicit stop reasons were only mapped in required completion mode, so a disabled-mode refusal fell through to the retryable empty-stream error and clients received a 502 overloaded_error for a verdict that repeats deterministically.

CONTENT_FILTERED and GUARDRAIL_INTERVENED now map to the non-retryable content_filter incomplete in every mode, which Messages renders as stop_reason refusal and Responses as a terminal failure. Kiro's refusal category and explanation are carried in the message.

Walkthrough

Kiro metadata parsing now extracts bounded refusal details. Stream handling treats CONTENT_FILTERED and GUARDRAIL_INTERVENED as non-retryable content-filter results unless a validated answer or real tool call takes precedence. Tests and documentation cover adapter behavior and API responses.

Changes: Kiro content-filter handling

Layer / File(s) Summary
Refusal metadata and stream handling
src/adapters/kiro-events.ts, src/adapters/kiro/stream.ts
The metadata parser retains nonblank refusal categories and explanations, limited to 64 and 400 characters. The stream parser uses these details for non-retryable content_filter results when Kiro reports either filter stop reason.
Filtered-turn and API response tests
tests/providers/kiro/kiro-content-filter.test.ts, tests/server/server-kiro-refusal-e2e.test.ts
Adapter tests cover filtered turns, partial text, required mode, bounded retries, and retryable empty streams. Server tests cover buffered and streaming Messages refusal responses, streaming Responses invalid_prompt failures, and buffered Responses incomplete results.
Documentation and test assignment
structure/providers/kiro.md, docs-site/src/content/docs/guides/providers.md, scripts/test-layout/layout.json, tests/fixtures/test-layout-expected.json
Documentation describes filter outcomes and protocol-specific responses. The test layout assigns the new test to the Kiro shard.

Same behavior: a validated final answer and a real tool call keep their precedence over a content-filter stop, now through an explicit guard. Placing both checks before the completion-answer branches keeps them first in order for every outcome that follows, and stops them sharing an insertion point with other changes to the same terminal sequence.

Review follow-up. A bounded retry that streamed text before CONTENT_FILTERED made the prior-attempt drain treat that text as superseding the first attempt's held progress, so the client lost it although neither attempt produced an accepted answer. The refusal check now runs before that drain and releases the first attempt's progress, then the retry's partial text, then the refusal. Server tests now cover partial text before a refusal on tool-less Messages (buffered and streaming) and Responses turns.

Maintainer integration into dev (account lidge-jun) at head a9e19fc054e3285c21b79f366d813a4ff5dfd526. The PR is moved out of draft by a maintainer for this integration; the contributor checklist gate re-drafted it after maintainer follow-up pushes.

fix(kiro): report content-filter stops as non-retryable refusals. Dev merge plus one docs conflict (both passages kept). Focused 66/0.

  • The branch merges current dev; exact-head required CI passed (ci, enforce-target, hygiene, test 1/4–4/4, ci:full).
  • Independent review (gpt-6.1-terra, high): fresh final review PASS on this head.
