When Codex CLI signs in with ChatGPT and sends its own bearer through a Pool-mode proxy, that request is request-owned: since #5019 the bearer makes main one more round-robin candidate, but the request never got an affinity key. Every turn therefore re-ran the strategy, so one conversation hopped between main and the stored accounts (~35% of consecutive turns in the report), and the request log still labelled every Pool pick affinity=new_bind, affinityReason=healthy although nothing was bound.
Before: round-robin, main + 1 stored account, one conversation → main, pool, main, pool…, each pool row new_bind/healthy, 0 reused. After (default): same routing, but each pool row reads unbound/request_owned, which says what actually happened. After (codexPool.requestOwnedAffinity: true): the conversation stays on one eligible account (new_bind once, then reused), including when that account is main.
What changed:
Honest labels for everyone. Right after selection returns, honestAffinityDecision keeps new_bind/rebound only when the key's ordinary or model-detour entry names the selected account. Otherwise new_bind → unbound (reason request_owned, no_conversation, or the original reason when a key existed but nothing was stored) and rebound → cleared. unbound is not counted as a spend move reason. Caller-owned main fallbacks now report what happened to main (detour/unbound), not another account's decision.
Opt-in request-owned affinity (codexPool.requestOwnedAffinity, default false). Key = request-owned:v1: + HMAC-SHA256 under a process-random secret over the exact Authorization value and the conversation key (src/codex/request-owned-affinity.ts). Final auth and both request previews (first and recovery, per candidate model) use one derivation; fixed account selectors and Reserve get no key; request-owned traffic never records lineage.
Carried state follows the real serving account. Request-owned Pool and caller-owned main contexts carry a separate bearer-scoped conversationStateKey, used only for conversation-state issuers and never passed to routing. Pool↔main moves (including retry and compact alternates, and the manual main pin path) scrub previous_response_id / refuse file references like any other account change. This also applies with the opt-in off, a deliberate behaviour change: those rows used to record nothing for main and borrow the stored-credential raw key for Pool. Manual account selection now clears routing bindings but keeps issuer records (clearThreadAccountMap({ preserveConversationStateIssuers: true })); a full reset still clears both.
A refused caller bearer does not pin the conversation. When a caller-owned main binding's own bearer is refused (quota → quota_refusal, 401/403 → unusable, or the final transient failure after the existing retry ladder → transient), that caller-scoped binding is released and the next turn is placed by the strategy. The release is fenced to the binding entries captured when main was selected, so a delayed failure never removes a newer binding, and nothing is written to shared physical-main health, quota or cooldown. Only a keyed caller-owned main inspects a 5xx body.
The request-owned main pin logic moved byte-for-byte to src/codex/request-owned-main-pin.ts (re-exported) to keep auth-context.ts under the 2000-line ratchet.
Ownership invariant. Before: a request-owned bearer owns no Pool affinity, lineage, persistence or shared state. After: unchanged by default; with the opt-in it may own a process-local binding from (exact Authorization, conversation) to the serving account. A caller's key cannot steer another principal's credentials: keys match only for byte-identical bearers; a __main__ binding is always served from the presenting request's own bearer; a stored-account binding names an account ordinary selection could already give that caller, re-checked for eligibility on every turn; account-id claims are never used as principal. Details: structure/providers/codex-request-owned-affinity.md.
Merge-order dependency with the affinity persistence lane (#8091). Persistence must never anchor, serialize or adopt rows for keys where isRequestOwnedAffinityKey is true (any scope, including issuers). Whichever of the two lands second adds a combined flush-and-restart regression.
Security review (MAINTAINERS.md). Dedicated review by a separate reviewer. Rounds 1–2 FAIL (manual-pin issuer gap; keyless request-owned issuers used the shared raw key; doc mismatch), all fixed; round 3 PASS-WITH-NOTES on the full diff: "The opt-in request-owned affinity key isolates bindings and issuer records by bearer and conversation while keeping credentials out of the key and logs. Caller-owned main selections continue to use the presenting bearer, and account changes use issuer records to scrub or refuse nonportable state. Fixed-account issuer sharing remains the existing stored-credential behavior." The later refusal-release commits got two more security+code delta reviews: round 4 PASS-WITH-NOTES (two medium findings: stale release, unkeyed 5xx body wait, both fixed) and round 5 PASS-WITH-NOTES with no findings: "The change fences caller-main refusal cleanup to the bindings selected by that request, preserving newer bindings. It also avoids quota-body inspection for unkeyed caller-main responses." Independent code review: PASS. Review feedback from Codex (release after refusals) and CodeRabbit (devlog security notes, docs workflow, structure present tense) is addressed in 87d6eeb5d/ca1551cd6.
Follow-ups noticed, not changed here (all present on origin/dev and shared with stored Pool traffic):
KNOWN_AFFINITY_REASONS in src/usage/log.ts lacks the existing lineage_parent / lineage_sibling reasons, so those are dropped from usage.jsonl.
The bounded application/json branch of deliverPassthroughResponse never invokes the terminal outcome recorder, so a 200 JSON body carrying response.failed or a quota incomplete records no outcome (Pool) and releases no caller-main binding.
Compact account-change scrubs omit the effective headers, so a stale x-codex-turn-state header is not removed, and compact forwards account-bound reasoning/compaction ciphertext on an account change.
Regression tests fail on old code: each round's new tests were run against a git archive export of the previous head (origin/dev for the feature: affinity suite 15/15 failing plus the Reserve, compact and retry cases; then the preview-closure, refusal-release (16) and fence/body-gate (3) cases) and pass on this head.
Wider local run of every passthrough/WebSocket test file (133 files, 4,571 tests): 9 failures plus 4 errors, all 1–21 s timeouts in server-live, server-auth (/readyz) and errors-adapter-failure, a different set on each rerun. bun scripts/test.ts --changed=dev --parallel=1 on the earlier head: 17,433 pass, 18 timeouts in local-credential-generation / provider-completion-test-ingress, then the runner timeout (exit 124). The same files fail the same way on a clean git archive origin/dev export, so these are host contention (three other lanes on the machine), not this change. The ci:full shards are the authority for them.
PR CI with ci:full: all 30 checks green on ca1551cd6 (all four full Ubuntu shards, gates, structure gate, docs build, smokes).

Walkthrough
This change adds opt-in, process-local affinity for request-owned Codex credentials. It derives keys from the bearer and conversation identity, updates account selection and affinity labels, and carries conversation-state bindings through account changes, previews, and response outcomes.
Auth resolution derives affinity and conversation-state keys, excludes request-owned credentials from stored lineage, and reports decisions for main and Pool selection paths. Tests cover key isolation, selection, fallbacks, Reserve handling, logging, and main-pin behavior.
Conversation state and preview handling src/codex/routing/active-account.ts, src/codex/routing/thread-affinity.ts, src/server/responses/account-change-state.ts, src/server/responses/compact.ts, src/server/responses/core-codex-account.ts, src/server/responses/request-prepare.ts, tests/codex-integration/codex-request-owned-affinity.test.ts, tests/responses/responses-preview-main-read-fence.test.ts, tests/responses/compact-transient-alternate.test.ts, tests/responses/responses-alternate-main-cancellation.test.ts
Account-change handling uses keyed main contexts to identify conversation-state issuers and scrub carried state when the serving account changes. Preview affinity keys come from each preview’s auth context. Manual routing resets can retain issuer records.
Eligible refusal and terminal outcomes release request-owned main bindings. Tests check refusal handling, retained bindings for other outcomes, and release reasons.

When a Codex conversation moves between the Pool and the main account, compact requests can still carry the previous account's turn-state header and encrypted content, so they may be rejected. A failed or quota-limited JSON response may also leave the conversation bound to the same account. Fix these before merging, particularly if request-owned affinity will be enabled.

Explanation
Issue #8080 requires either readable refusal behavior or opt-in affinity for request-owned Codex traffic. The PR provides both outcomes. honestAffinityDecision in src/codex/request-owned-affinity.ts changes an unrecorded new_bind to unbound with request_owned or no_conversation, and src/server/request-log.ts excludes unbound from spend move reasons. When codexPool.requestOwnedAffinity is true, requestOwnedAffinityKey derives a process-local HMAC key from the exact Authorization value and conversation key. src/codex/auth-context.ts supplies that key to eligible Pool selection, and the integration tests cover binding, reuse, bearer and conversation isolation, model detours, eligibility changes, previews, and default-off behavior. Fixed selectors and Reserve remain keyless. The separate conversation-state keys and account-change scrubbing in src/server/responses/account-change-state.ts, src/server/responses/compact.ts, and src/server/responses/core-codex-account.ts prevent carried state from crossing account changes. The later refusal-release path now captures exact bindings at selection in captureRequestOwnedAffinityBindings and releases only matching entries in releaseRequestOwnedMainOnOutcome; the added tests cover 401, 402, 403, 429, 503, and delayed newer bindings. No concrete coding requirement from #8080 remains unmet.

Explanation
The changed files remain connected to #8080. src/codex/auth-context.ts, src/codex/request-owned-affinity.ts, and src/codex/routing/thread-affinity.ts implement request-owned keying and honest selection labels. src/config/schema/leaf-validators.ts and src/types/config.ts expose the opt-in setting. The response modules update carried-state handling and caller-main refusal cleanup required by account changes. tests/codex-integration/codex-request-owned-affinity.test.ts and the added response and Reserve tests verify the same behavior. Documentation, structure metadata, logging updates, and the request-owned-main-pin.ts extraction support the feature and its review constraints. The persistence exclusion is documented as a dependency for the separate persistence lane; this PR does not add unrelated persistence behavior. No demonstrated unrelated change is present.

…ion affinity (#8080)
A Codex request carrying the caller's own ChatGPT bearer in Pool mode got no
affinity key, so round-robin re-picked every turn and the request log still
reported new_bind/healthy without any stored binding.
- Affinity decisions are checked when selection returns: new_bind/rebound stay
only if the key's entry names the selected account; otherwise the row reads
unbound (request_owned / no_conversation / original reason) or cleared.
- codexPool.requestOwnedAffinity (default off) gives request-owned traffic a
process-local key scoped by an HMAC of the exact Authorization value and the
conversation key. Final auth and both previews share one derivation; fixed
accounts and Reserve get none; lineage is never recorded.
- Keyed caller-owned main contexts carry the key and act as issuer __main__, so
Pool<->main moves (incl. retry and compact alternates) scrub carried state.
- Request-owned main pin logic moves byte-for-byte to request-owned-main-pin.ts.
…w findings (#8080)
- Request-owned Pool and caller-owned main contexts carry a separate
conversationStateKey (bearer-scoped HMAC), never passed to routing. Issuer
records no longer borrow the stored-credential raw conversation key, and
Pool<->main moves scrub carried state even with the opt-in off; the manual
main pin path keeps the issuer key.
- Caller-main fallbacks report what happened to main (detour/unbound), not the
other account's decision.
- Previews derive the key with each candidate model; recovery recomputes lineage
from its own headers and ownership.
…8080)
- Reserve and recovery preview regressions exercise the actual subagent
fallback and encrypted-task recovery closures instead of source text.
- Structure doc: manual selection keeps conversation-state issuers; stored
credential traffic keeps its shared issuer namespace, unchanged.
- Response-state store is cleared after the alternate-main test.
…efused (#8080)
With codexPool.requestOwnedAffinity on, a conversation bound to caller-owned
main stayed there after the caller's bearer returned 429s, because main
contexts bypass Pool outcome recording. Quota, credential and final transient
refusals now release that caller-scoped binding (ordinary and model-detour
entries) and carry the release reason to the next selection, without touching
shared physical-main health, quota or cooldown. Covers regular, compact,
streamed-terminal and WebSocket terminal paths.
Docs: guide documents issuer-based scrubbing with the option on or off; the
structure doc keeps only the present-tense persistence rule; the devlog unit
is reduced to a present-tense record.
)
- A delayed refusal releases only the binding entries captured when main was
selected, so a newer binding from a later turn of the same conversation
survives.
- Only a caller-owned main carrying a request-owned key inspects a 5xx body,
so unkeyed main requests are never delayed by the quota body reader.

Scrub the effective headers for every compact account-change send.
986-986: 🗄️ Data Integrity & Integration | 🟠 Major | ⚡ Quick win
Scrub the effective headers for every compact account-change send.
FORWARD_HEADERS includes x-codex-turn-state, and compact sends those headers through sendCompactAttempt. The primary scrub calls omit headers, so the scrubber cannot remove the previous account's turn state. The alternate helper has the same issue: it scrubs the bodies, then sends alternate.headers unchanged.
Pass headers to the primary scrub calls. Change scrubCompactStateForAlternate to accept the alternate headers and pass alternate.headers at both alternate call sites. Add a Pool-to-main regression test.

Remove account-bound ciphertext before alternate compact sends.
1441-1441: 🗄️ Data Integrity & Integration | 🟠 Major | 🏗️ Heavy lift
Remove account-bound ciphertext before alternate compact sends.
When the serving account changes, applyAccountChangeConversationStateScrub removes continuation IDs but deliberately preserves encrypted reasoning and compaction ciphertext. The compact path sends compactBody after this scrub, so the alternate account can receive ciphertext created by the previous account and reject the request. The initial compact account-change path has the same gap.
Apply the existing account-change ciphertext projection to both compact paths, and add regression coverage for encrypted reasoning and a native compaction item.

@coderabbitai Follow-up comments on compact.ts — verified, declined for this PR as pre-existing; both are listed as follow-ups in the description:
Scrub effective headers on compact account change (line 986). The compact scrub calls on origin/dev (compact.ts 984–994 and the alternate helper at 1437) already omit headers for every account change, Pool to Pool included. This PR only lets a caller-owned main context reach the same scrub; before it, that move was not scrubbed at all, so turn-state handling is unchanged or better, never worse. Passing headers changes compact behaviour for all Pool traffic and belongs in its own change.

@lidge-jun Agreed. I withdraw both comments as required changes for this PR.
Effective headers: The dev baseline already omits headers from the scrub calls in src/server/responses/compact.ts at Line 984, Line 990, and Line 1437. The current calls retain that behavior. Passing headers would change existing Pool behavior, not fix a regression established here.
Account-bound ciphertext:src/server/responses/account-change-state.ts, Lines 210–216, explicitly leaves encrypted reasoning and compaction ciphertext to the #2247 projection. The baseline compact alternate path already uses this scrub helper. I should not have treated that existing behavior as a new defect in this PR.
Both can remain separate follow-ups. Would you like me to open a GitHub issue to track them?

All 20 required checks passed on this head, including gates and the four ci:full Ubuntu shards; 10 more were skipped by PR CI policy.
Security review (required because this touches credential ownership): a dedicated Terra security review ended PASS-WITH-NOTES in round 3, after rounds 1–2 failed and were fixed. Two later combined security and code reviews covered the fix commits; the last found nothing. The verdicts are in the PR description.
Review threads: all resolved, including the Codex finding (release caller-owned main bindings after refusals). The Codex summary state still names the older commit 9407e17. Three CodeRabbit findings were declined with evidence that the same gaps exist on dev for all Pool traffic; they are listed as follow-ups.
Coordinator check of src/codex/request-owned-affinity.ts: opt-in only (codexPool.requestOwnedAffinity, default off). Each key is an HMAC under a process-random secret over the exact Authorization value and the conversation key, so two requests share a binding only with an identical bearer. The keys are process-local and never persisted, and refusal release removes only the entries captured at selection.
Lane B (#8098, persistence) lands after this one, so #8098 must keep request-owned: keys out of the snapshot and add the combined flush-and-restart test. Closes #8080, which I will close manually after this merge.
