fix(anthropic): normalize caller instructions in translated OAuth requests - #8194
System-role placement is now preserved by default for every translated OAuth caller and model. Relocation requires the explicit provider configuration providers.<name>.anthropicOAuthUserInstructions: true. This is operator-owned configuration, not an inference-request parameter. It changes instruction priority and is intended only for operators who accept that compatibility tradeoff. API-key requests and native Messages passthrough remain unchanged. The earlier unconditional behavior described in the investigation below is superseded by this opt-in.
The test registration moved from layout.json to the existing layout-adapters.json shard, retaining the expected-layout entry. All seven translated adapter references now link to the canonical English explanation of the option and precedence loss.
bun run privacy:scan, bun run structure:check, git diff --check: passed.
Documentation frozen install and build: passed, 641 pages and 93,157 internal links checked.
Layout checks still have three EPERM failures scanning the pre-existing inaccessible tests/routing/.tmp-probe-lease-dispatch-wiring; not claimed as passing.
Full suite and test:changed deliberately not run at the user's request. The shared adapter has broad import connectivity; focused coverage was selected instead.
No install, restart, or live probe of this opt-in revision was performed. Earlier live evidence applies to the preceding implementation, not this exact revision.
The demonstrated behavior is a wire-role change, not demonstrated unauthorized model execution. Maintainer security review remains outstanding; no objection is marked resolved on that basis alone.
Summary
Fix caller-instruction placement in translated Anthropic OAuth requests, including OpenCode requests entering through /v1/chat/completions.
An OpenCode request to claude-opus-5-5 returned HTTP 400 with the following upstream message, while a Codex request through the same account and model succeeded:
Third-party apps now draw from your extra usage, not your plan limits.
Add more at claude.ai/settings/usage and keep going.
Captured outbound bodies showed a placement difference. The working Codex request carried its agent instructions in user messages. Its top-level system contained the existing OAuth compatibility identity and OpenCodex's tool contract. The rejected OpenCode request put caller instructions and the tool contract together in top-level system.
This PR makes the translated OAuth adapter:
Keep the existing compatibility identity and proxy-owned tool contract in top-level system.
Prepend only caller instructions to the first user turn. For assistant-leading histories, insert a separate initial user turn instead of modifying a later tool-result message.
Keep existing tool call/result messages adjacent and preserve tool declarations, names, schemas and reverse mapping.
Cache the relocated instruction prefix explicitly, retaining the existing automatic prompt-caching policy and breakpoint limits.
API-key prompt placement and native Messages passthrough are unchanged. No authentication headers, model selection, tool naming, or tool_choice policy are changed. Moving caller instructions into user content changes their wire role; this follows the working Codex request's arrangement, rather than claiming equivalent system-role priority.
Evidence and limits
During the initial investigation, the captured OpenCode body returned 400. A replay relocating its instruction block and adding tool_choice: auto returned 200 with OpenCode's 13 tools. Replacing those tools with the captured Codex catalog also returned 200. These observations do not establish that tool catalogs never affect provider classification.
The initial local adapter patch subsequently returned 200 for the original captured chat request without changing its tool_choice, and both opencode run and codex exec returned ok. Those live checks were on the initial implementation, which also moved the tool contract. The revised runtime at 6e2feef0d separates that contract. It was then installed in the active OpenCodex 2.97.0 instance, restarted successfully, and exercised with opencode run --format json -m opencodex/anthropic/claude-opus-5-5#medium: the model returned ok. Subsequent commits change documentation only. No provider-classifier internals or hash/bucket mechanism are assumed.
Related reports describe similar symptoms, not dependencies or borrowed implementation:
After the final test typing cleanup, the new regression file alone: 9 pass, 0 fail.
bun run structure:check: passed.
git diff --check: passed.
Documentation: cd docs-site; bun install --frozen-lockfile; bun run build passed. Astro built 641 pages and checked 93,146 internal links.
Layout validation together with the focused files: 160 pass, 3 fail. All three failures are EPERM while scanning the existing tests/routing/.tmp-probe-lease-dispatch-wiring directory. The test is registered in both layout inventories; the inaccessible directory was left untouched.
bun run typecheck: unavailable with the installed Bun, which reports Script not found "check".
bun run typecheck:tsc: blocked by two errors in unchanged src/providers/cc-switch-import-source.ts:411,419, where the installed Database type has no function member.
Live OpenCode validation of the revised runtime passed. Type/layout checks remain blocked as documented above; readiness is not claimed.

New Features
Added an opt-in setting for Anthropic OAuth requests that places caller instructions at the start of the first user turn. If conversation history begins with an assistant turn, a user turn is added first.
The setting is off by default. API-key requests and native Messages passthrough retain their existing instruction placement.
Documentation
Updated Anthropic adapter references with details about the setting and its effect on instruction priority.
Anthropic's OAuth overage classifier treats a caller-supplied system prompt in
the top-level system array as third-party traffic. With extra usage disabled
the turn is rejected with 400 "Third-party apps now draw from your extra usage,
not your plan limits", which surfaced as a generic provider rejection.
Claude Code, and this proxy's own Responses lane, carry the caller's
instructions inside the first user turn while the top-level array holds only the
Claude Code identity. The chat lane put them in system instead, so the same
account reached the model through Codex and failed through every
OpenAI-compatible client.
Move the caller's system text to the head of the first user turn for OAuth
providers, keeping the identity block alone at the top level. API-key providers
are unchanged.

Walkthrough
The change adds an opt-in setting for Anthropic OAuth requests. When enabled, caller system instructions move to the first user turn, and prompt caching marks the relocated prefix. The provider setting, request-shape tests, and documentation are updated. API-key requests retain their existing instruction placement.

The change makes tests/adapters/adapter-usage.test.ts:153-179 fail: the identity block now has the cache breakpoint, and the caller prompt is in the first user message. Update the test to assert that shape and retain its automatic-caching check.

The new layout registration at scripts/test-layout/layout.json:188 brings that file to 2,000 lines, which makes it a new oversized file under the repository ratchet. On the local merge with current dev, tests/ci-workflows/file-size-ratchet.test.ts fails with NEW_OVERSIZED for this file; the PR head itself also has 2,000 lines, while its merge base and current dev have 1,999. Please move the registration into the appropriate existing domain shard (and retain the expected-layout registration) so the file stays below the threshold.

The relocation is unconditional for translated OAuth requests: src/adapters/anthropic.ts:1102-1103 passes isOAuth for every caller and model, and lines 1145–1147 put the caller's system instructions in the first user turn. An application that places trusted policy in its system prompt and accepts untrusted user input therefore loses the system-role instruction boundary, including on previously working OAuth clients and models. A base/head wire-shape probe confirmed the demotion; actual model compliance or unauthorized execution has not been demonstrated. The documentation acknowledges the changed precedence, but there is no operator choice before it applies. Please preserve system placement by default and gate this compatibility behavior behind an explicit operator-controlled option, with regression coverage for unrelated OAuth callers and models.
The reason will be displayed to describe this comment to others. Learn more.
The new OAuth instruction-priority explanation in docs-site/src/content/docs/reference/adapters.md:364 is absent from the translated Anthropic adapter references, such as fr/reference/adapters.md:91 and zh-tw/reference/adapters.md:93. Readers of those pages can miss the material change in instruction placement. Please synchronize the explanation, or link each translation to the canonical explanation, once the security behavior is settled.
Preserve system-role placement by default for translated OAuth requests. Validate a provider-owned boolean compatibility option and cover absent/false defaults across caller prompts and models. Move test registration into the existing adapter shard and link translated docs to canonical precedence guidance.
opencodex should:
A) Uniform "Connect" function to OpenCode, using Responses API
B) generalize the field in which there is a translation. that field is "developer" for Codex, and "instructions" for OpenCode, probably other harnesses have different (Which? I suppose 99% of users go with Codex). This is only needed for our beloved Anthropic adapter.
Also actually
C) Separate concerns about translating requests inside the Anthropic adapter, After all.... all of this code mess it's because of them, so we should put that logic into its adapter, not cluttered around the codebase.
