Summary

  • When a combo target reaches its output token limit before producing any visible output or running side effects, combo routing previously treated the terminal response as final and returned an empty incomplete response to the client instead of attempting the next candidate target.
  • On remote compaction tasks, Codex requests were additionally blocked from failing over because classifyZeroOutputTokenLimit was gated on !routedCompaction.
  • Non-runTurn streaming combo targets were also committed because preflightComboStreamResponse explicitly treated max_output_tokens as a non-retryable semantic reason.
  • Fixes applied:
    1. In src/server/responses/run-turn-execution.ts, removed the !routedCompaction guard so remote compaction tasks can safely fail over when a provider hits max_output_tokens before producing the summary.
    2. In src/server/responses/combo-stream-preflight.ts, updated retryableZeroOutputTerminal to classify response.incomplete with reason max_output_tokens as retryable whenever output is empty. failedTerminalResponse maps it to a 502 with code zero_output_token_limit, preserving token usage.
    3. Replay remains strictly prevented whenever output deltas or items were already emitted before the limit.

Verification

  • bun test tests/routing/combo-stream-preflight.test.ts tests/adapters/run-turn-queue.test.ts tests/server/server-combo-zero-output-failover.test.ts: 113 pass / 0 fail.
    • Verified remote compaction failover on zero-output max_output_tokens.
    • Verified stream preflight failover on zero-output max_output_tokens.
    • Verified that max_output_tokens after visible output does not fail over.
  • bun run typecheck: exit 0 (0 type errors in 2,317 files).
  • bun run structure:check: exit 0.
  • bun run privacy:scan: exit 0.

Summary by CodeRabbit

  • New Features
    • Requests that hit an output-token limit before producing visible content can now fail over to another available target. The same limit reached after visible output does not trigger failover.
    • Zero-output token-limit failures are reported with a specific error, and usage information is preserved when available.
    • A target that returns a zero-output token-limit failure is not placed in cooldown.
  • Documentation
    • Updated combo guidance to explain when zero-output token-limit responses can advance to another target.

Walkthrough

Eligible zero-output token-limit terminals are classified as upstream errors. Combo preflight and routing use this classification to advance to another target when no visible content has been emitted. The changes also preserve usage data when present and update the failover guidance.

Changes: Zero-output token-limit failover

Layer / File(s) Summary
Classify events and buffer preflight output
src/adapters/run-turn-queue.ts, tests/adapters/run-turn-queue.test.ts
Eligible incomplete and done events produce a 502 zero_output_token_limit error. Preflight buffers reasoning events and, when enabled, compaction text. It enforces event-count and payload limits. Tests cover classification conditions and event replay.
Apply classification during combo execution
src/server/responses/run-turn-execution.ts, src/server/responses/core-combo.ts, src/server/responses/request-transport.ts, tests/server/server-combo-zero-output-failover.test.ts
Combo execution classifies eligible terminals during preflight and buffered handling. It checks replay safety and binds usage from errors when present. Tests cover backup dispatch, visible output, reasoning, remote compaction, replay-unsafe heartbeats, and exhausted targets.
Set retry classification and document behavior
src/server/responses/combo-stream-preflight.ts, src/combos/failover.ts, tests/routing/*, docs-site/src/content/docs/guides/combos.md
Combo stream preflight maps zero-output max_output_tokens incompletes to a retryable failure. Routing treats the failure as a hop without cooling the candidate. Tests and guidance cover this behavior and preserve streams after output.

A provider could hit its output limit before emitting any text or tool
calls. Combo routing previously treated that terminal response as final
instead of trying another target.

Introduce classifyZeroOutputTokenLimit to check the first meaningful event:
when an incomplete response (with reason max_output_tokens) or done response
(with stopReason max_tokens/length/max_output_tokens) arrives before any
output or side effects, classify it as a 502 zero_output_token_limit error.
This allows combo routing to fail over to the next candidate target while
preserving usage data and avoiding replay after visible output, tool side
effects, or compaction.
…ht and compaction

Support zero-output max_output_tokens failover in preflightComboStreamResponse
when no output items or text deltas were emitted before the terminal.
Additionally allow compaction turns to fail over zero-output token limit
terminals by removing the !routedCompaction exclusion in run-turn execution.
…output limit

When a run-turn adapter (such as Devin) streams thinking_delta before hitting its
output token limit, preflightAdapterEvents previously committed on the first
reasoning event, preventing combo failover when the token limit arrived with no
visible text or tool calls.

Buffer pre-content reasoning events during preflight until first content or a
terminal arrives, classifying zero-output token limits for failover in both
streaming and non-streaming modes.

Major · Functional Correctness · Quick win

Scan all buffered events for replay-unsafe heartbeats.

If a runTurn adapter emits reasoning, then a replay-unsafe heartbeat, then a qualifying max_output_tokens terminal, the buffered scan misses the heartbeat. The resulting 502 can send the turn to another target after a local side effect. Streaming preflight latches the heartbeat.

Suggested fix

         const replayUnsafe = events
-          .slice(0, firstMeaningfulIndex === -1 ? events.length : firstMeaningfulIndex)
           .some(event => event.type === "heartbeat" && event.replayUnsafe === true);

Add a non-streaming regression test for this event order that asserts the backup target is not called.
