Summary

Two dev test flakes were reddening the test changed job on unrelated pull requests (both seen on #8282). One is a product race in the Node launcher; the other is a test that depended on the live network and then took ten neighbors down with it.

Launcher signal race (product fix). runDesktopCliHandoff and the Bun child path in bin/ocx.mjs registered their SIGINT/SIGTERM/SIGHUP forwarders only after spawn() returned. The child can exec and write output before that, so a signal sent to the launcher in that window took the default action. The launcher died, its sync-mutex exit cleanup never ran, and the child was orphaned without ever seeing the signal. That is the exit code null (expected 29) in tests/cli/ocx-launcher-desktop-handoff.test.ts:332. The forwarders now come from the new src/lib/launcher-signal-forwarding.mjs and are installed before both spawns. A signal observed before the child is attached is delivered as soon as it is. The update-safety tables that name the moved child.kill(signal) site, the launcher's reviewed imports, and the update-stop-first instrumentation seam were updated to match.

chat-completions network dependency (test fix). For a streamed request to the canonical ChatGPT URL, the proxy dials wss://chatgpt.com/backend-api/codex/responses before its HTTP fetch. The test only patched fetch, so ten cases in tests/responses/chat-completions-endpoint.test.ts dialed the real upstream and waited up to the 10 s upgrade deadline before the mocked HTTP fallback. Usually that took about 190 ms; when the handshake stalled, the case hit the 5 s test timeout. The cases already looked opted out, but their configs lacked openaiProviderTierVersion, so the legacy tier migration in startServer rebuilt the openai row from the seed and dropped any upstreamWebsocket value. The cases now declare OPENAI_PROVIDER_TIER_VERSION plus upstreamWebsocket: false.

Two new test helpers back this up. tests/helpers/websocket-dial-guard.ts makes any case that constructs a WebSocket fail deterministically. tests/helpers/leftover-servers.ts stops whatever a timed-out case left running. Bun abandons a timed-out case without running its finally, and its live server's spend-ledger owner then made every later startServer in the file throw SPEND_LEDGER_OWNER_HOME_CONFLICT, which explains the ten cascade failures.

Verification

All runs used Bun 1.4.4 in env -i with temporary HOME/OPENCODEX_HOME/CODEX_HOME/CLAUDE_CONFIG_DIR.

  • Launcher flake: 8 parallel workers × 60 runs of the releases its mutex after forwarding case. Before: 22/480 failed (launcher killed by SIGTERM, Desktop sh left orphaned). After: 0/480.
  • New regression test a signal that lands before spawn returns is forwarded to the child once it exists fails on the old ordering (killed is []) and passes on the fix.
  • chat-completions hang reproduced deterministically: HTTPS_PROXY pointed at a local listener that accepts and never answers. Old file: the first case that dials a WebSocket times out at 5000 ms, and 16 fail in the same SPEND_LEDGER_OWNER_HOME_CONFLICT cascade as CI. New file: 122 pass / 0 fail under the same blackhole.
  • Cascade isolation, with a forced hang in the target case: without the tracker, 11 fail (the exact CI set); with the tracker, only the hung case fails.
  • Guard check: with upstreamWebsocket: false removed, the guard fails exactly the 10 dialing cases.
  • bun run typecheck: no type errors. bun run structure:check, bun run privacy:scan: pass.
  • Layout, file-size ratchet, Lab boundary, structure SSOT, and repo-hygiene guards: 136 pass / 0 fail.
  • Source-oracle files that read bin/ocx.mjs or the handoff module (44 files): all pass except three codex-cli-update-apply "published Node launcher" cases, which fail identically on unmodified dev in this sandbox.
  • bun run test:changed: 193 pass, 1 fail. The failing case, update-stop-first › "npm launcher restarts the stopped runtime after a staged update failure", fails identically on the parent dev commit in this sandbox ("service registration ownership could not be verified") and passes under the env -i harness.
  • bun run test (full suite, 697 s, sharing the machine with another worktree's test run): 12 failures, all in real-npm, packed-launcher, and compiled-CLI cases. Rerunning those 8 files on the parent dev commit fails the same 12 (plus one more), so none come from this change.
  • NOT RUN: the update-mutation lint tables (update-mutation-guard "known-pattern" cases) skip on POSIX because the sync TypeScript parser does not start here. The table rows were updated by hand and are left to Windows CI.

Checklist

  • Scope stays focused and avoids unrelated cleanup.
  • Docs or release notes were updated when needed. (structure/desktop-terminal-command.md documents the forwarding order.)
  • Security-sensitive changes were reviewed for secrets, auth, and unsafe defaults.

Summary by CodeRabbit

  • Bug Fixes
    • Improved launcher signal handling so interruptions are forwarded to the child process, including signals received while the child is starting.
    • Reduced the chance of a child process continuing to run after the launcher is interrupted.
    • Ensured signal handlers are cleared when the child process exits or fails to start, helping prevent stale handlers from affecting later operations.
  • Documentation
    • Clarified how launcher signals are handled during startup.

The Node launcher registered its SIGINT/SIGTERM/SIGHUP forwarders only after
spawn() returned. The child could exec and print output in that window, so a
signal sent to the launcher took the default action: the launcher died, its
sync-mutex exit cleanup never ran, and the child was orphaned without seeing
the signal. Forwarders now come from src/lib/launcher-signal-forwarding.mjs and
are installed before both the Desktop handoff spawn and the Bun child spawn;
a signal observed before the child is attached is delivered once it exists.

tests/responses/chat-completions-endpoint.test.ts depended on the live network:
streamed canonical ChatGPT turns dial wss://chatgpt.com first, which the fetch
mocks cannot intercept, and wait up to the 10 s upgrade deadline before the
HTTP fallback. Its configs also lacked openaiProviderTierVersion, so the legacy
tier migration dropped any upstreamWebsocket opt-out at startServer. The cases
now declare the current tier version and upstreamWebsocket: false, a guard
fails any case that constructs a WebSocket, and leftover servers from a
timed-out case are stopped in afterEach so one hang cannot fail every later
startServer with SPEND_LEDGER_OWNER_HOME_CONFLICT.

Explain Change →

Walkthrough

The launchers now use shared signal-forwarding logic that queues signals received before child attachment. Chat completion endpoint tests add server cleanup and WebSocket dial guards, and update OpenAI fixture settings.

Changes: Launcher Signal Forwarding

Layer / File(s) Summary
Signal forwarding contract and behavior
src/lib/launcher-signal-forwarding.mjs, src/lib/launcher-signal-forwarding.d.mts
The module selects signals by platform, registers parent listeners, queues signals until child attachment, and provides cleanup.
Launcher integration and validation
bin/ocx.mjs, src/lib/desktop-cli-handoff.mjs, structure/desktop-terminal-command.md, tests/cli/ocx-launcher-desktop-handoff.test.ts, tests/cli/ocx-launcher-source.test.ts, tests/update/update-stop-first.test.ts, tests/helpers/update-helper-control-contracts.ts, tests/helpers/update-import-policy.ts, tests/helpers/update-primitive-allowlist.ts
Both launch paths initialize forwarding before spawning and clear it on errors or completion. Tests cover a signal received before spawn() returns. Documentation and update-helper checks reflect the forwarding structure.

Chat Endpoint Test Isolation

Layer / File(s) Summary
Endpoint test setup and fixtures
tests/helpers/leftover-servers.ts, tests/helpers/websocket-dial-guard.ts, tests/responses/chat-completions-endpoint.test.ts
Endpoint tests track servers for cleanup and guard against WebSocket dials. OpenAI fixtures use the provider tier version constant and disable upstream WebSockets.

Address review: a source-order assertion keeps forwardLauncherSignals() ahead
of the Bun child spawn, afterEach restores the real WebSocket before any cleanup
step can throw, and a duplicated openaiProviderTierVersion key is removed.

Track servers until shutdown settles
