Imported Claude Code and Codex threads now use the provider's session name and stay in step with the running terminal session.

  • Builds on upstream fix(server): preserve Codex titles on import pingdotgg/t3code#11919: Codex titles come from ~/.codex/session_index.jsonl, and injected context is skipped.
  • Claude titles use the latest custom-title record (from /rename), then ai-title, then the first visible user message.
  • AgentSessionSync runs 5 s after startup and then every 5 minutes. Runs are serialized and coalesced. It rescans projects that already have imported threads, adds new messages and new sessions, and updates provider titles.
  • Threads with activity started in T3 (messages, turns, a session or plans) and titles renamed in T3 are never changed.
  • New messages go through thread.history.append. They keep stable imported message IDs, so a rerun adds no duplicates, and they never settle a thread. Transcripts whose file identity hasn't changed are skipped.

Verification: apps/server vp test run src/project src/orchestration/decider passes 294/294 and tsc --noEmit is clean. Contracts tests pass 61/61.

Use Codex session index titles when available, fall back past injected context, and repair legacy placeholder titles during a repeated import.

Closes pingdotgg#10513
Bound session-index reads, preserve concurrent manual renames, recover fallback titles from imported history, and isolate repair failures.
