Fixes #15353.

On orchestrator v2, a Claude thread in Auto never asks about anything. Commands that Claude escalates for a human decision, such as a git push matching a user permissions.ask rule or a PreToolUse hook returning ask, run without a prompt, the same as Full access. The permission modes guide says Auto approves routine actions "and asks about others".

Cause

In auto mode, Claude approves routine actions with its classifier and sends the rest (ask-rule matches, hook asks, protected paths, classifier escalations) to the SDK canUseTool callback. The adapter's callback answers allow whenever requiresClaudeApproval is false, and installPermissionCallback was true only for Supervised and Auto-accept edits. So every escalation in Auto was approved without raising a request. The V1 adapter asked in every mode except Full access. #13786 fixed the same gap for Auto-accept edits.

Fix

  • The callback now asks in every mode except Full access. Auto still lets Claude's classifier approve routine actions before the callback runs, so only escalations reach the user. Full access and explicit approvalPolicy overrides are unchanged.
  • The approval prompt shows why it was raised. The SDK nearly always sets title, so a hook's permissionDecisionReason (the SDK's decisionReason) was dropped. It now appears on its own line under the title, with ANSI escapes stripped.
  • The web composer's approval panel wraps command prompts instead of hiding long lines off to the right. Mobile already wrapped.

Behavior change: scheduled, webhook and delegated threads in Auto now pause at escalations until someone answers, as Supervised already does. A "deny when nobody is attached" option could follow.

Pretool hook reason:
Before:
image

After:
image

Verification

  • vp test run apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts: 260 passed.
  • New tests drive the real callback through a stubbed query runner:
    • Auto raises a command request for an escalated git push, and accepting it allows the command.
    • Auto denies the command when the request is declined.
    • Auto shows a hook's escalation reason under the request title, without ANSI escapes.
    • Full access allows the command without raising a request.
    • Auto-accept edits still asks before a command (existing test, moved to a shared helper).
  • With the source change reverted, the Auto tests fail.
  • apps/server and apps/web typecheck pass.
  • Manually checked on web with a PreToolUse hook returning ask with a reason: the approval panel shows the title and the reason.

Fixed with Claude Opus 5.5 in Claude Code, run through T3 Code.

Merge Risk: 🟡 Moderate · up to 09817

Accepting one escalated Bash command for the session can allow unrelated later commands without another approval. Limit that grant before merging.

Security architecture risk: 🔵 Low · up to 09817

Auto now asks before allowing escalated actions, strengthening the approval boundary. The change is narrowly scoped, but end-to-end permission enforcement and cleanup remain partly unverified.

Retained concerns
No architecture-level concerns identified.

Security review details

Security Blast Radius

  • inferred — The changed control applies to escalated tools in Auto sessions without an explicit approvalPolicy. Bash approvals authorize command input within the existing query environment and working directory. Maximum credential, filesystem, network and downstream-service exposure cannot be determined from the inspected approval path.

Security Findings and Attack Paths

  • observed — The supplied security candidate is deferred because verification lacks a valid receipt bound to its exact evidence. Its attacker source and outcome descriptions are generic, so it does not establish an exploitable attack path. Local inspection confirms stronger callback gating but does not resolve the external SDK proof gaps.

Trust Boundaries and Controls

  • observed — Response admission checks that the projected request is pending. Execution checks its live capability against the provider session and dispatches the exact request ID. The adapter requires an approval decision and resolves a one-shot deferred value. These are request/session binding controls, not proof of end-user authentication or authorization at the external entrypoint.

Resilience and Maintainability Implications

  • inferred — Approval waiting races a decision against signal abortion and cleans up when that wait terminates. Query interruption and session closure rely on the provider to propagate abortion; inspected termination paths do not explicitly settle pending approvals. Auto newly inherits this lifecycle dependency, but no late execution or fail-open outcome is established.

Hardening Proposals

  • proposed — Validate the provider-boundary contract using the real permission evaluator: explicit ask-rule routing, whole-tool session approval followed by unrelated commands, continued ask/deny precedence, and pending-approval cleanup after interruption or query closure. These checks would resolve current uncertainty rather than demonstrate an already verified vulnerability.

Approvability

Verdict: Needs Human Approval

Macroscope's review found this PR not approvable — This PR changes Claude Auto mode's production permission-control behavior so escalated commands pause for approval, alongside related prompt and UI changes. A concrete unresolved risk remains around session-wide Bash permission grants, making the security-sensitive blast radius unsuitable for automatic approval.

Thanks for picking this up. The approach looks right to me. runtimeMode !== "full-access" also means any mode added later will ask by default, which is the safe direction. A few notes from reading it against current main and the pinned Agent SDK (^0.3.276):

1. The rebase conflict is small. The source file merges cleanly. In the test file, main has since added applyFlagSettings: () => Effect.void to the stubbed query session, so openClaudePermissionTurn needs that line too. I'm happy to push the rebase if that helps.

2. A hook's reason doesn't reach the user. #15353 is about a PreToolUse hook returning ask. The request prompt is built as title ?? description ?? decisionReason ?? …. The SDK nearly always sets title ("Claude wants to run …"), so the hook's reason in decisionReason gets dropped. The user sees an approval request but not why it was raised. The SDK describes that field as the reason the ask escalated, meant "for the consent line of the host's dialog", and notes it may contain ANSI escapes, so sanitize it before rendering. Could the prompt show it alongside the title?

3. Suggested test for the hook path. The new Auto tests send a plain git push through the callback. That proves the callback asks, but nothing covers the hook case from the issue. A test that passes decisionReason in the callback options and checks it appears in the raised request would cover #15353 directly, and point 2 too if it's addressed.

4. "Accept for session" in Auto (could be a follow-up). After this change, the requests a user sees in Auto are the ones Claude declined to auto-approve. The SDK passes suppressAlwaysAllowRule and defaultToNo for these, but the adapter doesn't read either. Also, when there are no suggestions, toSessionPermissionUpdates falls back to a whole-tool rule ({ toolName }). So accepting one escalated Bash command for the session allows every Bash command after it. That isn't new, but this PR is what starts putting these requests in front of Auto users. If it's out of scope here, it deserves its own issue.

5. Unattended runs (worth a line in the description). An approval only ends when someone answers or the SDK aborts the call. Scheduled, webhook and delegated threads in Auto will now pause at escalations instead of running through them. Supervised mode already works this way, so I think that's correct, but it changes how Auto behaves. If you want a follow-up option: without a canUseTool callback the SDK simply denies asks, so "deny when nobody is attached" would be easy to build.

🌐 Web query:

official Claude Agent SDK canUseTool PermissionUpdate addRules rules toolName session destination

💡 Result:

Yes. In the Claude Agent SDK, a `canUseTool` callback can return an allow decision with `updatedPermissions`; an `addRules` update has `rules`, a `behavior` (`allow`, `deny`, or `ask`), and a `destination`. Each rule uses `toolName` and optionally `ruleContent`. The documented destinations include `session`, `userSettings`, `projectSettings`, and `localSettings`; `session` applies only to the current session. ([github.com](https://github.com/seilk/claude-code-docs/blob/main/en/reference/sdk/permissions-api.md?utm_source=openai))

```ts
return {
  behavior: "allow",
  updatedPermissions: [{
    type: "addRules",
    rules: [{ toolName: "Bash", ruleContent: "git status" }],
    behavior: "allow",
    destination: "session",
  }],
};
```

The example is illustrative—not a quoted SDK excerpt. The cited permissions guide documents the wire format; the SDK’s Python implementation confirms the corresponding fields and destinations, but does not establish the exact TypeScript API for a particular package version. 🌐 Web query:

site:github.com/anthropics/claude-agent-sdk OR site:docs.anthropic.com "ruleContent" "addRules" Bash permission session omitted

💡 Result:

The search results do **not** establish that `ruleContent` or the `destination: "session"` field can be omitted from an `addRules` update. The SDK’s current Python test shows a Bash rule with `ruleContent: "git status"` and a destination of `"localSettings"`—not `"session"`. ([github.com](https://github.com/anthropics/claude-agent-sdk-python/blob/main/tests/test_tool_callbacks.py?utm_source=openai))

A Claude Code issue includes a logged `addRules` update with `destination: "session"`, but it’s a bug report, not API documentation. (GitHub issue 52748 in anthropics/claude-code (link omitted to avoid creating a cross-reference)) The precise behavior when either field is omitted remains unresolved by these results.

Do not persist a tool-wide Bash rule without an SDK suggestion.

When Auto accepts an escalated Bash request for the session and suggestions is empty, toSessionPermissionUpdates creates { toolName: "Bash" }. Claude SDK 0.3.276 treats that as a bare tool rule, so later unrelated Bash commands can bypass approval for the session. Keep the existing fallback for custom tools, but do not use it for Bash.

Thanks, this was useful.

  1. Rebased onto current main, with applyFlagSettings added to the stubbed session.
  2. Done in beb7427. The prompt now shows decisionReason on its own line under the title, with ANSI escapes stripped. The web approval panel also wraps command prompts now, since it was hiding long reasons off to the right. I checked it on web with a PreToolUse hook returning ask with a reason.
  3. Added a test that passes title and an ANSI-coloured decisionReason through the callback in Auto and checks the raised prompt.
  4. Agreed it's real and predates this PR, but it's out of scope here. Could you open a separate issue for it?
  5. Added a note to the description. Unattended Auto threads now pause at escalations, as Supervised does.

The prompt is still one string, though, so the panel can't tell Claude's label, the command and the reason apart. The command itself is also often missing, because the SDK's title is usually Claude's own label ("Commit the test edit"). Fixing that properly means separate fields on the approval item and changes on web and mobile, which is out of scope here. Feel free to take it on in a follow-up PR.

The SDK nearly always sets a title, so the reason an ask escalated, such as
a PreToolUse hook's permissionDecisionReason, was dropped from the prompt.
It now follows the title on its own line, with ANSI escapes stripped.

The composer approval panel wraps command prompts instead of hiding long
lines off to the right.
