Codex snapshots the model-facing tool catalog when turn/start is sent. Servers still starting are left out, so a turn that begins during MCP startup sees a partial catalog. Hold turn/start until each reported server leaves starting or cancelled, on a wall-clock bound. A later ready replaces a spurious cancelled status. A timeout skip stays on that thread and is dropped when a newer notification arrives, so it cannot hide a later global startup status.

🟠 High Adapters/CodexAdapterV2.ts:5567
Pressing Stop while mcpStartup.wait(threadId) is blocked does not prevent the delayed startTurn from sending turn/start, so the stopped turn can begin up to 30 seconds later. Because the turn is not yet in pendingRootTurns or activeTurns, interruptTurn(..., requestRuntimeRestart: true) finds nothing at lines 5698–5702 and returns successfully; track or register the pre-start turn before waiting so the interrupt cancels the later request.

ApprovabilityVerdict: Not approved Macroscope's review found this PR not approvable — This production change adds a substantial asynchronous MCP startup gate that changes existing Codex turn timing and introduces static-analysis suppressions. An unresolved high-severity concern indicates that stopping during the wait may still allow a delayed turn to start, so the change needs human review. Not approved because:

5559-5567: 🎯 Functional Correctness | 🔵 TrivialA pending MCP startup can delay
startTurnby 30 seconds and no caller can see it.
mcpStartup.wait(threadId)runs beforeturn/start. During the wait, the adapter emits no provider event. If a server staysstartingorcancelled, the run appears stuck in pending for up to 30 seconds. The timeout logs a warning on the server only. The linked issue asks for "a clear startup or incomplete-inventory state". Consider these changes:
- Emit a status event while the wait is in progress.
- Use the
timedOutresult to surface the incomplete catalog to the UI.If an interrupt arrives during the wait, the wait stops correctly.

Superseded: Macroscope flagged a blocking issue (Stop during the MCP startup wait can still let the delayed turn start). Redoing from scratch on a fresh branch from main. |
What Changed
CodexAdapterV2holdsturn/startuntil every MCP server that has already reportedstartingorcancelledreachesreadyorfailed. The wait is one deferred raced with a single 30-second wall-clock timer. The timer state lives only on that provider session.A timeout records a per-thread skip for the servers that were still pending. A newer notification for one of those servers drops only that skip. Thread-specific phases stay in their own map, so a later global
startingorreadyis still visible.cancelleddoes not replaceready. A laterreadyreplaces a spuriouscancelled.Codex replay transcripts that settle a server move that server’s startup lines to just before the
turn/startthat needs them. The JSON lines themselves are unchanged. Why
Fixes #13437.
This supersedes #15528 (Macroscope Not approved). Codex builds the model-facing tool catalog when
turn/startis sent. A server still instartingis left out after a short optional grace, so a turn that begins while stdio MCP servers are coming up sees a partial catalog. The wait is keyed by server name and stays bounded. A spuriouscancelledbeforereadydoes not close the wait.Verification
