fix(server): run event subscriptions close when a turn never starts - #16573

Conversation
When the provider turn is declined by the last ownership check or rejected by the provider right away, the ingestion fiber is interrupted before it first runs. Effect then skips the fiber's body, including the finalizer that closes the event subscription, so the session keeps queueing events for a subscriber nobody reads. Close the subscription after interrupting the ingestion fiber as well, and cache the close so a fiber that did start still closes it only once. ApprovabilityVerdict: Approved at Macroscope's review found this PR approvable — This is a focused server lifecycle fix that preserves normal detached event ingestion while reliably closing subscriptions on declined, rejected, failed, or interrupted startup. Targeted tests cover the new cleanup paths and exactly-once behavior, with no schema, deployment, security, billing, or configuration impact. You can add or adjust custom eligibility rules. Taking over this one for batch server-misc-02. I will preserve your commits, merge current main, rebuild the minimal cleanup regression with the current test context, and run independent reviews. The PR will remain unmerged. |

- Around line 1393-1395: Update startRootRun’s startup continuation so
interruption while awaiting mcpAppModelContext.forThread closes
closeEventSubscription, but only until provider-turn startup succeeds; preserve
the explicit stopProviderEvents paths and cached exactly-once close behavior.
Add a regression test through ProviderTurnStartService or an equivalent
interruptible caller that blocks forThread, interrupts the parent startRootRun
fiber, and verifies the subscription closes once.

A provider event subscription leaked when a turn was declined or rejected before its ingestion fiber first ran: interrupting an unstarted fiber skips its finalizer. Cancellation during startup could also leave detached ingestion running. Startup now owns ingestion until the provider accepts the turn, and every cleanup path shares one cached close operation. Successful turns retain their detached ingestion lifetime.
Maintainer takeover of @yaacovcorcos's #16573, preserving contributor history and merging current main. Rebuilt the tests on the existing termination fixture and current adapter/MCP context instead of retaining the original standalone setup.
Validation: main passes 54 tests. The two early-exit regressions fail against unchanged production with close count 0 instead of 1. A further Deferred-driven regression reproduces caller cancellation while the MCP context read is pending; it fails with the initial fix and passes after startup cleanup is bracketed. All 57 tests now pass. Existing tests also check exactly-once cleanup after ingestion starts and normal completion. Changed-file lint, formatting, and scoped server typecheck pass.
Two independent full-diff review rounds by Claude Opus 5.5 and Codex GPT-6-Sol found no blockers. Between rounds, CodeRabbit identified the startup cancellation leak; it was reproduced, fixed, and reviewed in the second round.
Proof is headless Linux service testing. No live provider, real client, memory measurement, remote/tunnel flow, web, macOS, Windows, iOS, Android, or Electron desktop was exercised. Screenshots do not demonstrate this resource-lifecycle fix.
