Problem

With the Working section on, a thread left it too early while the agent was still waiting on a background command, like tests or a CI wait. It also stayed stuck there after the agent finished but left a dev server running with a watchdog Monitor. Both came from deciding by task kind alone: commands never held the thread and monitors always did. But either kind can be a wait the agent will resume from, or a process it left running.

Change

  • The agent says what it waits on. A new MCP tool, t3_background_wait, marks background tasks the agent will act on when woken (awaited: true). waiting: false clears the mark. IDs that are not running come back in unmatched and change nothing.
  • One hold rule. In backgroundWorkHoldsCompletion, a task holds if it is marked awaited or is a subagent or background task. Unmarked commands and monitors never hold, so a dev server or its watchdog no longer pins a thread. PR watches are marked awaited, so they hold as before.
  • Same rule everywhere. That one rule drives:
    • Working on web and mobile
    • completion alerts
    • auto-settle
    • restart wake (RestartBackgroundNote)
  • Claude implements it on its roster. The mark lives on the adapter's roster entries:
    • It is dropped when the task ends, on roster REPLACE, or when the process resets.
    • It is serialized under sdkMessageLock.
    • The system prompt says when to call the tool. Without that, Claude would only see the tool's name, because Claude Code defers MCP tools behind ToolSearch.
  • Other providers return "unsupported" and keep today's behavior. Their background lists only ever hold subagents, and those still hold.

Edge cases checked:

  • Snooze: unchanged by this PR. Since fix(threads): snoozed threads stay hidden through background wakes #22202, a finished run never ends a snooze, so awaited work needs no extra snooze rule.
  • Settle: manual settle is unchanged. Auto-settle skips threads with awaited work.
  • Stop or interrupt, process exit or replacement: marks clear with the roster.
  • Subagents, native and delegated, and background agents: they still hold without being marked.
  • Subagent-owned shells (parent_task_id): they don't hold. Claude's schema says the parent "may be a foreground or already-ended task", so holding them would bring back the stuck bug for a dev server a subagent starts. While a subagent waits on its own shell, it is still running, and that already holds. Restart recovery (fix(server): wake threads whose subagents' background work a restart cut #19750) still reports that work as the subagent's.
  • Older clients and servers: awaited is an optional field.
  • Claude's native awaited and ambient roster flags: these mean something else, and T3 reads neither.

Scope and approval

Theo approved opening this PR for this fix. It changes only when background work holds a thread, plus the one tool agents use to declare a wait.

Verification

Live check: two dev apps, one on main and one on this branch, each with copied data and the Working section on. Claude Opus 5.5 got the same prompts in both. Other thread titles are blurred.

1. Tests in the background: "run sleep 90 && echo ALL 42 TESTS PASSED in the background, end your turn, tell me the result."

  • Before: the thread is back in the inbox while the command still runs.
  • After: Claude calls t3_background_wait on its own. The thread stays in Working until it reports the result.
Before (main) After, while waiting After, with the result
main: back in the inbox while tests run branch: in Working while tests run branch: back with the result

2. Dev server left running: "start python3 -m http.server and a Monitor watching it for errors, leave both running, you're done."

  • Before: the thread is stuck in Working, "Waiting on 1 command and 1 monitor".
  • After: it comes back to the inbox while the server keeps running. Claude didn't mark it.
Before (main) After
main: stuck in Working branch: back in the inbox

Tests and checks:

  • vp test run, all passing:
    • shared orchestrationV2PendingBackgroundWork and agentAwareness
    • client-runtime entities, threadExecution and t3ToolSummary
    • server ClaudeAdapterV2, ThreadManagementService, ThreadSettlementService, RestartBackgroundNote, RestartContinuation, ProjectionStore, DelegatedCompletionDelivery, runtimeLayer, AgentAwarenessRelay and src/mcp
    • web Sidebar.logic
    • mobile threadListV2
  • Typecheck is clean for contracts, shared, client-runtime, provider-core, server, web and mobile. vp check (lint and format) and knip pass.

background-wait summaries say background work was marked as awaited even when a selected call has input.waiting === false, so the work log reports the opposite of the operation performed. Derive the label from the selected calls’ input.waiting values, using a cleared label for false and a neutral update label for mixed batches.

Fixed in 4b4d183. The single-call and grouped labels now say "Updated background waits", which is true for both waiting=true and waiting=false. That's smaller than deriving the label from each call's input, and it never misreports.

Approvability

Verdict: Needs Human Approval

Macroscope's review found this PR requires human approval — This PR introduces a new background-wait capability and changes default thread behavior across orchestration, settlement, notifications, and web/mobile Working lists. In particular, background commands and monitors now require explicit marking to keep a thread active, making this a broad product-behavior change rather than a narrowly scoped fix.

📝 Walkthrough

The change adds t3_background_wait and explicit awaited state for background tasks. Provider rosters, completion handling, restart behavior, thread status, notifications, and documentation now distinguish awaited work from independent background work.

Changes: Explicit background-task waiting

Merge Risk: ⚪ Minimal · up to 6d2e1

The mixed-task title reflects all active work, including work the agent is not awaiting. No merge-blocking issue remains identified.

Security architecture risk: 🔵 Low · up to 6d2e1

Waiting changes are limited to existing work in the calling thread. No new cross-thread access or execution privilege was demonstrated, but concurrent updates and restart compatibility have limited verification.

Retained concerns
No architecture-level concerns identified.

Security review details

Security Blast Radius

  • inferred — The demonstrated scope is matching existing tasks on the calling thread's selected provider session. A permitted caller can influence that thread's completion visibility and restart continuation, but the tool supplies no arbitrary target-thread or session identifier.

Trust Boundaries and Controls

  • observed — The handler inherits the existing thread-scope, read-only-client rejection, and live-caller controls, then requires orchestration capability through readCaller. Provider selection checks app-thread ownership, and the adapter rejects unmatched task IDs without mutation. Initial credential-scope issuance was outside the inspected path.

Resilience and Maintainability Implications

  • inferred — The unknown-kind schema fallback drops awaited metadata, but normalizes the record to background_task, which always holds completion and maps to restart-waking task work. This does not establish a weakened completion or recovery guarantee for unknown kinds.

Same case here on desktop 0.1.1-nightly.20261020.3411 (macOS), Claude Opus 5.5. The agent started a ~25-minute video render with run_in_background and ended its turn saying it would check the exit status and report the path when the render finished. The composer shows Running: Render and master the YouTube video in the background · Stop, but the thread sits in the inbox and isn't marked Working.

A read-only copy of the database shows the provider thread idle with one roster entry, { kind: "command", description: "Render and master the YouTube video in the background" }. So it's the awaited-command case this PR covers: backgroundWorkHoldsCompletion returns false for command.

A thread left Working while the agent was still waiting on a background
command (tests, a CI wait), and stayed stuck in Working while a dev-server
watchdog Monitor ran. Both came from deciding "the agent will come back" by
task kind: commands never held and monitors always did, but either can be a
wait or a process left running.

Agents now mark the background work they will resume from with the
t3_background_wait MCP tool. Marked tasks hold the thread in Working, block
completion alerts and auto-settle, and wake the agent after a
restart. Unmarked commands and monitors do neither. Subagents and PR
watches hold as before. Claude implements the mark on its roster entries,
so it ends with the task and resets with the process, and its system prompt
says when to call the tool, since Claude Code defers MCP tools behind
ToolSearch. Other providers report the tool as unsupported.
