Problem

V2 copies V1's state.sqlite into statev2.sqlite once, then reuses that file on every later start. If the copy came from a preview build, a nightly run once, a rolled-back server update, or a channel switch back to V1, everything the user did in V1 afterwards never shows up in V2. Missing items include new projects, threads and messages. Restarting does not help and nothing is logged.

Repro on main (8d3b061121), with real apps sharing one T3 home:

  1. t3@0.0.45 (V1) with --auto-bootstrap-project-from-cwd projA creates project A and its thread.
  2. Main starts and seeds statev2.sqlite.
  3. t3@0.0.45 runs again from projB and creates project B and its thread.
  4. Main restarts: V1 has 2 projects and 2 threads, V2 still has 1 of each, and no warning is logged.

Change

The server now replaces a stale copy when two things are both true:

  • V1 has newer data: V1 has a project or thread that the copy lacks, or one with a later updated_at.
  • Nobody has worked in the V2 copy yet.

When both hold, the server moves the copy aside to statev2.sqlite.stale and copies V1 again.

  • What counts as V2 work. Work is judged by content, not by events, because the server writes its own events within a minute of starting (auto-settle, PR sync). Any of these counts:

    • a thread, message, project or scheduled task that did not come from V1;
    • a session or pairing link that was revoked or consumed in V2 but is still open in V1. This keeps a revocation made in V2 from being undone.

If V2 holds work, it is kept and the server logs a warning.

  • Where it runs. Only the server does this, right after it takes the T3-home ownership lock (replaceStaleV2Database in server.ts). CLI processes that open persistence, such as t3 pair and t3 project, keep main's seed-only-when-missing behaviour, so they never move a database another process has open.

  • How the copy is moved. The server checkpoints the WAL into the copy, then renames the main file. Any failure logs a warning and keeps the existing database, so a locked file never blocks startup. Exactly one backup is kept.

  • state.sqlite is only ever opened read-only.

  • The user and internal migration docs now describe when V2 copies V1 again and what that copy replaces.

Scope and approval

Fixes #15017

Triaged bug via the automated issue flow. Four more reporters on the issue hit the same trap on stable 0.1.0 and on nightlies, through previews, rollbacks, channel switches and rolled-back server updates.

Verification

  • vp test run apps/server/src/persistence/initializeV2Database.test.ts apps/server/src/orchestration-v2/legacy/: 16 passed.
    • The reproduction test re-seeds a migration-only V2 store when V1 gained threads after the copy failed on main with expected [ 'thread-a' ] to deeply equal [ 'thread-a', 'thread-b' ] and passes now. It also covers visit and server auto-settle events not counting as work, the single backup, and a silent second start.
    • Re-seed when V1 only updated an existing thread.
    • A table of keep cases: a V2 thread without messages, a V2 message, a V2 scheduled task, a V2 project, a session revoked in V2, and a pairing link consumed in V2. I removed the revocation and consumption checks one at a time, and each made only its own row fail.
  • vp lint on the touched files is clean. The apps/server typecheck (tsc --noEmit) exits 0.
  • Runtime transcript: the same real-app sequence as the repro, with this branch at step 4.
    WARN: The V2 database predates newer V1 data and holds no V2 work; moved it aside and copied V1 again { backupPath: '…/userdata/statev2.sqlite.stale' }
    INFO: Imported legacy v1 thread shells { importedThreadCount: 2, importedMessageCount: 0 }

V2 now lists both threads, c4672dfd… (project B) and f56663e5…. A second start logs nothing and leaves only statev2.sqlite.stale. Running node apps/server/src/bin.ts pair against a stale home leaves statev2.sqlite in place.
  • Server-only change with no UI, so there are no before/after captures.
  • Not checked: a real Windows run. The rename-failure path is only read in code: it logs and keeps the existing database.

…newer V1 threads

Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
statev2.sqlite left behind by a preview, rollback or channel switch was
reused forever, so projects and threads created in V1 afterwards never
appeared. When V1 has a project or thread missing from, or newer than, the
copied legacy tables and the V2 store holds no V2-native events, move the
stale copy (and its WAL/SHM) aside and snapshot V1 again. A V2 store with
V2 work is kept, with a warning that newer V1 data is not imported.

…ontent

The V2-work check now looks at content instead of events: threads or
messages beyond the copied legacy tables, projects missing from V1, or
scheduled tasks missing from V1. Server-written events such as auto-settle
no longer block a re-seed, and event-less scheduled tasks are kept.

The replacement moved out of SqlitePersistence.layerConfig into
replaceStaleV2Database, called right after the server acquires ownership,
so CLI processes never move the live database. It keeps one
statev2.sqlite.stale backup and renames -wal/-shm before the main file.
Docs describe the re-copy.

Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
A device revoked or a pairing link closed in V2 now counts as V2 work, so a
re-copy cannot reopen it from V1. The stale copy is checkpointed before it is
moved, and a failed move logs a warning and keeps the existing database
instead of failing startup.

🟠 High persistence/initializeV2Database.ts:83

Disabling or deleting an imported V1 task in V2 does not count as V2 work, so when V1 later gains a newer thread, replaceStaleV2Database replaces the database and restores the task's original enabled state. hasIdsBeyond only detects task IDs added in V2; compare task updates and removals too before replacing the copy.

Approvability

Verdict: Needs Human Approval

Macroscope's review found this PR requires human approval — This PR adds automatic server-startup detection and replacement of stale V2 SQLite databases, including persisted projects, tasks, threads, and authentication state. Because the replacement is heuristic and can discard or restore stored user state, the change warrants human review.

Re the scheduled-task finding: not applicable. No V1 release has a scheduled_tasks table (none in v0.0.45). The table is created only by the V2 migration Migrations/OrchestrationV2/ScheduledTasks.ts. So there are no "imported V1 tasks": when V1 lacks the table, hasIdsBeyond counts every V2 task row as V2 work. A task disabled in V2 therefore keeps the store, and a task deleted in V2 cannot be restored from V1, which never had it.

📝 Walkthrough

The server now checks an existing V2 database during startup. If V1 has newer project or thread data and the V2 copy has no detected V2-only work, the server preserves the old copy as a stale backup and imports V1 again.

Changes: V2 database refresh

Layer / File(s) Summary
Classify the V2 database copy
apps/server/src/persistence/initializeV2Database.ts
Compare project and thread shells in V1 and V2. The inspection classifies copies as current, stale, or diverged based on newer V1 data and detected V2-only work.
Replace stale copies and test the outcomes
apps/server/src/persistence/initializeV2Database.ts, apps/server/src/persistence/initializeV2Database.test.ts
For a stale copy, checkpoint the database, move it to a .stale backup, and initialize V2 from V1. Tests cover stale-copy replacement, backup cleanup, and V2 work that prevents replacement.
Wire the check into startup and document migration
apps/server/src/server.ts, docs/internals/legacy-orchestration-migration.md, docs/user/thread-migration.md
Run the check after the server acquires ownership. Update migration guidance to describe when V2 recopies V1 and retains the prior database.

Explanation

The description includes all required sections. It clearly explains the problem, implementation, scope, verification results, limitations, and agent usage. It also references the tracked issue and provides detailed test evidence.

Full details: Linked Issues check

Explanation

For #15017, the implementation detects newer V1 project or thread shells by ID and updated_at. It re-copies V1 when the V2 copy has no V2-specific threads, messages, projects, scheduled tasks, or protected V2 auth revocations. It preserves divergent V2 data and logs a warning. The server calls this check after ownership acquisition, while CLI initialization keeps seed-only behavior. Tests cover new and updated V1 data, V2 keep cases, WAL checkpointing, backup retention, and repeated startup.

Full details: Out of Scope Changes check

Explanation

The changes remain within #15017. The server integration enables stale-copy detection, the tests verify migration and preservation behavior, and the migration documents explain replacement and retention rules. No unrelated product behavior or UI changes are shown.

🗄️ Data Integrity & Integration | 🟡 Minor | ⚡ Quick win

Log inspection failures before keeping the V2 copy.

An inspection failure does not prove that the V2 copy is stale, so retaining it is a safe fallback. However, the current fallback is silent. If the copy is stale, newer V1 projects, threads, messages, or status changes can remain unavailable until a later inspection succeeds. This is an operational data-freshness risk, not only a logging cleanup.

Document authentication state as V2 work.

The current guard preserves V2-revoked sessions and revoked or consumed pairing links. The paragraph’s exhaustive-looking V2 work list omits this security invariant. Without it, a maintainer may remove the guard as unnecessary. This is a documentation-contract violation and a future security-regression risk, not a current authorization bypass.
