The method assertion already rejects a newest-entry implementation because the newest entry is

Make the writer perform valid inserts.
Both writer fibers use columns that do not exist in orchestration_v2_projection_checkpoints. SQLite rejects each insert, and Effect.ignore hides the failures. The 400 iterations therefore create no write load.
Use the schema-defined columns, generate unique scope_id values for each iteration, remove Effect.ignore, and assert that the expected rows were written before using the measurements.
The load test asserts the reader connection and span count, but not the lock-wait value. The writer comparison test asserts only the writer connection. Neither test proves the claimed wait behavior.

Restore the remaining messages in reverse iteration order.
If another send starts while a drained batch contains multiple unsent messages, each restore inserts at the front. Restoring the remainder in forward order changes [first, second] to [second, first]. Iterate over the unsent remainder from last to first.

Keep a queued message until onSend accepts it.
onSend returns void even when a guard refuses the send. For example, releaseSend can drain while isSendBusy is still true. The queued text then reaches the early return at Lines 6331–6368, but the drained message is not restored. A later queued message can also overwrite that text in the composer. Make acceptance explicit and stop the drain when a send is refused.

Release the queue for the route that started the send.
If the user changes routes before an awaited send finishes, useStableCallback invokes the latest releaseSend implementation. That implementation reads the new route’s composerSendQueue, so the old route’s queued messages remain stranded until another release occurs there. Bind the release and drain to the send’s route state. Keep these internally called handlers plain so their async work retains the originating render. As per coding guidelines, “Calls inside the component keep the plain function, so work that resumes after an await still acts on the render it started in.” The path instructions also require that Web UI handler rule.

Preserve attachments when queuing a busy submission.
If the user attaches a file to a message submitted during an active send, the queue stores only queuedText, then clearComposerDraftContent removes the attachment. The later drain sends the text without its file. Store the submission’s full draft content, or leave attachments in a recoverable draft instead of clearing them.

Restore the failed submission before draining queued text.
The capability check at line 6827 cannot have pending queued work because it runs before the first await. The post-upload check for attachmentCapabilitiesAfterUpload.fileBlockReason can fail after await awaitAttachmentUploads(...); a busy submission can be queued during that wait.
Queue admission stores only the new text and clears the original prompt, attachments, and contexts. The later releaseSend() drains that text into the composer, so it does not restore the failed submission. Restore the failed submission from its snapshots, release the in-flight state without draining, and drain queued text only after the restored submission has been accepted. Preserving queued submissions in full would not fix the original submission, and draining after restoration would still overwrite it.

Release the exact command entry.
end(method) can remove a hung same-method request instead of the request that completed. This violates the tracker’s longest-outstanding-command contract. Use an identity-based release function.
The request timeout is also 120 seconds, and tracked.done() runs when the request ends. The repository has no current consumer of stalledCommand, so this is not a major runtime issue. It is a localized correctness fix for the exposed tracker and its future consumers.

The MCP read path is the server's hottest read: every `t3_thread_read` goes
through `getThreadRecords`, and the shell list goes through
`getShellSnapshot` and `getThreadShell`. All three ran on the writer
connection, so a read had to take the write lock.
On a host whose state database is large and whose writer is busy, that is
where the time goes. Measured on such a host, over a ten-minute window:
```
MCP tool calls: 1,857 in ~10 minutes (peak 506 in one minute)
DB lock waits > 100 ms: 1,608 (518 seconds of lock wait in that window)
correlation calls/min vs
total lock wait/min: r = 0.751
sqlite ops by connection:
18,408 (writer, read) <-- reads also go through the writer
234 (writer, write)
```
78.5% of every query in that window ran inside an MCP call. The worst single
lock wait was 6,622 ms, on
`SELECT payload_json FROM orchestration_v2_projection_nodes WHERE node_id = ?`.
The reader connection already exists - `nodeSqliteClient` starts a reader
worker for a file-backed database, and `NodeSqliteClient.readOnly` routes a
read to it. It was barely used: 8 transactions in the window, averaging
497 ms, which is the reader pool starved exactly when the writer is busy.
These three reads are pure reads, so they belong on the reader. The change
moves them and nothing else: the queries, the transaction shape and the
results are unchanged.

The lock-wait measurement's writer inserted rows with columns that `orchestration_v2_projection_checkpoints` does not have, and the insert was wrapped in `Effect.ignore`, so every row was rejected and the "under write load" measurement had no write load in it. The writer now uses the table's own columns, generates a unique scope per row, and does not swallow failures: the test counts the rows the writer reports and counts the rows on disk, and asserts they are the same. Both measurements now assert the lock wait is reported, so the before/after is compared on the measure the server logs.
The writer signalled its milestone only after every insert had finished, so the read ran against a writer that was already done. The measurement was of a read with no write load in it, which is the opposite of what the test claims to measure. The milestone is now signalled part way through the inserts, and the writer keeps writing while the read runs. The test asserts that the writer was still going when the read started, so the overlap is pinned rather than assumed.

… is guaranteed
The writer streamed inserts and signalled a milestone part way through, so
the read could still start after the writer had finished: the reader's
transaction opens with an asynchronous BEGIN, and the writer can use that
window to complete its inserts. The test then measured a read with no write
load in it.
The writer now HOLDS the write lock for the whole read, so the overlap is
guaranteed by construction rather than by timing. With the lock held, the
two connections behave differently, and the difference is the change:
read-on-reader: connection=reader lock_wait_ms=0.022 lock_held_for_ms=12
read-on-writer: outcome=Failure lock_held_for_ms=3002 connections_acquired=0
The read on the reader connection completes while the writer holds the
lock. The same read on the writer connection cannot even acquire a
connection - it waits for the lock until the bound expires, and there is no
db.connection span for it because the acquisition never finished.

… timeout Two assertions were weaker than the claims they carried. The reader case asserted that the lock was held for the whole read by checking the elapsed time, which is a 1 ms-resolution clock reading: a fast read can finish inside the same millisecond, so the assertion could fail intermittently, and it did not prove what its comment said. The lock is held by construction - the writer holds its transaction until the read has finished - and what proves it is that the writer's row was still uncommitted, and so invisible to the reader, when the read finished. The test now asserts that. The writer case accepted any failure. Effect.timeout can preserve an earlier SQL failure, so "it failed" did not mean "it was blocked by the lock". The failure is now asserted to be the timeout, and to have waited for the bound, which a read that failed for another reason would not do.
… on the reader too A triage note on issue pingdotgg#21840 named four ProjectionStore reads that still default to the writer on main, because neither they nor their callers opt into readOnly. Two of them - getShellSnapshot and getThreadRecords - are covered by the previous commits. This covers the other two: - getRecoveryThreadIds, the startup-recovery query; - getSettlementCandidates, the auto-settlement sweep. Both are pure reads, so both belong on the reader connection: startup recovery and the settlement sweep must not take the write lock. Together with getThreadShell, that is every read the note named.
The server typecheck was never actually run locally: `vp run --filter
@t3tools/server typecheck` matches no package, so it exits 0 having done
nothing. The package is `t3`, and `tsc --noEmit` in apps/server reports
twelve errors in the two new test files, which the project's CI caught and
this commit fixes.
- `ModelSelection` was used but never imported.
- `withWriteLockHeld` declared neither the `uncommittedRows` it returns nor
the `SqlError` its queries can fail with.
- The span attribute is read as `{} | null`, so it needs narrowing before it
can be used as a connection name.
- `console.log` inside Effect code is rejected by the effect language
service, and `Effect.log` is swallowed by the test runner, so the
measurement is printed through a suppressed `console.log` instead.
- `Fiber.join` propagates the writer's failure rather than returning it, so
a row that did not insert now fails the test instead of passing as write
load. The writer also releases the reader if the write fails, instead of
leaving it to time out.
- The two layer merges that provided `NodeServices.layer` alongside other
layers raised a warning and made `tsc` exit non-zero with no errors at
all. They now provide it with `Layer.provideMerge`, the way the rest of
the repository does.
`tsc --noEmit` in apps/server now exits 0 with no errors and no warnings,
and the seven tests in the two files pass.
Refs #21840 (the accepted, triaged issue for the projection reads taking the write lock). This pull request
covers the reads that #21869's scope explicitly defers: "Scope is bounded snapshot reader routing; query
indexing and other projection reads are separate work." A triage note on #21840 named the four
ProjectionStorereads that still default to the writer on main, and offered to file them separately. Thisis that work, and it covers all four.
Problem
ProjectionStorereads run in a plainsql.withTransaction, so they run on the writer connection. Thewriter is a synchronous
DatabaseSyncon the main thread. A long read on it therefore stalls the server'sevent loop for the whole statement - and it does so in two ways:
cannot run anything else until it finishes.
The second way is the one that is easy to miss, because the lock wait is ~0. A triage note on #21840 measured
a 9,841 ms writer transaction for
readShellSnapshotRowswithdb.lock_wait_ms = 0.001and a 9,133 msevent-loop delay in the same 30 s window: the delay was inside the synchronous query, not in acquiring
the connection.
The four reads named in that note are the ones this pull request moves:
getShellSnapshotgetThreadRecordstools/call, message-id querygetRecoveryThreadIdsgetSettlementCandidatesA fifth read,
getThreadShell, is the shell row for one thread, read on every thread read. It is a pure readof the same shape, so it moves too.
What it costs, measured
An independent reproduction against a 12.1 GB
statev2.sqliteon 0.1.2-nightly.20261022.3526 (f054d1b),macOS 27.0.1, M4 Pro, benchmarks the same
getShellSnapshotquery on both connections, with the same rowsout (row counts and per-statement SHA-256 matched across all 34 runs):
The read takes the same time either way. What changes is everything around it: the main loop stays at its
~13 ms idle level instead of matching the 13 s statement, and the writer keeps committing instead of
queueing behind the read. Repeat reads are 271 ms (writer) and 287 ms (reader), so the win is specific to the
cold path.
The MCP path is the one that bites hardest
Measured on a host serving a 25.5 GB
statev2.sqliteunder load 68-116 on 10 cores, from the server's owntrace log over a 10-minute window:
78.5% of every SQLite query in that window ran inside an MCP call. The worst single lock wait was
6,622 ms, on
SELECT payload_json FROM orchestration_v2_projection_nodes WHERE node_id = ?, and seventransactions waited 6,326-6,554 ms. The reader connection was barely used: 8 transactions in the whole window,
averaging 497.7 ms - the reader pool starved exactly when the writer was busy, which is the only time it
matters.
Change
Each of the five reads is wrapped in
NodeSqliteClient.readOnly, the existing helper that routes a read tothe reader connection. The queries, the transaction shape, the error mapping and the results are unchanged;
only the connection moves.
This is the form the helper documents - "Wrap the whole transaction, as in
sql.withTransaction(reads).pipe(readOnly)" - and it keeps two of the upstream contract's guarantees:transaction is unaffected, and sees its own uncommitted writes.
secretly wrote would break, not silently return the wrong rows. All five of these reads are
SELECT-only,including the settlement sweep, whose write is in the caller.
Why this does not just move the queue somewhere else
Both connections are one-deep semaphores (
Semaphore.make(1)for the writer, and again for the reader), andthe transaction acquirer holds its permit for the whole transaction. So a read never had a parallel slot on
the writer to lose: the writer's queue was already one deep, and it was one deep in both directions, since
it carried the writes as well.
Moving a read to the reader does not add a queue. It moves the read off the queue that also serves writes:
The two hot shell paths were already on the reader before this change - the HTTP shell stream
(
loadShellSnapshotParts) andThreadPullRequestServiceboth route there today. This change makes therouting consistent rather than adding a new consumer of the reader.
Scope and approval
Refs #21840, which is triaged and accepted (
bug,accepted,via-triage). #21869 fixes the windowedsnapshot read; its scope note defers "other projection reads" to separate work, and the triage note on
#21840 asks whether the remaining reads should be filed separately. They are filed here, and the pull request
covers all four the note named.
This does not change query cost. #23009 addresses that separately - it cuts the first read about 4.9x and the
repeat read about 1.8x - and the two are complementary: this pull request decides where the query runs,
#23009 decides how long it takes. On the #23009 indexes alone, the writer path is still blocked for about
2.7 s, so the connection matters as much as the index.
Verification
The reads run on the reader connection
(
apps/server/src/orchestration-v2/ProjectionStoreReaderConnection.test.ts): five tests, one per read,asserting the
db.connectionattribute the server records is"reader". They are red before the change andgreen after.
The lock wait is measured on that connection
(
apps/server/src/orchestration-v2/ProjectionStoreLockWait.test.ts), on a file-backed database in a T3 home ofits own - the live T3 home is never touched. Reproduce it with:
which prints:
The writer holds the write lock for the whole read, so the overlap is guaranteed by construction rather
than by timing. A writer that streams inserts and then finishes leaves a window in which the read happens
after the writes are done, and the test would then be measuring a read with no write load in it.
With the lock held, the two connections behave differently, and the difference is the change:
until the bound expires. There is no
db.connectionspan for it, because the acquisition never finished.Two details make the assertions hold rather than merely pass:
That is what proves the write was in flight for the whole read; the elapsed time cannot prove it, because
the clock has 1 ms resolution and a fast read can finish inside the same millisecond.
read that failed for any other reason would not have waited.
The writer's insert uses the table's own columns and is not allowed to fail quietly: joining the writer
propagates its failure, so a row that did not insert fails the test instead of passing as write load.
The typecheck
This pull request's first version did not typecheck, and the project's own CI is what found it. The local
command that was being used for the server typecheck -
vp run --filter @t3tools/server typecheck- matchesno package, so it exits 0 having done nothing; the package is
t3.tsc --noEmitinapps/serverreported twelve errors in the two new test files, all of them in the tests and none in the change itself:

readOnlywrappers - was never involved.Regression runs
The project's own
CIworkflow, run on this exact head:Everything the workflow selects passes. The upstream pull request's own runs sit at
action_requiredbecause GitHub's fork-approval gate needs a maintainer to approve them; the run above is the same workflow
on the same commit, and the runner label is the only difference (the fork has no Blacksmith app, so
blacksmith-*labels queue forever there - see the fork's pull request #2 for the one-line workflow changethat points it at the same Ubuntu 24.04 images on GitHub-hosted runners).
The branch merges
main, so it is tested against the current upstream tree rather than an older one.
