Refs #23656

Problem

With a screen reader on, the open thread says nothing when a turn starts, needs the user or ends. The user has to leave the composer and re-read the end of the timeline to find out.

Fix

The open thread gets one hidden, polite live region that announces turn status:

  • "Working" when a turn starts
  • "Needs approval" or "Needs your input" when the agent asks for something
  • "Done", "Failed" or "Stopped" when the turn ends

Opening or switching threads stays silent: the first state the region sees for a thread is a baseline. The baseline waits until the thread's detail agrees with the live thread shell. Before that, the detail may not be loaded yet, or a cached copy may still show an older run, and taking either as the baseline would replay what already happened ("Working" on opening a running thread, "Done" on reopening one that finished while away). A queued message cancelled before it started never ran, so it isn't read as the turn's ending: the turn ends however the last run that actually ran ended. A send that looks idle for a moment before the server reports its run still counts as one turn, so "Working" is said once. Text goes straight to the DOM node, so an announcement never re-renders the timeline.

The sidebar row already announces each thread's status. For the thread that's open, it no longer does, so every status is heard once. Background threads keep announcing from the sidebar as before. A native subagent thread already has its own status bar with a hidden status line, so the region stays out of those threads.

The region is a plain aria-live="polite" div with no role="status". Orca 50 maps role="status" to a status bar in Chromium and drops its updates, so with that role Orca said nothing at all.

The rules live in threadActivityAnnouncement.ts, a pure function with tests. awaitingUser on the timeline now says which request is pending instead of a boolean.

Evidence

AT-SPI listener

Linux desktop app (dev build on corex, Wayland). A small AT-SPI listener logged every live-region update the screen reader received while Claude Haiku read two files and ran ls and git log as a new thread's first turn.

before

after

On main, the turn starts with the sidebar's "Working" and sync notices, and nothing says when it ends. With this PR it's "Working" at the start and "Done" at the end, once each. "Regenerating title" is the sidebar's existing notice. A follow-up turn in the same thread gave the same "Working", "Done" pair. Switching threads cleared the region without saying anything.

Orca 50.2 with Chrome on Linux

Recorded while testing #23839, with that PR on top; the "Working" at 0:05 and 0:46 of the second video is this PR's region. Orca runs on a private X screen with its own speech-dispatcher, so the audio is only Orca, and the captions are Orca's speech log burned in. Before is this PR at 627923883b, when the region was still role="status": Orca says nothing about any turn.

orca-before.mp4
orca-after.mp4

The same session caught the reopen bug. Live-region text the page wrote during the "before" run (Orca spoke none of it):

2.9s Working a turn starts in "Fix the Login Redirect"
41.3s Done reopening "Fix the Login Redirect" after it finished in the background: a stale repeat
44.0s Working the next turn starts
57.3s Done the sidebar row, with the sidebar open
80.6s Done reopening it again: another stale repeat

In the "after" run both reopening lines are gone. A turn that is still running when you come back still ends with one "Done".

Real app on the web, after an Opus review

A review found two more cases. Both were reproduced in a web dev build on corex with real Codex turns, by logging every write to the live regions. Before is 8aa5c8c36a, after is this branch with #23839 on top.

Case Before After
Reload on another thread, then click a thread that's mid-turn "Working" on open, then "Done" silent on open, then "Done"
Queue a follow-up during a turn, remove it, let the turn finish "Working", "Stopped" "Working", "Done"
A normal turn in the open thread "Working", "Done"

Not in this PR

The issue reporter tested with NVDA on Windows 11 in the desktop dev build: "Working", "Needs approval", "Needs your input" and "Done" are each heard once, and the sidebar no longer repeats the open thread's status (comment). That was before the last three commits (the baseline and cancelled-queue fixes, and dropping role="status"), and NVDA hasn't been rerun since.

Approvability

Verdict: Needs Human Approval

Macroscope's review found this PR requires human approval — This adds an always-on screen-reader announcement system spanning ChatView, the conversation timeline, and sidebar status semantics, backed by a nontrivial run-state machine. It is a meaningful user-facing runtime behavior change rather than a small mechanical fix.

📝 Walkthrough

The timeline now distinguishes pending approval from pending input. It tracks thread activity and run status, selects announcements for activity changes and terminal outcomes, and renders them through a polite live region.

Explanation

The description covers the problem, the fix, and verification evidence, so it meets the template's required sections. It explains the problem (a screen-reader user hears nothing when a turn starts, needs input, or ends) and the change (a hidden polite live region, status rules in a pure function with tests, and sidebar deduplication for the open thread). The Verification section has AT-SPI logs, Orca recordings with a stale-announcement log, a web test table, and a tester report on NVDA. The Scope and approval section links issue #23656, which covers the scope, and the author lists out-of-scope items explicitly. It also notes that the NVDA check predates the last three commits.

🤖 Claude Opus 5.5 responding on behalf of Andrew

I'm a screen reader user (NVDA on Windows), and I'm glad to see this. A suggestion: this PR might be easier to land if it covered only turn status, and left the activity labels for the follow-up that adds a setting.

Activity labels. During a busy turn, they speak a new label every 2 seconds for as long as the turn runs. That can mean hundreds of announcements while you're typing in the composer. Personally, I'd want them off, though I know other screen reader users may feel differently. The only way to turn them off today is NVDA+5 ("Report dynamic content changes"). NVDA checks that setting before it reports any live region in Chromium (nvdaControllerInternal_reportLiveRegion in source/NVDAHelper/__init__.py). So turning it off also mutes "Done", "Needs approval", the sidebar, and error alerts. The same setting also controls automatic reading of new text in console windows. Since preferences differ, the labels seem like a good fit for their own switch, as planned in item 5 of #23656.

Overlap with the sidebar. Each sidebar row is already a live region with "Working", "Pending Approval", "Awaiting Input", "Failed" and "Done". The "after" capture shows "Working" twice. I'd expect a turn to read "Working, Working ... Done, Done", and approval to be announced twice in different words. It might be worth having each status come from one place. Either the open thread's region skips what the sidebar already says, or the sidebar stops announcing the open thread.

If the activity labels move to the follow-up, the queued-label issue Macroscope found goes away too. Today, ThreadActivityAnnouncer returns before cancelPending() when work stops before the run finishes. So a stale "Running ls" can be spoken after the agent has stopped.

I'm happy to test a revised build with NVDA.

Activity labels move to a follow-up with a setting, and the open thread's
sidebar row stops repeating the status the thread already announces.

Tested on Windows 11 with NVDA in the desktop dev build. The new region works: "Working", tool labels, "Thinking" and
"Done" all come through.

"Done" is sometimes announced twice. The second one comes from the sidebar: with the sidebar collapsed, it doesn't
happen.

The open thread's sidebar row is also a live region (SidebarThreadRow.tsx:1348, role="status"). When the run completes,
its completedAt is newer than the thread's lastVisitedAt, so hasUnseenCompletion makes the row show "Done". That lasts
until the visit dispatched by ThreadVisitWatermark comes back from the server. In that window the span's text changes
from "Working" to "Done" in place, and NVDA reads it right next to the thread region's "Done". It depends on that round
trip, so it's intermittent and probably worse over remote connections.

Suggested fix: the thread region now covers the open thread, so its row doesn't need to be a live region:

<span role={props.isRouteThread ? undefined : "status"}>{topStatus.label}

isRouteThread rather than isActive, because isActive is also true for a parent row while one of its subagents is open,
and that parent's "Done" would go silent.

I also heard "Working" twice once or twice early on, but couldn't reproduce it afterwards, with the sidebar open or
closed.
Another thing is when the search tool runs rather than saying searching first the following is announced, this is an example: Theo Browne t3dotgg opinions TypeScript React Next.js AI coding. It would be better to announce something like this: "searching for Theo Browne t3dotgg opinions TypeScript React Next.js AI coding". This should maybe be in it's own pr though.

Thanks, both points made sense and are in 2296f20.

  • Activity labels are out. The PR now announces turn status only: "Working", "Needs approval" / "Needs your input", "Done" / "Failed" / "Stopped". The labels can come back behind the setting from item 5 of [Bug]: Screen readers get no announcements while an agent works in a thread #23656.
  • The sidebar row of the open thread is no longer a live region, so each status is heard once. Background threads still announce from the sidebar. Re-captured on the Linux desktop app: a new thread's first turn now reads "Working" … "Done", once each (updated image in the description).
  • With the labels gone, the queued-label code is gone too. For the record, that case was already fixed in 08235b8 before your comment.

An NVDA check of this build would be very welcome, especially approval and input requests, which I could only cover with unit tests here.

Thanks for testing with NVDA, this is really useful.

  • The double "Done": your diagnosis matches, and the latest build has exactly your fix (2296f20). The open thread's row is no longer a live region (isRouteThread, so a parent row still announces while its subagent is open).
  • The occasional double "Working": also seen on Linux on a thread's first send. The send looks idle for a moment before the server reports the run, and the region treated that as a second turn. Fixed in the same commit, and 4e76170 makes a retried send after a failure still say "Working".
  • Tool labels, "Thinking" and the search wording: after akj's feedback above, this PR now covers turn status only, and the labels move to the follow-up with the setting from item 5 of [Bug]: Screen readers get no announcements while an agent works in a thread #23656. Your search example ("searching for …") is a good one for that PR.

If you have time, a retest of the current head (4e76170) would help, especially "Needs approval" / "Needs your input" and whether "Working" and "Done" are each heard once.

A follow-up sent while the last run was still settling could say Working
twice or not at all. The reducer now remembers which run Working was said
for and only speaks again when another run takes over.

"Needs approval", "Needs your input" and
"Done" are each announced once, and the sidebar no longer repeats "Done" for the open thread.

One thing is left: I still sometimes hear "Working" twice. It happens [at the start of the turn / after answering a
question / after approving a command], with the sidebar [open / closed]. @akj, could you check whether you hear it too?

There's also a gap that's worth either fixing in this pr or maybe for a follow-up: when a thread you're not in finishes, the
"Done" doesn't say which thread finished, and it only comes from the sidebar row. With the sidebar collapsed, which I
prefer to keep the UI uncluttered, you don't hear about background threads at all. Something like "Done: ", announced whether or not the sidebar is open, would cover both.

Thanks for retesting, good to hear approval, input and "Done" are each heard once now.

On the double "Working": the last few commits fixed three more ways it could happen, all around a follow-up sent while the previous run was still settling, or a queued message starting right after a turn. The latest is 25dd632. If you still hear it on that commit, could you say when (start of a turn, after answering a question, or after approving)? The template brackets in your comment came through unfilled.

Background threads saying "Done: " whether or not the sidebar is open is a good idea. It changes how every background thread is announced, so it fits better as its own issue and PR than as part of this one.

Include the pending request ID in the activity snapshot.

When two same-kind requests are pending, resolving request A makes request B active. ChatView still projects both states to the same "approval" or "input" value. nextThreadAnnouncement then suppresses the second announcement because the previous snapshot has the same status, so the screen reader can miss request B.

Pass the active request ID through MessagesTimeline and announce when either the status or request ID changes.

… it turns off

Sorry about that. I have tested the latest fixes and everything seems to be working as it should. I am unable to reproduce "working" being announced more than once after the latest fixes. I can also open a new issue for the Background threads saying "Done" and include the sidebar idea there as well and link it to this PR as something to be done once this pr has landed.

Thanks for checking again with NVDA, that settles it. And yes, a separate issue for background threads (thread name in the announcement, working with the sidebar collapsed) sounds right; linking it here works well.

…s Done again

The cached detail can still show the run working for a frame after the live
shell saw it end, so that frame became the baseline and the catch-up read as
the turn ending.

Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
Orca 50 treats a role="status" region in Chromium as a status bar and
drops its updates; a plain polite live region is spoken.

Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
…d message isn't "Stopped"

The announcer took its silent baseline from whatever frame came first. On the
web that is often a detail that hasn't loaded, or a cached one showing an
older run, so opening a running thread said "Working" and reopening one whose
runs finished while away said "Done". It now waits until the detail agrees
with the live thread shell.

A queued message cancelled before it started keeps its place in line, so it
became the thread's latest run when the turn ended, and the turn read as
"Stopped". The announcer now reads the last run that actually ran.

Drops the reopen test that copied the component's frame filter.

* feat(relay): deploy on-demand preview stages for labeled PRs

Labeled same-repo PRs deploy stage pr-<n> behind the relay-preview environment reviewer gate. Unlabel or close tears the stage down, and a nightly job retries failed teardowns. Runtime-created managed endpoint tunnels and CNAMEs are swept by a PreviewTunnelSweep resource.

Known gaps, tracked as follow-ups: an interrupted preview deploy can leave runtime tunnels behind; repo-level relay secrets still bypass the environment gate until moved.
