- A finding whose fix is a mechanical edit of up to five lines, all of them added by the pull request, carries a GitHub suggestion block on a comment that covers those lines, so it applies with one click; any other fix stays in prose. The hidden marker on each inline comment also names its reviewers, and comments posted before still count as already posted ([#54](https://github.com/NCodeGroup/code-review/issues/54)).
- A per-review cost ceiling guards against a runaway review: $8 plus $4 for each part of the diff, overridable with `CODE_REVIEW_MAX_RUN_USD`. Reviewer calls not started once it is reached are skipped and shown as `skipped: run budget` in the check run, synthesis still runs on what was found, and the summary warns that the review is incomplete ([#50](https://github.com/NCodeGroup/code-review/issues/50)).
- A verification stage, off by default: with `CODE_REVIEW_VERIFICATION=on`, a Sonnet call with the checkout proves or refutes each merged blocker and should-fix, a file's findings at a time at up to $0.50 a call. Confirmed findings get the code path in their evidence, refuted ones are dropped and listed with the reason under "Refuted by verification" in the check run and in the trace, and ones it can't settle become questions one severity lower. The summary names the refuted findings, and verification turns a request for changes with nothing serious left into a comment, never an approval. Nits aren't verified, and the calls show as `verification` in the cost table. It stays off until the eval shows a gain, and the eval report now counts posted blockers and should-fixes and refuted findings ([#41](https://github.com/NCodeGroup/code-review/issues/41)).

- `confidence` is your probability, from 0 to 1, that a careful maintainer would agree the finding is real and worth acting on. The maintainer would rather answer a doubtful finding than miss a real one, so report anything at 0.3 or above, and say in the problem what you're unsure of.
- Don't pad: a finding still needs a line and a concrete consequence. When the change is clean, an empty list is the right answer.
- `line` is a line number in the new version of the file, on a line the diff adds or changes when the finding is about the change. Use the nearest changed line otherwise.
- `suggestion` is optional. Give it only when the fix is a mechanical edit of 5 lines or fewer, all of them lines the diff adds, that needs no change anywhere else, and you have read the exact lines in the checkout. `startLine` and `endLine` are the first and last lines it replaces in the new version of the file, a range that includes `line`; `replacement` is the exact text that replaces those lines whole, with their indentation, at most 5 lines, or empty to delete them. Leave it out for anything else. Always write the fix in prose as well.

3. Hold a missing-test finding to `should-fix` only when the untested path is new behavior with a plausible failure a user would see, and nothing in the diff or the finding shows a test below it covering that path. Lower it to `nit` when the untested code only wraps or composes pieces that have their own tests, when the test would only pin current behavior that nothing shows is wrong, or when it would be a second test for a path another test already runs. A test-gap finding decides whether the author keeps iterating, so don't let one hold a finished pull request open.
4. Rank by severity, then by how likely and how bad the consequence is.
5. Tighten every finding: the problem in one sentence, then the consequence, then the fix. Cut hedging, praise and restatement of the code. Keep file paths, identifiers and line numbers exact.
6. Keep within the number of findings you're given as the limit, which is generous. If merging duplicates still leaves more, fold related nits in one file into a single finding rather than leaving any out.
7. Write a summary of two to four sentences: what the pull request does, how healthy and risky it is, and whether its design holds together or leans on workarounds. Don't list the findings again.
8. Give a verdict: `request-changes` when a blocker remains, or should-fixes that together make the change unsafe or unsound to merge as it is; `comment` when there are findings worth acting on but nothing that should stop the merge; `approve` when nothing remains worth raising.
6. Keep a reviewer's `suggestion` exactly as it is, on the finding you post for it, including one you merge, as long as your fix is the one the suggestion makes and your `line` stays inside its range. Leave it out when you change the fix or the line, or when merged findings suggest different code. Never write a suggestion of your own.
