Summary

A failed Windows ACL operation currently loses the execution detail needed to distinguish a spawn failure, a runner rejection, a caller watchdog, a pre-spawn deadline and a completed child result. Preserve a bounded diagnostic record through the existing sanitized error boundary.

The record contains fixed stage/step enums, finite timing/budget values, success/exit/timeout information and an allowlisted spawn error code. It excludes paths, account identities, arguments, raw errors and process output. Existing error codes, required-write refusal, retries, timeout memo and reap policy are retained.

The bearer-admission fixture also gains opt-in OPENCODEX_BEARER_STAGE_TRACE=1 timing for configuration save, startup, request, stop and capture close. It records both the whole-test and iteration clocks. Request tracing now covers the original body reader, including JSON parsing. Chat still rejects an unexpected 200 before draining its body. Whole-test finalization runs after normal cleanup unwinds, including assertion failures. Synchronous stages remain synchronous when tracing is off; no retry or timer is added.

Verification

URL type follow-up — 2517c7c85c515781c7ea3565e0c19ba8981b79c8

The two shared bearer trace helper parameters now accept string | URL, matching Bun Server.url and the existing post functions. This follow-up changes only type annotations in tests/codex-integration/bearer-admission-routed-provider.test.ts.

  • Pinned Bun 1.4.4 on Linux: bun test ./tests/codex-integration/bearer-admission-routed-provider.test.ts --test-name-pattern 'bearer stage tracer seam' passed 5 tests / 42 assertions, including the real local-server URL path.
  • bun run typecheck, bun run structure:check, bun run privacy:scan, git diff --check, and the size-ratchet suite (9 tests / 85 assertions) passed.
  • A separate TypeScript configuration that includes this test file removed exactly the reported URL-to-string error: 5 diagnostics before, 4 unchanged baseline diagnostics after, none added. The four remaining diagnostics concern Http2Stream.respond, two optional visionSidecar accesses and an account fixture label; this broader test-file check is not claimed as passing. The official source-only typecheck does pass.
  • The published tree 58fadc17251d8df330f3d0289aa7261e12241dce matches the locally verified file. Current-head Cross-platform CI and Catalog async contracts both completed successfully on this exact head. Policy-skipped native jobs are not native-platform acceptance evidence. Native Windows/macOS and the unchanged ACL suites were not rerun for these two type annotations.

Earlier diagnostic and trace validation

Previously validated head: c587dc3740eb3f1b45f723c64d854cc17dd134f4. The ACL implementation and its diagnostic suite are unchanged from 13d7abc2; this follow-up changes the bearer trace fixture. Local validation used Windows and pinned Bun 1.4.4.

  • Restoring the baseline helper produced 12 semantic failures for missing diagnostics. A cleanup gap in the new watchdog test was then repaired with finally; its baseline case still fails meaningfully with no teardown timeout.
  • New and existing ACL suites: 186 passed, including mandatory atomic-write refusal. After the test-only cleanup repair, the final diagnostic file passed 13 cases / 50 assertions. The unchanged suites were not repeated.
  • Current shared-flow trace regressions: 5 passed / 42 assertions. A final test-only body-entry guard was rechecked separately (1 passed / 6 assertions). Controlled old-behavior variants produced two header-only trace failures and one success-only finalization failure, with exact candidate restoration and no cleanup hang.
  • Actual local Responses and Chat bearer fixtures: each 11-iteration loop passed in 18.2s / 17.9s on the follow-up, retaining the existing 30-second test budget.
  • Layout/tooling: 29 passed. Typecheck, structure, size ratchet, privacy and whitespace checks passed.
  • Commands had an external 55-second bound, native exit/EOF and owned-process cleanup checks. Source hashes were checked before and after execution; no owned process or sandbox remained.

The original intermittent CI ACL failure did not recur in these local fixture runs. This PR improves diagnosis; it does not claim to fix that incident or infer an external process kill from timing alone. Injected-runner late-reap tests establish the tracking contract, not real OS-child termination. Retained output is bounded; complete retained trace records were checked separately from truncated output.

No installed runtime, account, certificate, permissions policy or global configuration was changed. The prior 13d7abc2 head passed Cross-platform CI and Catalog async contracts. The earlier c587dc3 head passed Cross-platform CI and Catalog async contracts. The two earlier trace follow-up review threads are resolved. Hosted Windows and macOS lanes were skipped; native Windows validation above was local. Explicit security review remains required before merge.

Checklist

  • Scope is limited to diagnostics, focused fixtures and their owning contract.
  • Sensitive values are excluded from the diagnostic schema and covered by regression tests.
  • Validation and remaining limits are documented.
  • Explicit maintainer security review.

Summary by CodeRabbit

  • New Features
    • Windows ACL hardening failures now include bounded diagnostics identifying the failed operation, failure stage, timing, and result. Sensitive paths, arguments, and process output are excluded.
  • Bug Fixes
    • ACL failure details are retained across synchronous and asynchronous operations, including timeouts and startup failures, while preserving existing error behavior. Required ACL hardening failures prevent atomic-file publication, and diagnostics distinguish pre-startup failures from child-process results.

Distinguish spawn, runner rejection, caller watchdog and child-result evidence without exposing paths, principals or command output. Keep existing hardening and reap behavior, and add opt-in stage timing for the bearer-admission fixture.

Walkthrough

Windows ACL failures now carry validated, bounded diagnostics in sanitized errors. Windows tests and test-layout mappings cover those diagnostics. The bearer-admission integration test adds opt-in stage tracing for indexed cases.

Changes: Windows ACL failure diagnostics

Layer / File(s) Summary
ACL runner provenance
src/lib/windows-secret-acl.ts
ACL runner results distinguish spawn failures, runner rejections, and watchdog timeouts. The module defines allowed ACL steps and failure stages.
Per-run diagnostics and sanitized errors
src/lib/windows-secret-acl.ts
Sync and async ACL sequences record failure stage, elapsed time, and budget. Sanitized errors receive diagnostics only after validation and allowlist checks succeed.
Diagnostics validation and test mapping
tests/windows/windows-secret-acl-diagnostics.test.ts, tests/fixtures/test-layout-expected.json, scripts/test-layout/layout.json, structure/ops/service-and-sidecars.md
Tests cover sync and async failure paths, diagnostic sanitization, and atomic-write behavior. The Windows test mappings and operations documentation include the diagnostics test and record fields.

Bearer-admission test tracing

Layer / File(s) Summary
Trace wrappers and request stages
tests/codex-integration/bearer-admission-routed-provider.test.ts
Opt-in tracing records elapsed-time events. The request stage includes response-body reading, and Chat checks for a non-200 status before reading the body.
Indexed bearer matrix and trace tests
tests/codex-integration/bearer-admission-routed-provider.test.ts
The indexed matrix records completion for each iteration, including failures. Tests check event order, disabled tracing, request timing, and cleanup.

Copy the value that passed validation.

If a runner throws an error with a changing aclFailure.step getter, the first two reads at Line 1189 can return "/grant:r", while the read at Line 1207 returns a private path. A required harden then copies that path into the public aclFailure record. The same repeated-read pattern affects the other validated fields. Read each field once into a local value, then validate and copy only those local values. Add a regression test with a changing getter. The required-error path passes runner errors through this sanitizer. (raw.githubusercontent.com)

Owner

Maintainer dev integration (L5 lane), per MAINTAINERS.md.

Decision: squash-merge at exact head a32f7a1170b8407eaec1bce3bd79278fc61519cd.

  • Security review: independent round 1 PASS (0 blocking, 0 non-blocking) at 2517c7c8. The head then moved only by an origin/dev update-branch merge, which brought in fix(claude): drain parent ACL flights before intercept secret writes #7777's ACL barrier. A delta security round at a32f7a11 also PASSED with 0 blocking findings: diagnostics, sanitization, barrier timeout ownership, refusal and timeout memo all integrate correctly. In that round windows-secret-acl-diagnostics plus claude-intercept-acl-barrier passed 43/0.
  • PR CI at the exact head: Cross-platform CI 40509141113 success, Catalog async contracts 40509141118 success, PR hygiene and target checks success.
  • lane=all 40513796719 at the exact head: every macOS, Ubuntu, desktop and Windows job succeeded except windows 9/9 (and the ci aggregate that it fails). That shard failed in cli-start-journal-order > "a healthy proxy owner preserves the journal for both start and ensure". The same case failed on dev itself in lane=all 40434080650 (1de2b71b), and this PR touches none of the start, liveness or journal code. The failure is tracked in [Bug]: start can recover the Codex journal under a live owner when the first liveness probe misses it #7827; it was investigated, not rerun.
  • Current with dev 962546af: the 4 newer dev commits overlap only the union files scripts/test-layout/layout.json and tests/fixtures/test-layout-expected.json. On the locally merged tree (git merge-tree, no conflicts), test-layout, test-layout-tooling, file-size ratchet and the diagnostics test passed 51/0, and tsc --noEmit and structure:check exited 0.
