fix(config): re-arm the atomic writer's ACL timeout memo per destination - #7891
On Windows, atomicWriteFile (src/config/atomic-write.ts) hardens each temp file with { required: true, timeoutMemoKey: <destination> }. After one icacls timeout, timeoutMemoErrorIfBlocked refused every later write to that destination, because none of the five harden calls opted into the existing 5-minute re-arm (TIMEOUT_MEMO_REARM_MS, added for the response-spill directory in #3522). A single icacls stall therefore froze files such as provider-account-quota-cache.json until the proxy restarted, while the proxy kept reporting healthy.
This carries the intent of #7843 onto the current atomic writer:
All five harden calls (normal sync, streamed and async writes) use the destination-keyed recovery policy: after the re-arm window, the next write to that destination retries icacls once.
A destination is refused only while it has an icacls child retained past its caller's timeout and not yet reaped. Repeated re-arms therefore cannot pile up live icacls children on one file. Healthy overlapping writes to the same destination proceed as they do on dev, including while an earlier write is hardening its temp file or waiting to rename it.
required: true stays fail-closed: no bytes are written or published to a destination whose temp file was not hardened. Hardening still happens before content and before publication, and non-timeout ACL failures are not re-armed.
Both residual cleanup paths keep the recovery attempt for a later write instead of consuming it.
New tests/windows/windows-atomic-write-acl-rearm.test.ts (deterministic runner and clock seams, no real waiting): red on dev's writer, 5 pass / 11 fail; green on this branch.
Retained-child regression from review: on the first implementation the probe retained 1 -> 2 -> 3 icacls children for one destination across re-arms. On this head it retains one while that child is stalled (1 -> 1 -> 1), none after the child is reaped, and the next write then recovers.
Healthy-overlap regressions (added after CodeRabbit noticed the refusal was too broad): on 92f1b53e3, a second write to a destination with an earlier write in flight was refused with ETIMEDOUT even with healthy icacls (3 failures). On this head both writes succeed, including when the earlier write is paused before its rename.
The new file followed by the native ACL tests in one Bun process: 196 pass / 0 fail. This proves the test restores both Windows-principal runners before resetting identity, so later native tests do not run real icacls with a fake SID.
Caller inventory: the four async writer families are serialized by their production callers, and the quota and streamed writers run synchronously, so on dev and on this head a healthy overlap is last-publication-wins. Only 92f1b53e3 refused it.
bun run typecheck, bun run structure:check, bun run privacy:scan, git diff --check: pass. Docs site build passed on the first implementation (601 pages).
Not run: bun run test:changed. On this shared host it waited behind other worktrees' runs and hit its existing 900 s limit before reporting, so it is left to the PR's test changed job. Also not run: the full suite, macOS/Linux, and a real prolonged icacls stall.
Review history: an independent security review of the first implementation returned two blockers (retained-child pile-up and the test-runner teardown leak), both fixed. A second security review of 92f1b53e3 passed. CodeRabbit then found that the per-destination guard there covered the whole write, which also refused healthy concurrent writes. 858e59b7c narrows it to retained children. A third independent security review of 858e59b7c passed with no blockers. It confirmed that healthy overlap proceeds, that retained-child counts stay at 1 -> 1 -> 1, that a healthy peer completing does not drop the guard before the actual reap, and that the retention is registered before the timeout resolves, which closes the admission race. Fresh run: 524 pass / 2 platform skips / 0 fail.

Summary by CodeRabbit
Bug Fixes
Improved reliability of Windows configuration and process-state writes when access-control updates time out. A later write can retry recovery after the five-minute cooldown; writes are promptly refused while earlier access-control work remains unfinished.
Healthy overlapping writes to different destinations can proceed independently. Permission-denied errors still fail without automatic retry, and failed-write cleanup does not use up a recovery attempt.
Documentation
Added guidance on Windows access-control timeout recovery and write behavior.
Reimplement the intent of #7843 on the current dev atomic writer. Opt normal
sync, streamed and async writes into the existing destination-keyed recovery
policy while keeping both residual cleanup paths from consuming recovery.
Required hardening, identity checks and publication ordering remain intact.
Cover all five harden calls, cooldown boundaries, continued stalls, permission
denial and cleanup with deterministic runner/clock regressions. Document the
recovery behavior and retain the current config-root ACL settlement boundary.

Reserve default atomic-writer destinations across active writes and retained
Windows ACL children. Refuse overlapping sync, streamed and async writers
without spending timeout recovery or relinquishing removal ownership.
Cover repeated re-arm windows, mixed writers and recovery after actual reap.
Restore both principal runners before resetting regression identity state so
subsequent native ACL tests use the real account.
Keep the exact required/destination assertion and require the explicit
retryTimedOutOnce opt-in added for #7834. The old exact options object
rejects the intended recovery contract during test:changed.

Walkthrough
Windows atomic writers now retry timed-out ACL hardening under a per-destination recovery policy. They reserve destinations while ACL work or cleanup remains active. Regression tests cover synchronous, streamed, and asynchronous writers.
Writers track admission by resolved destination and block writes while a retained ACL child remains unreaped. Tests cover destination-specific guards, pending child cleanup, and overlapping writes.
Timeout retries and cleanup src/config/atomic-write.ts, docs-site/src/content/docs/troubleshooting/windows-acl-timeouts.md, structure/config.md
Sync, streamed, and async writers retry a timeout during normal writing, but not during cleanup. The documentation describes re-arming, refusal conditions, and per-destination tracking.

Explanation
The PR satisfies the coding requirements in [#7834]. In src/config/atomic-write.ts, all five destination-keyed hardening paths now pass retryTimedOutOnce: true for normal, streamed, and async writes (writePrivateTempFile at lines 231 and 272, writePrivateTempFileAsync at line 295, and the sync/async pre-rename hardeners at lines 348 and 521). admitAtomicAclWork at lines 35-51 refuses only while a timed-out ACL child remains pending. Its release path waits for flushWindowsSecretAclReapsBeforeRemoval before removing the destination reservation (lines 431 and 586). The existing required: true calls remain before content publication and rename. cleanupHarden prevents residual cleanup from consuming the next recovery attempt (lines 324, 392, 518, and 550). tests/windows/windows-atomic-write-acl-rearm.test.ts covers re-arm timing, fail-closed ACL errors, pre-rename hardening, residual cleanup, retained-child limits, destination independence, and healthy overlap. The reported test results support the implementation.
Full details: Out of Scope Changes check
Explanation
The changes stay within [#7834]. The production changes in src/config/atomic-write.ts implement destination-keyed ACL timeout recovery, retained-child admission control, fail-closed hardening, and cleanup recovery. tests/windows/windows-atomic-write-acl-rearm.test.ts, the test-layout entries, and tests/server/config.test.ts verify or integrate these changes. docs-site/src/content/docs/troubleshooting/windows-acl-timeouts.md and structure/config.md document the implemented recovery behavior. The diff shows no unrelated production feature or unrelated cleanup.

#7891 brought scripts/test-layout/layout.json to 2000 lines on dev, which the ratchet rejects as NEW_OVERSIZED. Move its registration onto the preceding windows line; the mapping is unchanged.

Summary
On Windows,
atomicWriteFile(src/config/atomic-write.ts) hardens each temp file with{ required: true, timeoutMemoKey: <destination> }. After one icacls timeout, timeoutMemoErrorIfBlockedrefused every later write to that destination, because none of the five harden calls opted into the existing 5-minute re-arm (TIMEOUT_MEMO_REARM_MS, added for the response-spill directory in #3522). A single icacls stall therefore froze files such asprovider-account-quota-cache.jsonuntil the proxy restarted, while the proxy kept reporting healthy. This carries the intent of #7843 onto the current atomic writer:
dev, including while an earlier write is hardening its temp file or waiting to rename it.required: truestays fail-closed: no bytes are written or published to a destination whose temp file was not hardened. Hardening still happens before content and before publication, and non-timeout ACL failures are not re-armed.Closes #7834
