fix(orchestration-v2): restore earlier app agent transcript pages

Problem

In the new orchestrator, app agent prompts were counted as user turns. That bypassed history page limits, so agent details could show only recent text or tools with no way to load the rest.

Fix

Count only actual user prompts as turns. Page agent-only history by bounded activity rows in both SQLite and memory, so Load earlier turns reaches the full transcript. Long histories still load in pages.

Verified with 43 focused server tests, targeted format/lint/typecheck, and a local web capture using fake agent data.
