fix(cli): recheck live ownership before journal recovery - #7851
ocx start could recover the Codex journal while a healthy proxy owner was still running (#7827). handleStart first calls findProxyOwnerBeforeJournalRecovery(). On a loaded Windows runner, that probe can miss a live owner even with its three attempts. The start then deleted the stale-looking PID record and ran reconcileStartupJournal(), restoring Codex's config from the journal. Only afterwards did the lease-held bind fence find the owner and refuse with "Proxy appeared before bind". The result was a destructive recovery under a live owner, seen in cli-start-journal-order on hosted Windows (lane=all 40434080650 on dev, and 40513796719).
The lease-held recovery step now rechecks liveness first, using the same START_OWNERSHIP_LIVENESS budget in src/cli/start-owner-fence.ts. That recheck runs before the PID record is removed and before the journal is reconciled. If it finds an owner, the start keeps the journal and the PID record and follows the existing decideStartWithLiveOwner outcomes (refuse, service stay-out, or sibling), exactly as the bind fence would. Cross-home sibling marking order and the synchronous startServer window are unchanged, and no timeout or retry is added.
Bun 1.4.4 on Windows, OCX_TEST_NO_QUEUE=1, one test process at a time:
New cases in tests/cli/cli-start-journal-order.test.ts (-t 'missed first owner probe') make the first pre-recovery probe miss a live owner while the later probes see it, for both start and ensure. They fail 6/6 on the base (journal and config recovered) and pass 6/6 with the fix.
tsc --noEmit, structure:check, privacy:scan and the file-size ratchet pass. src/cli/index.ts and structure/runtime.md stay within their caps.
NOT RUN: full local suite and test:changed (CI runs test changed and ci:full), macOS locally, hosted lane=all (to be dispatched on the merge head), physical service-manager activation.

Bug Fixes
Startup recovery now rechecks proxy ownership before changing saved startup state. If ownership cannot be verified, startup is refused and existing state is preserved; retry after the owner responds or exits.
Recovery proceeds automatically when the recorded owner has exited and no listener remains.
Server binding consistently maps blank or localhost hostnames to the IPv4 loopback address.
Proxy discovery handles IPv4 and IPv6 loopback addresses more reliably.

Walkthrough
Startup recovery now rechecks proxy liveness under an ownership lease before changing PID or journal state. Proxy discovery distinguishes live, absent, and unknown results. Uncertain evidence preserves the PID record, recovery journal, and shared config. Bind-host normalization and loopback-family checks support discovery across IPv4 and IPv6.
start and ensure defer PID cleanup and run recovery under an ownership lease. A live or unverified owner preserves state; definitive absence permits PID cleanup and journal reconciliation. Tests and documentation describe these outcomes.

Docstring coverage is inconclusive: files from earlier reviews still need to be rechecked within the check's analysis limits. Not blocking.
Full details: Linked Issues check
Explanation
Issue #7827 requires that start must not recover the Codex journal or config under a live owner, and that recovery must not rest on a negative probe that a lease-held recheck contradicts.
src/cli/index.tshandleStart: the early probe findProxyOwnerBeforeJournalRecovery now passes deferPidCleanup: true. The stale PID write is no longer made there. When owner.live is null, recoverStartStateUnderOwnershipLease runs. Its recover callback is recoverStartJournalAfterOwnerRecheck(owner, siblingStart).
src/cli/index.tshandleEnsure: same pattern, with supervised: false and an unconditional proceed decision.
src/cli/start-owner-fence.tsrecoverStartJournalAfterOwnerRecheck: calls observeProxyLiveness(START_OWNERSHIP_LIVENESS) first. A live result returns without mutation. A non-absent result (unknown) throws "Proxy ownership is unverified; startup refused" and keeps the PID record and journal. Only a definitive absent reaches removePidIfValueIs, markCrossHomeSibling and reconcileStartupJournal. This ordering matches the requirement.
If the recheck finds a live owner, handleStart takes its existing if (owner.live) branch and decideStartWithLiveOwner decides the outcome.
The bind fence (findLiveProxy(START_OWNERSHIP_LIVENESS)) is unchanged and uses the same budget.
Tests: tests/cli/cli-start-journal-order.test.ts adds the a missed first owner probe preserves shared state for <scenario> (#7827) cases for start, ensure, service, supervised, sibling and restart-parent. They assert that the config, journal and PID file are unchanged, that recover and pid-remove do not occur, and that the event order is probe, lease, recheck probe, release. Other new tests (cli-start-owner-absence, cli-start-owner-evidence, proxy-liveness-recovery, proxy-liveness-owner-evidence) cover unknown-result refusal and absent-result recovery.
Limits of this assessment:
The journal-order test splices source text from cli/index.ts and start-owner-fence.ts, and stubs observeProxyLiveness to return live. It checks ordering, not the real probe. Absence semantics rest on the proxy-liveness tests.
The recheck uses the same three-attempt budget as the early probe and the bind fence. A recheck that misses all attempts while the owner is live still records absence. The issue's reported scenario (first probe misses, later probe sees the owner) is covered.
Unsupervised start and ensure now acquire the ownership mutation lease before recovery. The previous code bypassed it for unsupervised starts. No deadlock or contention path was verified.
Full details: Out of Scope Changes check
Explanation
The whole-PR diff (merge base 07161c5 to head 12f10d7) is broader than the PR description's summary. The changed areas and their links to #7827 are:
src/server/proxy-liveness.ts: observeProxyLiveness returns live, absent or unknown. The connection-refusal test requires every error to be a refusal. Identity probes distinguish dead from unknown. Loopback families are probed in parallel and unavailable ones are filtered. This is the classification the lease-held recheck depends on, and the recheck cannot prove absence without it.
src/lib/loopback-family.ts and src/cli/loopback-family.ts (re-export): loopbackFamilyAvailability supplies the family availability used by the liveness probe. This targets the IPv4/IPv6 endpoint mismatch that the Windows probe miss depends on.
src/server/bind-host.ts, src/server/index.ts: normalizeBindHostname maps blank and localhost. spellings to 127.0.0.1. The liveness probe uses the same normalization, so the probe and the listener target the same address. This is a server bind behavior change for those spellings. It is a boundary case, but the link to the probe target is concrete. The PR description does not mention it.
tests/windows/windows-deploy-close-regressions.test.ts: the source-contract assertion is updated for the moved normalization. This supports the bind-host change.
docs-site/.../quickstart.md, structure/runtime.md: document the new unverified-ownership refusal and the recheck ordering.
scripts/test-layout/*, tests/fixtures/test-layout-expected.json: register the new tests required by the layout check.
tests/fixtures/refused-local-probe-preload.ts, tests/cli/cli-start-auxiliary-bind.test.ts, tests/cli/cli-client-start-owner-e2e.test.ts, tests/cli/cli-client-start-fence.test.ts, tests/cli/cli-dispatch.test.ts: supporting test fixtures and assertions for the changed recovery path.
No change was shown to be unrelated to #7827. The PR description's claim that scope "stays focused" understates the breadth. The bind-host change should be named in the PR description.

Preserve the completed-drain restart handoff
For an unsupervised dashboard restart whose drain completes, src/server/management/system-restart.ts:468 waits for replacement health before exiting the parent. The listener has stopped, but the parent's PID record is only removed by syncCleanup() at process exit. Consequently, the replacement reaches this check with refused endpoints and an alive ocx start parent, gets classified as unknown, and exits before binding. All replacement retries encounter the same condition, after which the parent exits without a serving replacement. Accommodate the known, drained restart parent, or retire its ownership records safely before waiting for replacement health; merely waiting for parent exit here would deadlock the handoff.

Keep bind scope separate from the responding loopback address
When ensure's early probe misses a live standalone proxy bound to a wildcard such as 0.0.0.0, this recovery probe returns 127.0.0.1 as live.hostname, losing the actual bind scope. handleEnsure() passes that value to reconcileEnsureDesiredIntegrations(), and src/grok/sync.ts uses it to determine admission-token requirements. This bypasses the non-loopback auto-registration guard in src/grok/inject.ts:1150 and writes a Grok configuration without the token required by the wildcard-bound listener, leaving Grok requests unauthorized. Preserve the bind hostname separately from the concrete address used for discovery, and pass the former to integration policy checks.

Catch direct ensure failures.
The restart-recovery caller catches handleEnsure errors and maps them to the existing restart failure report and exit code. The direct ensure runner does not. Its rejection reaches the top-level dispatcher, while the global crash guard only records the rejection.
Catch the direct handleEnsure call, print the terminal-safe error message, and return exit code 1.

Minor · Functional Correctness · Quick win
Fix the vacuous ordering assertion. It searches for a string that does not exist in start-owner-fence.ts.
recoverStartJournalAfterOwnerRecheck calls observeProxyLiveness(START_OWNERSHIP_LIVENESS) at src/cli/start-owner-fence.ts line 11. It does not call findLiveProxy. So recovery.indexOf("await findLiveProxy(START_OWNERSHIP_LIVENESS)") returns -1. The value -1 is always less than the index of removePidIfValueIs(owner.pidSnapshot), so the assertion passes for any code order. The test does not check that the liveness recheck runs before PID removal.
Use the real call text and assert that it is found. The assertion then fails if someone moves the recheck after removePidIfValueIs.

Follow-up validation of the two existing functional review findings, after the merge.
I checked merge commit 7a1565bb968b343f68eddd6ea266222fcfd95ff1 against current dev713fdfbe22e2c7b33851435dc0065659269f8a1b. The relevant restart, liveness, recovery fence, ensure and Grok integration sources are unchanged. The existing P1 and P2 threads are still unresolved; the security acceptance in the merge comment does not provide a disposition for either functional finding.
Using the unchanged product modules, pinned Bun 1.4.4, the repository test preload and isolated temporary homes, four reproduction/control cases completed with 23 assertions:
P1: completed-drain handoff is reproducible at the product-module boundary. Actual observeProxyLiveness classifies refused endpoints plus the still-alive ocx parent as unknown, and actual recoverStartJournalAfterOwnerRecheck refuses startup. Connecting that gate through the existing I/O seam of actual acceptSystemRestart gives completed drain → stopped listener → replacement health wait while parent is alive → parent exit 1. A control changing only parent liveness to exited permits recovery. This is a deterministic module/composition reproduction, not a real service-manager or dashboard process-replacement E2E.
P2: bind scope versus probe address is reproducible through the real Grok writer. With a 0.0.0.0 runtime, findLiveProxy preserves that bind hostname but the recovery observation returns 127.0.0.1. Passing the recovery result to actual syncGrokConfig/injectGrokConfig writes the temporary loopback configuration without the admission token required by the wildcard listener. Passing the original bind hostname instead produces the expected non-loopback refusal and no file. Server-side authentication was not bypassed; this confirms the incorrect configuration/admission-policy path.
The passing cases assert the reported defect and its control; they do not mean the defects are fixed. #7866 changes bearer-matrix test setup and does not change these product paths.
Please retain these existing threads for the maintainer follow-up: a safe completed-drain parent handoff for P1, and separate bind-scope/discovery-address values for P2, followed by their focused regressions. No duplicate issue or competing implementation was created, and neither thread was resolved by this validation.
