Re-Run the Full Cleanup After a Step Modifies the File - #1113
Process.ProcessFile() repeats cleanup after VerifyAndRepair(), because a rewrite by FfMpeg or HandBrake can undo earlier cleanup. It used to repeat only RepairMetadataErrors(), SetUnknownLanguageTracks(), RemoveTags() and RepairDefaultFlags(). It skipped cover art, the Matroska structure check, closed captions, and unwanted language and duplicate track removal.
The re-run now repeats every step from RemoveCoverArt() through RepairDefaultFlags(), in the main pass's order, except VerifyMediaInfo() and DeInterlace(). VerifyMediaInfo() still runs once at the end.
The re-run is gated on a step from RemoveCoverArt() on having modified the file. modified is set aside just before the first cleanup step and folded back in at the common exit, so the reported result is unchanged.
Otherwise the first cleanup pass already ran on the final media, and each re-run step would see the same media and the same or stronger state flags, so repeating it changes nothing.
A remux or rename before the cleanup, such as RemuxNonMkvContainer() or MakeExtensionLowercase(), does not call for a re-run.
Without the gate, the wider re-run would read the file again on every run. One example is a second FfProbe closed caption scan under --quickscan with Verify off.
The gate counts every rewrite from the cleanup on, rather than only the DeInterlace(), ReEncode() and VerifyAndRepair() results. That is because RemoveClosedCaptions() also rewrites with FfMpeg before the verify, and the old unconditional re-run cleaned up after it.
ARCHITECTURE.md "Per-File Pipeline" gains the gate as node G6 and the full re-run as P24 through P32. Suspect branch S8 is removed, and the unreleased 3.23 HISTORY.md entry is updated.
The re-run blocks drop their // Error comment lines, so the change adds no comment line.
Tests
ArchitectureDiagramTests.PipelineDiagram_MatchesProcessFileStepOrder pins the re-run's step order in Process.cs to the diagram's P24 to P34 nodes.
No test drives Process.ProcessFile() end to end, so the gate itself has no unit test.
dotnet test: 338 passed, 0 failed.
Local review
Five recorded local-strict-review passes, the last over the current head with no findings.
Fixed after the local passes
PlexCleaner/Process.cs (introduced, behavior): the first version re-ran the cleanup on every file, so file-reading steps ran twice with nothing changed. Gated on modified in 9d850c4.
HISTORY.md (introduced, wording): the entry implied a verify and repair always ran first. Reworded in 9d850c4.
ARCHITECTURE.md (introduced, wording): "each step already found nothing to do" was false for the S7 and S10 cases. It now reads "running the steps again would change nothing", in 4593ff7.
Fixed after review
PlexCleaner/Process.cs (Copilot): the run-wide modified gate also fired for the remuxes and rename before the cleanup. Narrowed to changes from the first cleanup step on in 60c8ea9.
Declined with evidence
Qodo, "Reintroduced captions escape cleanup": a repair re-encodes media the first pass already cleaned of captions, and a re-encode does not add captions its source lacks. ARCHITECTURE.md "Sidecar Refresh" states that design.
Qodo, "Remuxed files skip future verification": every re-run rewrite is lossless (MkvMerge, MkvPropEdit, or an FfMpeg stream copy with a bitstream filter), and Verified surviving a lossless rewrite is existing behavior. The old re-run's RepairMetadataRemux() already remuxed with Verified set.
Cleanup now repeats when later processing changes a media file, helping ensure cover art, tags, metadata, captions, track settings, and default flags remain cleaned after file rewrites.
Unchanged files no longer undergo an unnecessary second cleanup pass.
A re-encode, deinterlace or repair by FfMpeg or HandBrake can undo
earlier cleanup, so the steps from RemoveCoverArt() through
RepairDefaultFlags() run again, not only the metadata, unknown
language, tag and default flag steps.

Walkthrough
ProcessFile now conditionally reruns cleanup and repair operations when later processing modifies the file. The sequence includes cover art, tags, metadata, Matroska structure, captions, language tracks, duplicate tracks, and default flags. The architecture and release notes describe this behavior.

Summary
Conditional cleanup and documentation PlexCleaner/Process.cs, ARCHITECTURE.md, HISTORY.md
ProcessFile saves the prior modification state and reruns the expanded cleanup sequence when later processing modifies the file. Each rerun step checks for failure and cancellation. The method restores the saved modification state before returning. The architecture and release notes describe the rerun sequence and note that FFmpeg or HandBrake rewrites can undo earlier cleanup.

Explanation
Issue #1075 requires a second cleanup pass after a Tier 2 re-encode can undo cleanup. PlexCleaner/Process.cs gates the pass on a modification from RemoveCoverArt() through RepairDefaultFlags(). The pass includes unwanted-language and duplicate-track removal, closed-caption removal, cover-art removal, and Matroska structure repair, plus the existing cleanup steps. The pass preserves the main order and excludes VerifyMediaInfo() and DeInterlace(). The architecture test checks the documented rerun order.
Full details: Out of Scope Changes check
Explanation
The changes remain within issue #1075. PlexCleaner/Process.cs implements the rerun. ARCHITECTURE.md documents the gate and pipeline. HISTORY.md records the behavior. The architecture test supports the implementation. The PR does not implement the separate IETF tag-loss work from issues #148 and #149.

High-Level Assessment
The following are alternative approaches to this PR:
1. Extract a shared cleanup pipeline
➕ Keeps the first and second passes aligned as cleanup steps evolve.
➖ Broadens this fix into a refactor and complicates per-step failure reporting.
Recommendation: Keep the explicit, gated re-run for this focused fix: it preserves the existing operation-specific failure handling without changing the main pass. Consider a shared pipeline if maintaining the two sequences becomes error-prone.

The repeated RemoveClosedCaptions(true, ref modified) call lets FindClosedCaptionTracks treat
Verified or ClearedCaptions as a reason to skip its frame-level scan. When an FFmpeg or
HandBrake repair reintroduces captions that are not present in the ordinary video properties, the
post-repair pass returns without detecting them.
The first removal sets ClearedCaptions, and a successful repair sets Verified before the new
cleanup call. Either flag makes conditional caption detection return before GetClosedCaptions, the
frame-level scan needed when ordinary properties do not report captions.
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution
## Issue description
The repeated caption cleanup can skip frame-level detection because verification or an earlier removal left cached state flags set.

## Recommended Fix
When caption removal is enabled for the cleanup rerun, perform detection without the cached-state shortcut, while preserving the option's ability to disable removal. Add coverage for captions detected only by the frame-level scan after a repair.

The new post-verification RemoveUnwantedLanguageTracks call can remux the file while leaving its
Verified state set. If it removes a track after VerifyAndRepair, the refreshed sidecar records
the changed media as verified, and a later conditional Verify returns without checking its
streams.
VerifyAndRepair precedes the added track removals and sets Verified on success. The removals
remux and refresh without clearing it; refresh retains the state, and conditional verification
explicitly skips a file carrying Verified. The same post-verification state issue applies if the
newly repeated duplicate-track removal remuxes.
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution
## Issue description
New cleanup remuxes can run after stream verification, but their refreshed sidecars retain verification state for the previous file contents.

## Recommended Fix
Track whether the post-verification cleanup changes media and invalidate its verification-related state when it does, including when a subsequent cleanup step fails. Ensure a later run verifies the changed output rather than accepting the prior result, and test a post-verification track-removal remux.

🟡 Changes recommended
The only active finding is a moderate issue: the re-run gate uses the run-wide modified flag, so it can repeat cleanup after changes that happened before the cleanup ran, including an extra closed-caption read under --quickscan.
This PR fixes a cleanup gap in Process.ProcessFile(). A rewrite by FfMpeg or HandBrake can undo earlier cleanup, but the re-run after VerifyAndRepair() repeated only four steps. The re-run now repeats the full cleanup sequence, and it runs only when a step in the run has modified the file. This closes #1075.
Changes:
The cleanup re-run now covers cover art, tags, metadata, the Matroska structure check, closed captions, unknown and unwanted language tracks, duplicate tracks, and default flags, in the main pass order.
The re-run is gated on the modified flag, so it runs only when a step rewrote the file.
ARCHITECTURE.md gains the gate and the re-run nodes, the S8 suspect branch is removed, and the 3.23 HISTORY.md entry is updated.

Add a test for the if (modified) rerun gate.
The order test in ArchitectureDiagramTests reads only operation = nameof(processFile.X); lines. It matches the rerun steps at lines 329-391, but it does not inspect the if (modified) gate at line 327. If the gate is changed to always false, the step sequence stays the same and the test still passes. No test in PlexCleanerTests names a rerun step or runs a file with modified set through the rerun path.
Add a test that runs ProcessFile with a step that sets modified, and assert that a rerun step such as RemoveCoverArt or RepairDefaultFlags executes. This covers the re-run behavior that the gate enables.

Declined with evidence. No harness in PlexCleanerTests runs Process.ProcessFile() past MakeExtensionLowercase(). PerFileExceptionTests.ProcessFile_StepThrows_FailsAndKeepsFile is the only call, and it fails at that step. To reach the gate, a test would have to fake MediaInfo, MkvMerge, FfProbe, FfMpeg and the verify through every step before it. The first-pass and re-run steps are the same methods with the same inputs, so such a test could observe the re-run only by counting tool calls through those fakes. The gate's case analysis is in ARCHITECTURE.md after the "Per-File Pipeline" diagram, and the re-run's step order is pinned by ArchitectureDiagramTests.PipelineDiagram_MatchesProcessFileStepOrder.
