[minor] Suppress task notifications for automatic compaction · logic
Automatic compaction creates an ordinary run whose completion triggers task-completion alerts and relay activity. Users with notifications enabled receive alerts for background maintenance; compaction failures can likewise produce failure alerts.
When it breaks: Enable cache-expiry compaction and client notifications or relay activity publishing. Leave an eligible large Claude or Codex thread idle until automatic /compact runs and finishes.
Evidence (level 4: ran code): CacheExpiryCompactionWorker.ts:44-56 dispatches /compact with a command-only marker. Orchestrator.ts:6077-6105 creates a normal run without preserving that marker. RunExecutionService.ts:654-700 routes successful termination through checkpoint capture; CheckpointCaptureService.ts:201-216 marks the run completed with a fresh timestamp. ProjectionStore.ts:1514-1519,1593-1601 exposes it as the latest run. ThreadNotificationCoordinator.tsx:144-173 detects the newer completion and plays sound; lines 175-223 produce a focused-client toast or an unfocused-client OS notification. packages/shared/src/agentAwareness.ts:61-81,98-108 projects completed activity; AgentAwarenessRelay.ts:98-103,540-559,613-617 publishes changed run states without maintenance filtering. A scratch test importing the real worker, orchestrator, shell projection and notification coordinator passed all 6 tests using node_modules/.bin/vp test run --config /home/kyleb/.cache/crev/20261016T151308-no5qq/scratch/c2/proof.config.mjs --reporter verbose. Injecting compaction completion demonstrated sound, toast, mocked OS notification and completed relay projection. No running-app or phone-push reproduction was performed. The claim overstates defaults: packages/contracts/src/settings.ts:300-303 defaults notifications to off. The worker does not exist at the supplied base commit.
Suggested fix: Persist an explicit automatic-maintenance marker on compaction runs and expose it in thread shells. Use it to suppress task-completion and failure notifications and relay activity transitions for those runs, while retaining normal alerts for user-requested turns.
Raised by bugs-opus; verified by gpt-6.1-sol. Finding r1-5.

Verify this finding against the current code before changing anything. In apps/server/src/orchestration-v2/CacheExpiryCompactionWorker.ts:44-56, automatic /compact dispatches become ordinary runs because Orchestrator.ts:6077-6105 does not preserve their maintenance identity. Their terminal states trigger ThreadNotificationCoordinator.tsx alerts and AgentAwarenessRelay.ts activity updates. Persist an explicit automatic-maintenance marker through run creation and shell contracts, then suppress notification and relay transitions for these runs. Do not suppress all agent-created runs. Add focused tests covering successful and failed automatic compaction, including a subsequent ordinary turn that must still notify.
[minor] Prompt cache TTL still comes from aggregate usage while token counts come from the last main iteration · apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.ts:236
[minor] Prompt cache TTL still comes from aggregate usage while token counts come from the last main iteration · logic · pre-existing, on changed lines
claudeProviderTurnTokenUsage now takes context tokens from the last type === "message" iteration, but it still derives promptCacheTtlMs from top-level usage.cache_creation, which adds up every iteration, advisor and compaction passes included. claudePromptCacheTtlMs returns 1h whenever any 1h write is present. A side inference's cache write can therefore set the TTL that schedules cache-expiry compaction for the main conversation. It can also override the carried-forward knownPromptCacheTtlMs.
When it breaks: A root assistant message whose usage.iterations has an advisor_message (or compaction) entry with cache_creation.ephemeral_1h_input_tokens > 0, and a final message iteration that wrote only 5m cache. The aggregate gives a 1h TTL, so compaction is scheduled about an hour out, but the main cache expires in 5 minutes. The reverse also happens: the main iteration only reads cache while an advisor writes 5m. The 5m write then replaces a known 1h TTL and compaction fires early.
Evidence (level 3: traced the failing path): - ClaudeAdapterV2.ts:230 picks context = last "message" iteration and uses it for input, output and cached tokens (lines 231-234, 241).
- ClaudeAdapterV2.ts:236 still calls
claudePromptCacheTtlMs(usage.cache_creation)on the aggregate, notcontext. - ClaudeAdapterV2.ts:190-197: any 1h write wins over 5m.
- ClaudeAdapterV2.ts:206-209: the change's own comment says advisor_message and compaction entries "are side inferences with their own context".
- .repos/effect-smol/packages/ai/anthropic/src/Generated.ts:3304-3377: BetaMessageIterationUsage and BetaCompactionIterationUsage each carry their own
cache_creation: BetaCacheCreation | null, so a per-iteration TTL is available. - ClaudeAdapterV2.ts:~6858-6864: the result is stored as
context.promptCacheTtlMsand carried into later responses. - Pre-existing: base cfafb4b line 216 already had
claudePromptCacheTtlMs(usage.cache_creation)on the aggregate, so for any single message the computed TTL is unchanged. This change adds the mismatch with the token source and carries the possibly wrong value forward. - Not run: node_modules is absent and Bash was restricted. I did not verify whether the Claude CLI actually emits advisor iterations with a TTL different from the main request.
Suggested fix: Use the selected main iteration's cache breakdown when iterations are reported, and fall back to aggregate usage otherwise:
interface ClaudeUsageIteration extends ClaudeUsageCounters {
readonly type: string;
readonly cache_creation?: ClaudeCacheCreation | null;
}
...
const mainIteration = usage.iterations?.findLast((i) => i.type === "message");
const context = mainIteration ?? usage;
const promptCacheTtlMs =
claudePromptCacheTtlMs(mainIteration ? mainIteration.cache_creation : usage.cache_creation) ??
knownPromptCacheTtlMs;Add a test where an advisor_message iteration writes 1h and the last message iteration writes 5m, and assert a 5-minute TTL.

Verify this finding against the current code before changing anything. In apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.ts, `claudeProviderTurnTokenUsage` (around lines 220-247) selects `context` as the last `usage.iterations` entry with `type === "message"` and takes token counts from it. At line 236 it still computes `promptCacheTtlMs` from the aggregate `usage.cache_creation`, which includes advisor_message and compaction iterations. Each iteration carries its own `cache_creation` (see BetaMessageIterationUsage in .repos/effect-smol/packages/ai/anthropic/src/Generated.ts). Fix: add `readonly cache_creation?: ClaudeCacheCreation | null` to `ClaudeUsageIteration`. When a main message iteration was found, compute the TTL from `mainIteration.cache_creation`; otherwise fall back to `usage.cache_creation`. Keep the `?? knownPromptCacheTtlMs` fallback. Add a focused test in ClaudeAdapterV2.test.ts next to "measures context from the last sampling pass of a server-side tool loop": an advisor_message iteration with ephemeral_1h_input_tokens > 0 and a final message iteration with only ephemeral_5m_input_tokens > 0 should give promptCacheTtlMs === 5 * 60_000. Run `vp test run src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts` from apps/server.
[minor] Cache-expiry compaction runs on a cold cache when the last response took longer than the lead time · apps/server/src/orchestration-v2/CacheExpiryCompactionWorker.ts:35-42
[minor] Cache-expiry compaction runs on a cold cache when the last response took longer than the lead time · logic
The worker starts the prompt-cache lifetime at tokenUsage.updatedAt. The Claude adapter stamps that field when a root assistant message arrives, which is the end of the response. The cache was refreshed when the request started. For a 5-minute TTL the lead is a fixed 2 minutes, so if the last response took 2 minutes or more, every sweep window falls after the cache has already expired. The /compact then reads the whole large context cold, which is the cost this feature exists to avoid.
When it breaks: Claude thread over the min-token threshold with a 5-minute cache TTL. The final API request of a turn starts at 10:00 and its last assistant message arrives at 10:03, so usageUpdatedAt is 10:03. The window is idleMs in [3m, 5m), which is 10:06 to 10:08. The 5-second scheduler tick dispatches /compact at about 10:06, but the cache expired at 10:05.
Evidence (level 3: traced the failing path): - CacheExpiryCompactionWorker.ts:35 sets reportedAtMs = Date.parse(candidate.usageUpdatedAt).
- CacheExpiryCompactionWorker.ts:40-42:
leadMs = max(120000, ttl/10), which is 120000 for a 300000 TTL. The window isidleMs ∈ [ttl-lead, ttl)= [180000, 300000) after reportedAt. Nothing accounts for how long the response took. - The comment at lines 37-38 admits the gap ("a long response eats into the window") but only covers it with the fixed lead.
- ClaudeAdapterV2.ts:6848-6862 handles
message.type === "assistant"by computingnow = DateTime.nowand passingDateTime.formatIso(now)asupdatedAttoclaudeProviderTurnTokenUsage. That is the arrival time of the assistant message, so it lands at the end of generation. - ProjectionStore.ts:3737 reads that same
$.tokenUsage.updatedAtasusage_updated_at. The projection replaces tokenUsage on each update (ProjectionStore.ts:749-751), so the latest message wins. - Scheduler.ts:57 ticks every 5 seconds, so dispatch happens at the start of the window, about 10:06 in the trigger and after the 10:05 expiry.
- ClaudeAdapterV2.ts:1139 sets
includePartialMessages: trueand :6589 handles rootstream_events, so the adapter does seemessage_start, which is close to when the cache was refreshed. git show cfafb4bb54:...CacheExpiryCompactionWorker.tsfails because the file is new in this change.- I traced the arithmetic by hand. A scratch run was blocked by tool permissions, so the code was not executed.
Suggested fix: Anchor the cache lifetime to when the final request started, not when its response arrived. In ClaudeAdapterV2, record the time of the root stream_event message_start for each message id. Pass that time as updatedAt to claudeProviderTurnTokenUsage, or add it as a separate tokenUsage.cacheRefreshedAt and have the projection query and worker use that field. Then reportedAtMs is close to the cache refresh time, and leadMs only has to cover the sweep cadence and provider resume time.

Verify this finding against the current code before changing anything. In apps/server/src/orchestration-v2/CacheExpiryCompactionWorker.ts:35-42, `cacheExpiryCompactionCommand` measures cache age from `candidate.usageUpdatedAt`. That value comes from `$.tokenUsage.updatedAt` (ProjectionStore.ts ~3737), which ClaudeAdapterV2.ts (~6848-6862) stamps with `DateTime.now` when a root assistant message arrives, i.e. after the response finished generating. Anthropic's prompt cache is refreshed when the request starts. With a 5-minute TTL the lead is 2 minutes, so any final response taking 2 minutes or more makes the worker dispatch `/compact` after the cache has expired. Fix: the adapter already receives root `stream_event`s (includePartialMessages is on; handler near line 6589). Record the `message_start` time for each root message id, and use it as the cache-refresh timestamp, either as the `updatedAt` passed to `claudeProviderTurnTokenUsage` or as a new `tokenUsage.cacheRefreshedAt` field read by `getCacheExpiryCompactionCandidates` and the in-memory `cacheExpiryCompactionCandidateFromProjection`. Fall back to the arrival time when no message_start was seen. Add a focused test in CacheExpiryCompactionWorker.test.ts / ClaudeAdapterV2.test.ts: a response that starts at T and finishes at T+3m with a 5m TTL must produce a compaction window that ends before T+5m.
Uncertain: the verifier could not decide (1)
apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.ts:8328Claude prompt-cache TTL is only carried within a turn, not across turns [minor]: ClaudeAdapterV2.ts:8328: the new turn context sets promptCacheTtlMs to undefined. ClaudeAdapterV2.ts:236 and 6858-6864: claudeProviderTurnTokenUsage falls back to context.promptCacheTtlMs only within the current turn. ClaudeAdapterV2.ts:191-198: when both ephemeral counts are 0 the result is undefined. ProjectionStore.ts:3740-3745 and 3790: the candidate is built from the latest provider turn's tokenUsage.promptCacheTtlMs only. CacheExpiryCompactionWorker.ts:18: a Claude candidate with an undefined TTL gets no command. So the code path exists. What is not settled is whether it can be reached: a normal follow-up turn adds new prompt tokens after the cached prefix, and Claude Code's breakpoint on the latest message makes them a cache write, so the trigger's "read-only follow-up" may never happen. The only test with a read-only response (ClaudeAdapterV2.test.ts:3840-3859) is synthetic and within one turn. What would settle it: a captured real Claude SDK usage frame for a follow-up turn's final root assistant message on a large thread showing cache reads with zero 1h/5m cache creation. If one exists, the claim is confirmed. If it is impossible, the claim is refuted. Bash was unavailable, so I could not run a repro.
Dropped by verification (2)
apps/server/src/orchestration-v2/Orchestrator.ts:4507-4511Automatic /compact can run after the cache expired or the setting was turned off: no realistic path [minor]: - apps/server/src/scheduling/Scheduler.ts:57 runs every registered source every 5 seconds. Line 37 (withPermitsIfAvailable) skips a tick while the previous sweep is still running, so sweeps never pile up.- apps/server/src/orchestration-v2/CacheExpiryCompactionWorker.ts:40-42: the lead is at least 2 minutes (
Math.max(2*60_000, ttl/10)). A thread becomes eligible at ttl−lead, and the next 5s tick picks it up, about 3:00–3:05 idle for a 5-minute cache. It is not picked up near 4:58. - CacheExpiryCompactionWorker.ts:64-72: the sweep reads the settings and candidates, then immediately calls
threads.dispatchfor each one. ThreadManagementService.ts:672-675 goes straight toorchestrator.dispatch. The orchestrator decides events with no provider I/O (AGENTS.md "How it works"), so per-thread lock holds are short commits, not long-running work. - The commandId is deterministic (
cache-expiry-compact:${threadId}:${providerTurnId}, Worker.ts:43-46). A later tick does not create a second, late compaction for the same turn. - What is left is a sweep read that lands milliseconds before ttl or before the user turns the setting off, followed by a dispatch milliseconds after. That window is far smaller than how imprecise the TTL boundary already is: usageUpdatedAt is recorded when the response arrives, after the cache-refreshing request started (Worker.ts:37-39). Re-checking the TTL inside the lock would not add meaningful protection.
packages/client-runtime/src/state/sharedSettings.ts:19Scoped settings already report unsupported compaction writes [minor]: SettingsPanels.tsx:2145 uses useUpdateScopedSettings, which calls planScopedSettingsPatch at useScopedSettings.ts:114-119, bypassing useSettings.ts shared synchronization. scopedSettings.ts:403-449 compares returned settings against the requested change and reports an unsupported-setting failure; useScopedSettings.ts:93-107 displays it. findSharedSettingsMismatches has no production callers. Ran scratch/c7/verify-compaction-setting.mjs against the real planning and persistence modules: selecting only the fork writes only there; selecting both and simulating an upstream success response with unchanged null produces one saved environment and one explicit unsupported-setting failure. The base sharedSettings.ts lacks the added key.
Tier 7/10 from claude-opus-5-5: Adds a background worker that automatically sends /compact to idle threads. It also changes orchestrator dispatch for settle and snooze state and changes Claude token usage accounting. A mistake could silently start unwanted runs or corrupt thread state.
Complexity high: The candidate logic lives in two places, a SQL query and an in-memory version, and they must agree. It also involves timing windows, rechecks under the thread lock and a contract field that crosses modules, spread over many server logic files.
apps/server/src/orchestration-v2/ProjectionStore.ts:209-217
[major] Block automatic compaction while background agents are running · race
A completed root run qualifies for cache-expiry compaction even when its provider thread still has running subagents, monitors, or background tasks. The orchestrator repeats the same incomplete check, allowing automatic /compact to summarize context during live background work.
When it breaks: Enable automatic compaction at 150,000 tokens. A large Claude root turn completes with a 5-minute cache TTL while background work continues beyond 3 minutes.
Evidence (level 4: ran code): ProjectionStore.ts:209-249 and :3773-3781 check runs and runtime requests but omit pendingBackgroundTasks. ClaudeAdapterV2.ts:5874-5890 preserves the background roster while completing the root turn and clearing activeTurn; CheckpointCaptureService.ts:208-216 then marks the run completed. Orchestrator.ts:4508-4510 reuses the incomplete predicate. ClaudeAdapterV2.ts:8081-8094 reuses the matching live query before the background-work replacement guard, and :8367-8370 offers the new prompt. Ran node /home/kyleb/.cache/crev/20261016T151308-no5qq/scratch/c1/background-compaction.mts, importing real modules and using in-memory SQLite: each of subagent, monitor, and background_task produced sqlCandidates=1, memoryCandidate=true, and a start_immediately /compact command at 3 minutes. This demonstrates selection and command generation, not a live CLI compaction. The base ProjectionStore has no cache-expiry compaction feature.
Suggested fix: Reject candidates whose active provider thread has completion-holding background work. Apply equivalent checks in both cacheExpiryCompactionCandidateFromProjection and the SQL candidate query, preserving the orchestrator's dispatch-time recheck. Add focused coverage for each blocking task kind and for background work appearing after candidate selection.

Keep the Orchestrator.ts:4508-4510 recheck effective when background work starts after a sweep.
