fix(messages): normalize Messages error responses and Anthropic headers - #8153
Messages and count_tokens error responses now carry proxy-written error bodies, and Anthropic rate-limit headers are normalized. Same approach as the earlier Chat Completions error-response normalization.
Non-2xx error bodies on native, passthrough, translated, converter and direct-encoder paths use a fixed message for each classified condition, closed error.type / code tables, and an integer Retry-After. Proxy-authored local refusals keep their existing wording.
Error records and stream error events that the proxy builds itself use the same owned messages. This covers folded native responses, converter and direct streaming, and collected responses.
Status codes and Claude Code compaction are unchanged. On every path, the status code and whether the client message contains prompt is too long / input is too long for requested model match the previous behavior case for case. When the previous response would have triggered compaction, the new body contains the fixed compaction sentence.
Successful (2xx) passthrough Messages and count_tokens bodies are relayed byte for byte as before. Native SSE relay bytes are unchanged.
Headers:
anthropic-ratelimit-* keeps recognized names with regenerated values (bounded numbers; RFC 3339 resets re-rendered in UTC with the validated fraction). Unrecognized names and invalid values are omitted.
The upstream request id is no longer forwarded. x-opencodex-upstream-request-id and its CORS exposure are removed. The proxy's request-id and x-opencodex-request-id remain.
Successful passthrough bodies get a media type from a closed list.
Internal evidence is unchanged. Routing, rotation, billing, quota, compaction and request-log classification still read the original upstream error.
Docs:reference/proxy-formats.md (request id) and guides/claude-code.md (rate-limit headers) are updated. No locale copy mentions either passage.
Verification
Comparison with the previous behavior:tests/claude-integration/messages-overflow-verdicts.test.ts compares 5,097 cases across every Messages path against a committed baseline, tests/fixtures/messages-overflow-decisions.json. The baseline was generated from 72e80d280. The test checks status and compaction-trigger parity, byte parity for 2xx passthrough (hashes of the raw client bytes), and that error-shaped responses contain no upstream bytes. It needs no git at test time.
Native streaming:tests/claude-integration/messages-native-stream-parity.test.ts compares native stream bytes, settlement, cancellation and callbacks with the base outcomes in tests/fixtures/messages-native-stream-parity.json. Those outcomes were generated from 72e80d280.
New behavior tests:messages-error-contracts, messages-error-normalization, messages-header-normalization, messages-lane-errors, messages-success-media-types, messages-overflow-baseline-contract (all under tests/claude-integration/). They are registered in the test layout maps.
Focused run: every touched and new test file run in isolation with a separate OPENCODEX_HOME/CODEX_HOME, plus one aggregate run of the Messages suites. 0 failures.
Full suite:bash scripts/ci/run-bun-test-batches.sh N/4 for N = 1–4 on this head.
Shards 1, 2 and 4: 0 failures.
Shard 3: three failures in tests/codex-integration/codex-cli-update-apply.test.ts. The same three cases fail on origin/dev; they are blocked by Codex sessions active on the test machine.
Failures present only on this branch: none.
Other checks:bun run typecheck, bun run privacy:scan, bun run structure:check, git diff --check and the file-size ratchet all pass.
Labels:ci:full is set for the full Ubuntu shards.

Summary by CodeRabbit
Improvements
Messages errors now use consistent, sanitized client-facing messages while retaining relevant status and error classifications.
Upstream request IDs are no longer exposed in Messages responses; proxy request IDs remain available.
Anthropic rate-limit headers are validated and normalized, with invalid or unrecognized values omitted.
Successful Messages responses use normalized content types, with unsupported types falling back to a safe default.
Documentation
Updated guidance for Messages error responses, request IDs, rate-limit headers, and Claude context-limit behavior.
Restore compaction wording from original overflow evidence while retaining
throughput exclusions and existing status, type and code decisions. Render
successful passthrough media types from closed media and charset tables.
Add executable baseline-versus-current native stream fixtures alongside
buffered lane regressions and document the response contracts.
Use original error evidence for compaction presentation and preserve the
existing status, type and code decisions. Retain original adapter fields
separately from shared terminal classification.
Compare native streams with a generated baseline golden during tests,
keeping git history access in the explicit fixture generator only.
Select compaction wording from the existing native, translated and direct
path verdicts. Match the client phrase predicate for buffered passthrough
Messages and count_tokens replies.
Add a baseline decision golden across all Messages paths and bind the
unchanged shared predicate and its behavior suite to their source bytes.
Select normalized overflow wording from each path's baseline client message
while retaining its status and internal decisions. Compare actual recovery
recognition and status against regenerated baseline outcomes on every lane.
Preserve success bodies and baseline client recovery outcomes across relayed
and incomplete error shapes. Retain millisecond precision in reset dates and
extend the baseline comparison to cover the accepted envelope variants.
Normalize only non-2xx passthrough bodies and compare 2xx replies byte for byte
with baseline. Record literal case classifications and retain full validated
reset fractions through integer UTC calendar conversion.
Record error-event dispatch independently of payload parsing and use it in
native fold and raw collection. Preserve original status and recovery outcomes,
and verify all payload shapes with literal baseline expectations.
Messages error handling now creates owned Anthropic envelopes, preserves structured upstream evidence, classifies proxy errors, normalizes rate-limit and request-ID headers, and adds coverage for native, translated, streaming, passthrough, refusal, and overflow paths.

Skip the overflow inspection on successful passthrough responses.
For a successful non-stream response without a top-level message, messagesRelayedOverflowPresentation runs JSON.stringify on the full body. The result is discarded because only error responses use overflow. Return the original body before this parse and inspection. Deeply nested JSON can also make this serialization throw, which the repository does not bound.

Route untyped upstream event: error frames to the error branch.
When an SSE error event omits top-level type, the non-stream path returns the raw object. The gate at src/server/messages-native.ts:972 does not match it, so the request logs finishLog(200) and returns HTTP 200 with an error body. The same payload with type: "error" returns 502 through the same function. Include collectorVerdictSink.collectedErrorEvent in that gate, and set the status for untyped error events in the same change.
This path predates the PR. The merge-base code has the same gate and success return. The PR keeps status codes unchanged, so the fix needs a status decision.
Based on learnings: "In src/server/claude-messages.ts, tapAnthropicSseForLog must preserve original forwarded SSE bytes." The tap forwards the frame that the collector reads.

Keep legacy error records only for buffered delivery, distinguish iterator read
faults from writer faults, and return successful passthrough before error work.
Partition the unchanged overflow comparison into bounded per-lane slices and
cover local refusal and delivery behavior with isolated fixtures.
Skip the overflow inspection on successful passthrough responses (claude-messages.ts): fixed in 9cac7ce. A successful passthrough now returns its original body right after the existing guarded usage parse, before any error parsing, overflow inspection or serialization. A test covers a deeply nested 2xx body relayed byte for byte.
Route untyped upstream event: error frames to the error branch (messages-native.ts): deferred. As the review notes, this predates the PR, and fixing it means changing the response status. This PR keeps status codes equal to the previous behavior on every path. The response body for that case is already the proxy-written error record. The status decision is tracked for a follow-up change.

Kiro continuation drops the proxy marker before the presentation is recorded.
When a Kiro continuation ends with a proxy-authored refusal, normalizeFinalKiroHttpError returns a new unmarked Response. The guard at src/server/responses/adapter-continuation.ts:705 then skips rememberProxyErrorMessage, so the presentation argument at line 706 is never recorded. The Messages encoder then emits the normalized upstream text instead of the fixed presentation message. Drive the Copilot refusal test through copilotRefusalResponse.
The copilot case builds its own marked response and never calls production code. The fixture's __localRefusalReply hook replaces handleResponses, so copilotRefusalResponse never runs. Removing its "copilot" presentation leaves this test passing. Make the fixture trigger a real Copilot negotiation failure, so the production producer runs.
As per path instructions: "A behavior change in src/ should come with a focused regression test near the existing tests for that subsystem." The new production presentation in copilotRefusalResponse has no test that reaches it.

messagesRelayedOverflowPresentation serializes an entire parsed error when there is no truthy top-level message. A sufficiently deep, otherwise valid upstream error JSON can make JSON.stringify throw during response normalization.
Instead of a normalized response retaining the upstream status, the request can fail while inspecting overflow.
This can occur when: A non-2xx upstream Messages or count_tokens response contains deeply nested JSON and no top-level message.

Add a Content-Type check to rebuilt Messages error rows, scoped to lanes with a rebuilt JSON body.

Their error rows carry no Content-Type in the baseline, and their SSE stream bodies are not established as rebuilt JSON error bodies. Pin the expected error type for shared-8128-27 in four lanes.
tests/helpers/messages-overflow-assertions.ts:63-64 only checks that the type belongs to ANTHROPIC_ERROR_TYPES when the baseline type is outside that set. server_error is outside the set, so for passthrough, count_tokens, native-http, and direct-collector, a different closed-set type passes the row. The status-based fallback in ownedAnthropicErrorPayload (src/lib/messages-error-body.ts:71-73) gives the expected types for this fixture:
passthrough and count_tokens (status 429): rate_limit_error. Both lanes reach the call at src/server/claude-messages.ts:463-470.
native-http (status 429): rate_limit_error.
direct-collector (status 502): api_error.
Add a per-fixture expectation for these four lanes. Keep the strict comparison for baselines inside ANTHROPIC_ERROR_TYPES. Do not apply the status table to every out-of-set baseline in assertRow. The fallback rule is not established for other fixtures.

branch after the fixedLocal return, only for lanes whose error bodies are
rebuilt JSON; exclude converter and direct-stream lanes. Assert the literal
application/json rather than comparing with before.mediaType.
- Around line 59-62: Update assertRow in the messages-overflow assertions to
apply explicit expected error types for shared-8128-27 in the passthrough,
count_tokens, native-http, and direct-collector lanes: rate_limit_error for the
first three and api_error for direct-collector. Keep the existing strict
comparison when the baseline type is in ANTHROPIC_ERROR_TYPES, and do not
generalize the status-based fallback to other out-of-set baselines.
Review comments at @tests/helpers/messages-proxy-refusal-case.ts:
- Around line 13-14: Update the source transformation in the fixture’s
`build.onLoad` handler to verify the failure-verdict anchor exists before
replacing it. Use a small helper that throws when the anchor is missing, so the
fixture fails rather than silently omitting sink observation.

Drop the upper bound on spend ceiling values in the workflow refusal sentence, document the bounded terminal-frame accounting difference, and state the 64 KiB error-body bound in bytes.
Pin the exact guidance text for the cooldown and recovery refusals.
The loop checks only the status, the code, and that the body is non-empty. A generic sentence that replaces the cooldown or recovery entry in FIXED_MESSAGES would still pass. Both entries are returned through normalizeMessagesProxyRefusal, so the test can assert their exact error.message values.

buffered collector charges the same byte size reserved from legacy, without
changing baseline admission decisions. Reconcile replaceRetained accounting for
the error frame’s longer payload so it cannot exceed the cap solely because
payload is longer than legacy, while preserving the original 502 or 400 status
from collectAnthropicMessageResponse.
Review comments at @src/protocols/encoders/messages.ts:
- Line 220: Update the terminal error emission through sink.emit so an owned
error frame that exceeds the remaining budget cannot replace the original 400 or
502 with a 413. Preserve the budget safeguards by using the existing legacy
error frame as the bounded fallback when the owned frame cannot be collected.
Review comments at @src/server/claude-messages.ts:

without the diagnostic size gate, causing unbounded CPU and allocation costs
when bodyMaxBytes is zero. Before changing the parse near parsed or
messagesRelayedOverflowPresentation, establish a bounded recognition design that
preserves matching behavior for large overflow bodies; do not simply add a size
gate or pass raw text to the overflow check, since either changes recognition.

behavior for other refusal presentations unchanged.

passes logCtx.spendRefusalDetail, preserving the existing arguments and ensuring
recorded unbound pool history selects the unbound refusal wording.

Maintainer integration decision (dev, maintainer bypass per MAINTAINERS.md).
Exact head: e61e3df. One commit behind dev; no file overlap with it.
Required CI on this head: ci, enforce-target, hygiene, gates, test 1/4-4/4 (ci:full) all pass.
Review threads: 0 unresolved. Codex review: no findings. CodeRabbit findings fixed, rebutted with evidence, or deferred as non-blocking follow-ups.
Code review: fresh final review passed with no blocking findings; the later delta was rechecked.
Security review (required for this change, which touches client error bodies and headers): independent review passed on c5cc65e. The only change after that head is a two-line bound removal in src/lib/workflow-budget.ts (numeric values only) plus docs and a test, reviewed by the maintainer.
Acceptance rule: HTTP status and the client-visible compaction trigger equal origin/dev on every covered case (golden comparison, 5,097 cases); 2xx passthrough bodies and native SSE relay bytes unchanged.
Known limit, accepted as a follow-up: buffered Messages collectors charge the proxy-written terminal error frame against the turn byte budget, so a turn within a few hundred bytes of the limit can answer 413 instead of the earlier status.
NOT RUN: cross-platform CI, installed Claude Code runtime checks, real-provider calls.
Resolve messages-native.ts against dev's owned Messages error responses
(lidge-jun#8153/lidge-jun#8164): keep dev's fail/failEvidence and stream frame hooks, with the
combo commitment flag marking both non-replayable after preflight accepts.
Drop this branch's tap changes that logged an upstream error event with its
mapped status and recorded usage on client cancel. Dev's executable parity
goldens (messages-native-stream-parity, messages-sse-passthrough-errors) pin
the existing 200 / usage-less rows, and neither change is needed for the
native combo cache replay; the combo preflight still reads the failure before
the first output byte. Tests now assert dev's normalized error wording.
