fix(responses): preserve adapter body stall status - #8200
Adapter response-body stalls previously lost their explicit 504 and bounded duration while passing through the Responses terminal. Initial, recovery, terminal-guard continuation and empty-completion retry stalls now emit upstream_stall_timeout with the guard's bounded duration; request-log classification records 504 and can retain body_stall. Already-committed SSE HTTP headers are unchanged.
Buffered initial and terminal-guard continuation stalls return HTTP 504. A request-local marker survives continuation event spreads without being serialized or forgeable by a provider, so other buffered failures and provider-supplied timeout codes keep their existing HTTP status. The approved scope includes src/server/responses/adapter-continuation.ts and src/server/responses/empty-completion-guard.ts; the empty-completion guard preserves only locally marked stalls and still replaces ordinary retry failures.
Document adapter-body timeouts separately from the bridge watchdog in the public Responses architecture reference and owning structure docs.
Reference: RFC 9110 §15.6.5 defines 504 for a gateway that did not receive a timely upstream response. The existing response-body inactivity guard and bounded failure-detail classifier supply the local contract. Aside was unavailable on the Linux host; the hosted search tool returned 401, so the RFC was fetched directly.

Summary by CodeRabbit
Bug Fixes
Adapter response-body inactivity timeouts now return HTTP 504 with a bounded-duration message for buffered initial and continuation responses.
Streamed initial, recovery, and continuation timeouts now include the upstream_stall_timeout code; already-sent stream headers remain unchanged.
Empty-completion retries preserve stall-timeout errors, and timeout details are recorded as body_stall. Other upstream failures retain their existing classification and status behavior.
Documentation
Updated architecture and error-detail references to describe timeout behavior, status codes, and classification.
Adapter body-inactivity errors now retain their duration message and explicit upstream_stall_timeout classification. Buffered initial and locally marked continuation stalls return HTTP 504. Tests and documentation cover status classification and body_stall details.
Changes: Adapter body stall handling

Summary
Identify continuation body stalls src/server/responses/adapter-continuation.ts, src/server/responses/empty-completion-guard.ts
Continuation stalls receive a request-local marker and structured timeout error. The empty-completion guard can preserve selected retry errors.
Deliver and classify adapter body stalls src/server/responses/adapter-delivery.ts, src/lib/errors.ts
Delivery uses the inactivity error message for timeout errors and preserves locally marked continuation stall errors. Failed buffered responses return HTTP 504 for locally classified continuation stalls. Terminal classification maps upstream_stall_timeout to HTTP 504.
Validate and document timeout outcomes tests/server/*, structure/data-planes/error-detail.md, structure/transports/streaming-health.md, docs-site/src/content/docs/reference/architecture.md
Tests check streamed and buffered timeout statuses, duration messages, body_stall details, and cases that retain HTTP 200. Documentation describes the timeout classification and handling.
lidge-jun/opencodex#8206: The issue reports a translated Chat body stall with generic upstream classification. This change adds upstream_stall_timeout to adapter timeout errors; native Responses SSE timeout enforcement is separate.

Split this into a short, dedicated section. Link the canonical owner doc instead of repeating the buffered-status rules.

Review: a streamed adapter body stall keeps its explicit 504 with code upstream_stall_timeout and the measured duration through initial, recovery, terminal-guard continuation and empty-completion retry paths; buffered folds return HTTP 504 for local stalls while provider-supplied timeout codes and ordinary failures keep their existing status. Red-to-green regressions for each path; public architecture docs updated.

Canonical ChatGPT and official OpenAI native /responses/compact sends
forwarded OpenCodex-owned ocx1:/ocx2: compaction envelopes verbatim, so the
upstream rejected them as invalid_encrypted_content. Only the custom gateway
send lowered them. Run scrubOcxCompactionItems on the non-gateway send as
well, so the envelopes become plain user summaries like on ordinary turns,
while native blobs stay byte-identical for these decoding destinations.

to a non-negative safe integer.
* fix(terminal-guard): pass completion summaries that follow tool results (lidge-jun#8154)
* fix(terminal-guard): pass completion summaries that follow tool results
The Anthropic terminal guard read the latest user text and ignored tool
activity after it, so a normal turn (request -> tool calls -> tool
results -> short "done" summary) was classified suspicious_no_tool. The
relay then appended the plan-only nudge and re-asked the model, which
re-ran tools it had already run and repeated the cycle on every client
turn.
Tool calls or results after the latest user message now mark the turn as
already executed; only a renewed plan announcement (PLAN_INTENT_RE) is
still continued.
* fix(terminal-guard): stop the tool-activity pass at a stalled plan announcement
A text-only plan announcement after the last tool activity means the turn
stalled there, so a following completion is continued again, matching
recentTurnHasToolActivity. Pin the boundary for next-step wording: first-person
plans still continue, while a summary that suggests a next step to the user
passes.

* fix(mistral): keep replayed images within each model's per-request cap (lidge-jun#8174)
Mistral rejects requests that carry more images than a model accepts,
counting the whole replayed history: most vision models refuse a ninth
image with HTTP 400, and DeepSeek V4.1 Flash answers an opaque HTTP 503
(code 3800) past 20. A screenshot-heavy session then fails on every turn.
For the first-party Mistral destination, messagesToChatFormat now keeps
the newest image_url parts within the model's cap and replaces older ones
with a text note naming the cap, in the outbound copy only. The cap is the
new modelCapabilities.<id>.mistralMaxImages axis: an operator declaration
over the measured rows on the mistral registry entry, else the documented 8.
