Summary

When the web-search consumer paused, a buffered semantic event kept the upstream raw-byte inactivity timer running and could incorrectly fail a healthy stream. Pause and resume the remaining upstream inactivity budget across the semantic handoff, without resetting it, and separately bound a stalled consumer so the original reader is cancelled. Keep the separate post-terminal drain deadline and identify consumer timeout distinctly from upstream inactivity. Synchronous empty chunks now yield to the event loop so they cannot starve the upstream inactivity timer.

Closes #8159

The shared structure/runtime.md contract is updated because it owns the web-search timeout behavior. The coordinator approved the scope expansion to src/images/loop.ts and its tests: image-loop consumer inactivity is now classified as 504 before generic provider-stream errors, matching the web-search loop. Prior art: the WHATWG Streams Standard describes demand, backpressure and cancellation; the HTML event-loop processing model describes microtask checkpoint processing.

Verification

  • Before the fix, PATH="$PWD/node_modules/.bin:$PATH" bun test tests/web-search/web-search-progress-stream.test.ts -t 'a paused consumer does not count as upstream byte inactivity' — exit 1, reproducing RoutedModelInactivityError.

  • Before the empty-chunk fix, PATH="$PWD/node_modules/.bin:$PATH" bun test tests/web-search/web-search-progress-stream.test.ts -t 'synchronous empty chunks cannot starve the inactivity deadline' — exit 1; the task-owned subprocess exceeded its five-second watchdog despite a 100 ms inactivity limit. After the fix this test passed in 139 ms, exit 0.

  • Before the image-loop fix, bun test tests/images/loop.test.ts -t 'a stalled search-stream consumer reports a timeout in the terminal' — exit 1: emitted status 502 and Provider stream error instead of status 504 and the original timeout message. After the fix the same command exited 0, 1 pass.

  • bun test --isolate tests/images/loop.test.ts tests/images/loop-reasoning-replay.test.ts tests/web-search/web-search-progress-stream.test.ts tests/web-search/web-search-timeout-contract.test.ts tests/web-search/web-search.test.ts — exit 0, 189 pass, 0 fail.

  • bun run typecheck — exit 0.

  • bun test tests/ci-workflows/file-size-ratchet.test.ts — exit 0, 9 pass.

  • bun run structure:check — exit 0.

  • bun run privacy:scan — exit 0.

  • bun run test:changed — exit 1, 19,814 pass, 39 skip, 23 fail, 9 errors across 795 files. The following failing files were each run alone with bun test --isolate <file> on head c4c03026431332c325d27207d5fd56f7d7a77c08 and clean origin/dev 3b23d46e7debb62800a4a4427c7fddef897288a4:

    File Head dev Assessment
tests/codex-integration/catalog-omit-disabled-native.test.ts exit 1, 5 fail exit 1, 1 fail Same first retained-sync timeout; head had further cascading timeouts.
tests/codex-integration/codex-config-write-lock.test.ts exit 1, 1 fail exit 1, 1 fail Same lock timeout on both.
tests/codex-integration/native-codex-toggle.test.ts initial exit 1, 8 fail; rerun exit 0, 14 pass exit 0, 14 pass Passed in isolation on head rerun. The same off/on case also timed out on both head and dev when run alone with -t 'the status read follows an off-then-on round trip against the same stale server config'; its codex --version child exceeded the test timeout.
    tests/codex-integration/native-ultrafast-picker.test.ts exit 1, 1 fail exit 1, 1 fail Same retained-sync timeout on both.
tests/config/settings-desktop-switch-apply.test.ts exit 0, 12 pass exit 0, 12 pass Passed in isolation on both.
tests/routing/combo-management-api.test.ts exit 0, 41 pass exit 0, 41 pass Passed in isolation on both.
tests/server/server-native-catalog-priority.test.ts exit 0, 7 pass exit 0, 7 pass Passed in isolation on both.

The changed suite remains failed; these comparisons do not certify it green. PR CI must pass before merge.

  • Full bun run test not run locally because six lanes run concurrently. Hosted CI covers the wider Ubuntu test shards; macOS and Windows are covered by maintainer batch policy.

Summary by CodeRabbit

  • Bug Fixes
    • Improved reliability of web-search progress streaming when processing pauses or incoming data stalls.
    • Streams that stop receiving events now time out, while pauses handing events to the consumer are handled separately.
    • Abandoned streams are bounded by a consumer timeout, reported as a gateway error for search and image requests.
    • Empty data chunks no longer incorrectly reset the upstream inactivity timer.

Walkthrough

The progress stream now distinguishes upstream byte inactivity from downstream consumer delays. It pauses the upstream inactivity budget during blocked event delivery and applies a separate consumer timeout. The search and image loops map consumer timeout errors to 504 responses.

Changes: Web-search progress stream

Layer / File(s) Summary
Stream deadlines and error handling
src/web-search/progress-stream.ts, tests/web-search/web-search-progress-stream.test.ts
The stream adds a configurable consumer timeout, defaulting to at least 300,000 ms. It pauses the remaining upstream inactivity budget while event delivery is blocked. Empty chunks do not renew that budget, and upstream inactivity tracking stops after a terminal event. Tests cover timeout behavior, cancellation, aborts, empty chunks, and terminal draining.
Loop timeout mapping and response validation
src/web-search/loop.ts, src/images/loop.ts, tests/images/loop.test.ts, tests/images/loop-reasoning-replay.test.ts, structure/runtime.md
The search and image loops convert consumer inactivity errors to 504 LoopErrors. An image-loop test checks the 504 error event and failed terminal SSE response. A mock preserves the progress-stream module exports, and the runtime documentation describes the timeout behavior.

Full details: Linked Issues check

Explanation

[#8159] src/web-search/progress-stream.ts pauses the remaining upstream inactivity budget during semantic handoff and resumes it without resetting it. Nonempty bytes reset the budget; synchronous empty chunks yield to the event loop. A separate consumer timeout cancels the original reader, and post-terminal draining keeps its own deadline. tests/web-search/web-search-progress-stream.test.ts covers these cases, including cancellation. The author reports that focused tests passed. The changed-test suite remains failed; the reported comparisons do not establish that it passed.

Full details: Out of Scope Changes check

Explanation

[#8159] The progress-stream changes, web-search timeout mapping, regression tests, and structure/runtime.md update support the issue. src/images/loop.ts maps the same consumer timeout to 504, and its tests cover that mapping and preserve the new export in mocks. These changes support the shared stream behavior. The whole-PR summary shows no unrelated changes.

Add a remaining-budget regression for blocked handoffs.

In tests/web-search/web-search-progress-stream.test.ts, consume part of the inactivity budget and confirm that a semantic delivery blocks. Then resume into a stalled upstream read and assert that the remaining budget expires before a fresh budget would. The current tests could miss a reset specific to blocked deliveries.

Bound the wait for the consumer’s next request

In src/web-search/progress-stream.ts:256-270, the consumer timer ends when an event is delivered. If the consumer never requests another event, the parser can keep reading SSE comments without another handoff. Each comment resets upstream inactivity, so the reader can remain open indefinitely.

Start a consumer deadline when the generator yields an event, and clear it when the consumer requests the next event. Keep the existing bound for pending handoffs. Add a regression test with recurring SSE comments after the first delivered event.

Maintainer integration into dev under MAINTAINERS.md (lidge-jun, admin).

  • Checked head: c4c0302; all 30 PR checks on this head passed (the test 4/4 cancelled at the 15-minute job deadline in attempt 1 was rerun on the same head and passed); no unresolved review threads.
  • Review: a paused downstream consumer no longer counts against the upstream byte-inactivity timer; it has its own deadline (default 300 s) and surfaces as 504 in both the web-search and image loops (CodeRabbit finding fixed with a red-to-green regression). Synchronous empty-chunk streams yield to the event loop so the inactivity timer still fires.

* test(claude): cover generic overflow parity and document direct errors

Preserve the existing direct-encoder fix and cover four generic error codes across streaming and folded responses.

Verified tree matches local commit 25f1104c7631e713199de631505c1dba01ff49d3.

* test(messages): align generic overflow normalization expectations

* fix(responses): lower ocx envelopes on native v1 compact (lidge-jun#8042) (lidge-jun#8151)

Canonical ChatGPT and official OpenAI native /responses/compact sends
forwarded OpenCodex-owned ocx1:/ocx2: compaction envelopes verbatim, so the
upstream rejected them as invalid_encrypted_content. Only the custom gateway
send lowered them. Run scrubOcxCompactionItems on the non-gateway send as
well, so the envelopes become plain user summaries like on ordinary turns,
while native blobs stay byte-identical for these decoding destinations.

* fix(codex): preserve pre-output hold across safe WS heartbeats

* test(codex): cover heartbeat cancellation and sequence bounds

* test(codex): assert the relayed refusal frame reaches the body

The parameterised "relayed in the body" cases only checked the status, so a
regression that dropped the frame would still pass. Assert the event marker and
message, and say in the transport contract that sequence_number is also bounded
to a non-negative safe integer.

* fix(terminal-guard): pass completion summaries that follow tool results (lidge-jun#8154)

* fix(terminal-guard): pass completion summaries that follow tool results

The Anthropic terminal guard read the latest user text and ignored tool
activity after it, so a normal turn (request -> tool calls -> tool
results -> short "done" summary) was classified suspicious_no_tool. The
relay then appended the plan-only nudge and re-asked the model, which
re-ran tools it had already run and repeated the cycle on every client
turn.

Tool calls or results after the latest user message now mark the turn as
already executed; only a renewed plan announcement (PLAN_INTENT_RE) is
still continued.

* fix(terminal-guard): stop the tool-activity pass at a stalled plan announcement

A text-only plan announcement after the last tool activity means the turn
stalled there, so a following completion is continued again, matching
recentTurnHasToolActivity. Pin the boundary for next-step wording: first-person
plans still continue, while a summary that suggests a next step to the user
passes.

* fix(mistral): keep replayed images within each model's per-request cap (lidge-jun#8174)

Mistral rejects requests that carry more images than a model accepts,
counting the whole replayed history: most vision models refuse a ninth
image with HTTP 400, and DeepSeek V4.1 Flash answers an opaque HTTP 503
(code 3800) past 20. A screenshot-heavy session then fails on every turn.

For the first-party Mistral destination, messagesToChatFormat now keeps
the newest image_url parts within the model's cap and replaces older ones
with a text note naming the cap, in the outbound copy only. The cap is the
new modelCapabilities.<id>.mistralMaxImages axis: an operator declaration
over the measured rows on the mistral registry entry, else the documented 8.

* fix(openai-chat): flatten Claude gateway root tool schemas

Apply the supplied Claude Chat gateway compatibility patch to current dev.
Flatten root oneOf/anyOf/allOf by upstream model id while preserving nested
schemas and literal values. Keep existing Zen and Azure sanitization intact.

Cover root compositions, required fields, wire-model overrides, non-Claude
exclusion, nested schema preservation and input immutability. Register the
new suite in the existing providers layout shard and document the lossy
root normalization contract.

* fix(openai-chat): preserve Claude root reference arguments

Preserve own __proto__ argument names and resolve local root-branch ref
chains with a shared 64-dereference budget per tool schema. Keep nested
schemas and existing Zen/Azure handling unchanged. Preserve the original
composition for cyclic, missing, external, over-budget or nested-composition
references instead of emitting an incomplete flattened argument catalog.

Add regression coverage for local references, safety fallbacks, property
precedence and non-final Claude name segments. Clarify allOf required-field
promotion and reference limits in the public and architecture docs.
