fix(terminal-guard): pass completion summaries that follow tool results - #8154
The Anthropic terminal guard (src/server/responses/terminal-guard.ts) misclassifies an ordinary completed agent turn as a plan-only stop.
analyzeTerminalTurn reads the latest user text and looks at tool activity only before that message, and only for an explicit "continue". In a normal agent turn the messages after the user request are tool calls and toolResults. Example: user "Create hello.txt containing hi, then reply DONE", then exec_command, then exit 0, then the assistant says "DONE". The latest user text is still the original actionable request, the final text is short and matches PLAN_OR_COMPLETION_RE ("done", "fixed", "I've updated ..."), and the turn is classified suspicious_no_tool.
The relay then appends TERMINAL_GUARD_NUDGE ("你刚才只描述了计划,没有执行任何工具…") and re-asks the model in the same response. The model has already run the tools, so it answers the accusation ("我其实已经调用过工具了…") and re-runs them to "verify". The client receives both legs merged into one message. Every later client turn ends with a short summary again, so the cycle repeats. In agent harnesses this shows up as runaway duplicate tool calls.
Observed through a hub on 2.88.0, with Claude OAuth (anthropic/claude-sonnet-5-5, claude-opus-5-5, claude-opus-4-7, claude-haiku-4-5):
Raw /v1/messages with one tool round trip in history: every response carried the extra nudged leg (25/25, on both /v1/messages and /v1/responses). It made no difference whether the tool name was prefixed, thinking was on or off, or ids/text were unique.
Codex CLI / Claude Code / another agent CLI asked to "create hello.txt containing hi, then reply DONE": 30 / 13 / 25 tool calls, with the model itself reporting garbled history.
Capturing the outbound body with a fake upstream showed two upstream calls for one client request; the second ends with the nudge as a user message.
Fix
Tool calls or results after the latest user message mean the turn already executed tools. A text-only completion summary in that turn now passes with reason recent_tool_activity. A renewed plan announcement (PLAN_INTENT_RE: "Let me …", "I'll …", "Next I …", "我接下来…", "下一步…") after tool results is still continued, so a model that stalls mid-task is still nudged. A text-only plan announcement that sits after the last tool activity in history also stops the pass, as in recentTurnHasToolActivity. The explicit-continue path and turns with no tool activity are unchanged.
Subject-less next-step wording ("Done. Next, run bun test locally to verify.") is deliberately left passing: in a closing summary it is usually advice to the user, and nudging it brings back the duplicate tool calls this PR fixes. A test pins that boundary.

The plan-announcement and user-directed next-step tests pin the boundary in both directions.

bun run typecheck: no type errors. bun run privacy:scan: passed. bun scripts/file-size-ratchet.ts: passed.
Applied the first commit (ef993d9c3) to a running 2.88.0 hub and restarted it:
The raw /v1/messages reproduction went from 25/25 nudged responses to 0/15 (sonnet-5-5, opus-5-5, opus-4-7).
/v1/responses returns a single DONE message.
The three agent CLIs complete the hello.txt task in 2 / 1 / 1 tool calls.
The follow-up commit only narrows the pass for histories that end in a text-only plan announcement; those reproductions end in a tool result, so it was not re-run on the hub.

Summary by CodeRabbit
Bug Fixes
Terminal-turn checks now recognize tool activity within the current turn when assessing completion summaries. Summaries after tool results can proceed, including those that suggest a user follow-up. Plan announcements that stall after tool activity continue to be flagged, while a later completion remains eligible to proceed. Tool activity occurring after a plan announcement also allows the subsequent completion to pass.
The Anthropic terminal guard read the latest user text and ignored tool
activity after it, so a normal turn (request -> tool calls -> tool
results -> short "done" summary) was classified suspicious_no_tool. The
relay then appended the plan-only nudge and re-asked the model, which
re-ran tools it had already run and repeated the cycle on every client
turn.
Tool calls or results after the latest user message now mark the turn as
already executed; only a renewed plan announcement (PLAN_INTENT_RE) is
still continued.
all PR quality gates passed; the review readiness checklist is complete.

Walkthrough
The terminal guard now checks for tool activity since the latest user message. It passes responses with reason recent_tool_activity when the current assistant text is not a plan announcement. Plan announcements continue through existing checks.

The PR adds tool-activity detection to analyzeTerminalTurn in src/server/responses/terminal-guard.ts so text-only completion summaries pass with reason recent_tool_activity. The added tests in tests/server/terminal-guard.test.ts cover the same behavior. The title is specific, concise, and free of file lists or vague terms. * fix(mistral): keep replayed images within each model's per-request cap (lidge-jun#8174)

counting the whole replayed history: most vision models refuse a ninth

For the first-party Mistral destination, messagesToChatFormat now keeps
the newest image_url parts within the model's cap and replaces older ones
with a text note naming the cap, in the outbound copy only. The cap is the
new modelCapabilities.<id>.mistralMaxImages axis: an operator declaration
over the measured rows on the mistral registry entry, else the documented 8.

Summary
The Anthropic terminal guard (
src/server/responses/terminal-guard.ts) misclassifies an ordinary completed agent turn as a plan-only stop.analyzeTerminalTurnreads the latest user text and looks at tool activity only before that message, and only for an explicit "continue". In a normal agent turn the messages after the user request are tool calls andtoolResults. Example: user "Create hello.txt containing hi, then reply DONE", thenexec_command, thenexit 0, then the assistant says "DONE". The latest user text is still the original actionable request, the final text is short and matchesPLAN_OR_COMPLETION_RE("done", "fixed", "I've updated ..."), and the turn is classifiedsuspicious_no_tool.The relay then appends
TERMINAL_GUARD_NUDGE("你刚才只描述了计划,没有执行任何工具…") and re-asks the model in the same response. The model has already run the tools, so it answers the accusation ("我其实已经调用过工具了…") and re-runs them to "verify". The client receives both legs merged into one message. Every later client turn ends with a short summary again, so the cycle repeats. In agent harnesses this shows up as runaway duplicate tool calls.Observed through a hub on 2.88.0, with Claude OAuth (
anthropic/claude-sonnet-5-5,claude-opus-5-5,claude-opus-4-7,claude-haiku-4-5):/v1/messageswith one tool round trip in history: every response carried the extra nudged leg (25/25, on both/v1/messagesand/v1/responses). It made no difference whether the tool name was prefixed, thinking was on or off, or ids/text were unique.Fix
Tool calls or results after the latest user message mean the turn already executed tools. A text-only completion summary in that turn now passes with reason
recent_tool_activity. A renewed plan announcement (PLAN_INTENT_RE: "Let me …", "I'll …", "Next I …", "我接下来…", "下一步…") after tool results is still continued, so a model that stalls mid-task is still nudged. A text-only plan announcement that sits after the last tool activity in history also stops the pass, as inrecentTurnHasToolActivity. The explicit-continue path and turns with no tool activity are unchanged.Subject-less next-step wording ("Done. Next, run
bun testlocally to verify.") is deliberately left passing: in a closing summary it is usually advice to the user, and nudging it brings back the duplicate tool calls this PR fixes. A test pins that boundary.Verification
