A busy thread made every connected client do repeated full work. Each burst of tool output re-counted the thread's whole history and re-sent an unchanged shell to every subscriber. Codex sent each growing reply twice. MCP waits polled SQL every 50 to 250 ms for up to an hour. Active tool labels repainted about 14 times a second. This is the wave 1 perf batch from the tech debt audit.

Fix

  • Shell reads (P1): run rows and per-run item counts are read only for threads that another thread forks from, the only place they are used. The item count groups on the covering (thread_id, run_id) index and looks up each run once, not once per item. Semantics are unchanged.
  • Unchanged shells (P2): a live shell update equal to the last one sent on that subscription, apart from updatedAt, is skipped. An unchanged shell is still sent once its last send is 5 s old, so the client cursor and updatedAt never lag further and resume replays stay short. On the client, an unchanged update keeps the existing thread object and list, and the notification coordinator skips thread objects it already saw.
  • Codex text once (P3): while streaming, Codex sends text only on the turn item. The message is sent once with the final text. Web and mobile already render streaming text from turn items. flushTurn completes every buffered item when a turn ends, interrupted or not, so the message always lands.
  • MCP waits (section 7): wait_for_thread and blocking delegate_task now wait on the thread event stream instead of polling. They subscribe only to the event types that can end the wait, so large transcript events never fill a stream buffer.
  • No repaint animations (P4): the active tool label is static text-foreground instead of the infinite live-tool-shine sweep. The running automation dot no longer pulses.
  • ChatComposer (section 10): the composer subscribes to thread shells only while the @ menu is open, and its menu memo now caches. Path search results were a new array on every render.

P2 counts a frame as changed when an event adds a turn item, changes a run, thread, request, plan, or user message, or changes the status of a command, tool, or subagent item. Codex turns with streaming output should save more than these mostly Claude runs.

The captures come from a worktree dev server seeded with a copy of real data. No turn was running, so the active state was applied to a real tool row in the browser. The images are cropped to that row.

Nothing was dropped. All items still applied on current main, and no open PR fixes them. #14859 and #14047 touch the same shell stream files for other reasons.

Not in this PR: the sidebar row subscriptions (wave 2), the mobile tool shimmer (React Native, not CSS), and the "Thinking" shimmer, which animates only transform on its own layer and does not repaint.

Testing

  • vp test run on ShellStream, ThreadManagementService, ProjectionStore, CodexAdapterV2, assistantStreaming, the MCP service and toolkit integration tests, the replay fixture suites, shellReducer and shell, ThreadNotificationCoordinator, and MessagesTimeline.
  • New tests: shell dedupe and resend, unchanged client shell stays the same object, Codex sends the message once, wait re-reads only on its own run update, and shell counts after a source run is rolled back.
  • tsc for server, web, and client-runtime. Targeted lint and the CI knip export check are clean.

Reviewed with sol-loop: 2 rounds with gpt-6.1-sol on default effort.

pingdotgg#15907 and is deleted whole (Kevin ruled it expendable). Carried pingdotgg#15315 is
closed upstream but kept: its held commands compose with pingdotgg#16204's PR-watch
holds. ThreadNotificationCoordinator gains awaitsAnswer so pingdotgg#15033's
unchanged-shell skip no longer closes standing answer toasts. Fork files move
from effect/unstable/* to effect/* for Effect 4.0.1.
